The prognostic significance of respiratory rate in patients with pneumonia: a retrospective analysis of data from 705,928 hospitalized patients in Germany from 2010-2012.

BACKGROUND: Measurement of the respiratory rate is an important instrument for assessing the severity of acute disease. The respiratory rate is often not measured in routine practice because its clinical utility is inadequately appreciated. In Germany, documentation of the respiratory rate is obligatory when a patient with pneumonia is hospitalized. This fact has enabled us to study the prognostic significance of the respiratory rate in reference to a large medical database. METHOD: We retrospectively analyzed data from the external quality-assurance program for community-acquired pneumonia f

ALS Cognitive Behavioral Screen-Phone Version (ALS-CBS™-PhV): norms, psychometrics, and diagnostics in an Italian population sample.

BACKGROUND: Up to 50% of motor neuron disease (MND) patients show neuropsychological deficits which negatively affect prognosis and care. However, disability-related logistical issues and uneven geographical coverage of healthcare services may prevent MND patients from accessing neuropsychological evaluations. This study thus aimed to standardize for the Italian population the ALS Cognitive Behavioral Screen-Phone Version (ALS-CBS™-PhV), an MND-specific, telephone-based screening for frontotemporal dysfunction. METHODS: The cognitive section of the ALS-CBS™-PhV, the Italian telephone-based Mi

Highly active antiretroviral therapy is necessary but not sufficient. A systematic review and meta-analysis of mortality incidence rates and predictors among HIV-infected adults receiving treatment in Ethiopia, a surrogate study for resource-poor settings.

BACKGROUND: Owing to the introduction of highly active antiretroviral therapy (HAART), the trajectory of mortality and morbidity associated with human immunodeficiency virus (HIV) infection has significantly decreased in developed countries. However, this remains a formidable public health challenge for people living with HIV in resource-poor settings. This study was undertaken to determine the pooled person-time incidence rate of mortality, analyze the trend, and identify predictors of survival among HIV-infected adults receiving HAART. METHODS: Quantitative studies were searched in PubMed,

Mortality and its predictors among adolescents and adults on antiretroviral therapy in the era of test-and-treat, Ethiopia: a retrospective cohort study.

BACKGROUND: HIV/AIDS remains a leading cause of morbidity and mortality in resource-limited settings. Although several studies have assessed mortality among patients on antiretroviral therapy (ART), evidence from Ethiopia during the test-and-treat era, where all diagnosed individuals start ART immediately regardless of clinical stage or CD4 count, is limited. METHODS: A retrospective cohort study was conducted at public hospitals in the West Arsi Zone, involving 446 adolescents and adults on ART between August 2016 and June 2019. Participants were selected by stratified random sampling at thre

Survival and predictors of mortality among people living with HIV (PLHIV) on antiretroviral therapy in Ethiopia (2016-2023): a retrospective cohort study.

OBJECTIVE: Despite global progress in reducing HIV-related morbidity and mortality, preventable deaths persist, particularly in sub-Saharan Africa. Ethiopia has achieved near-universal antiretroviral therapy (ART) coverage, yet survival outcomes and predictors of mortality in the era of 'test and treat' remain underexplored. This study examined survival rates and predictors of mortality among people living with HIV (PLHIV) on ART in Ethiopia between 2016 and 2023. DESIGN: We conducted a retrospective open cohort study. SETTING: 65 public health facilities across Ethiopia. Kaplan-Meier surviv

Postoperative rehabilitation exercise experiences of geriatric patients with femoral neck fractures based on the perspective of medical staff: a qualitative study.

BACKGROUND: The incidence of femoral neck fractures in older adults is increasing each year. Active and reasonable postoperative rehabilitation exercises can restore the activity of geriatric patients with femoral neck fractures to a great extent, while also avoiding bedridden complications and re-fractures. This study explores the perspectives, experiences, and recommendations of medical staff regarding the implementation status, existing problems, promoting factors, and hindering factors of post-surgical rehabilitation exercises for geriatric patients with femoral neck fractures. The ultimat

Preliminary Exploration of CBCT in IVCFT Patient Filter Retrieval.

BACKGROUND: Inferior vena cava filter related thrombosis (IVCFT) poses significant challenges in patients undergoing mechanical thromboembolic prophylaxis, often leading to filter retrieval failure and recurrent pulmonary embolism. Current diagnostic modalities, including ultrasound, CT venography, and digital subtraction angiography (DSA), suffer from limitations such as operator dependency, ferromagnetic interference, patient transfer risks, and suboptimal multiplanar reconstruction capabilities. Cone-beam computed tomography (CBCT), offering real-time three-dimensional vascular imaging with

Is "More" Always Better than "Less"? Deprescribing to Improve Patients' Clinical Outcomes: A Case Series.

INTRODUCTION: Polypharmacy represents a significant clinical concern, particularly among elderly patients, due to the increased risk of adverse drug reactions (ADRs). This population is often affected by multiple comorbidities and is typically followed by various specialists, which can lead to the concurrent use of multiple medications, commonly defined as polypharmacy when five or more drugs are prescribed. The use of numerous medications heightens the potential for drug-drug interactions (DDIs), thereby increasing the likelihood of ADRs. Deprescribing has emerged as a potential strategy to m

Sensitivity and specificity of the Bamberg Dementia Screening Test's (BDST) full and short versions: brief screening instruments for geriatric patients that are suitable for infectious environments.

BACKGROUND: Currently, many patients suffering from dementia do not have a diagnosis when admitted to geriatric hospitals. This is the case despite an increased risk of complications affecting the length of stay and outcome. Unfortunately, many dementia screening tests cannot be used on geriatric inpatients, who are often bedridden. Therefore, we aimed at evaluating the diagnostic accuracy of a small battery of bedside tasks that require minimal vision and fine motor skills in patients with suspected dementia. METHODS: In this prospective study, the Bamberg Dementia Screening Test (BDST) was

A Case of Headache Treated by Online Telemedicine in Collaboration With a Midwifery Home.

Midwifery centers are places where midwives not only provide antenatal checkups and delivery care but also offer a wide range of health guidance to pregnant women, postpartum mothers, newborns, and older women. In recent years, midwives have also provided onsite and online health guidance. However, diagnosis and prescribing medication are impossible in midwifery centers because no doctor is present. If the midwife determines that the patient should consult doctors, the patient may have to go to a hospital and see doctors in person, which can be burdensome. Online telemedicine facilitates midwi
