BACKGROUND: Intravitreal bevacizumab has been used off-label to treat multiple ocular conditions for almost two decades. Reference bevacizumab, Avastin, was discontinued in Australia in 2021. A bevacizumab biosimilar, MVASI, was approved by the Therapeutic Goods Administration for the treatment of metastatic cancer in 2020. PURPOSE: To evaluate the outcomes of intravitreal injection of bevacizumab (IVB) for retinopathy of prematurity (ROP).
METHODS: IVB was selected to be the first treatment for type 1 ROP in 8 eyes (4 patients). Bevacizumab (0.25 mg/eye) was injected into the vitreous cavity under either general anesthesia or sedation. Fundus photography and fluorescein angiography were performed before the IVB. CLINICAL CASE: Female in her sixth month of pregnancy, with a history of refractive surgery, who consulted due to a metamorphopsia and decrease in visual acuity in the right eye. A presumed ocular histoplasmosis syndrome (POHS) and a subfoveal choroidal neovascular membrane were observed in the fundoscopy. METHODS: A patient with an inflamed nasal primary pterygium, refractory to artificial tears and naphazoline, was enrolled in this study. Multifocal lymphangioendotheliomatosis with thrombocytopenia (MLT) is a rare disorder characterized by a proliferation of cutaneous and gastrointestinal (GI) lesions that have characteristics of both lymphatic and vascular lesions. Thrombocytopenia is associated with this syndrome and is thought to represent platelet destruction within the lesions. The natural history is one of multiple, life-threatening episodes of GI bleeding. We report the successful use of bevacizumab in this disease, an antibody to the vascular endothelial growth factor.
