[Internal biliary fistulas of cholelithic origin].

The investigations performed have established that the cause of formation of internal biliary fistulas in 162 patents was cholelithiasis in combination with obstruction of the bile tracts, purulent cholelithiasis, cholangiolitic abscesses of the liver, pancreatitis and cholelithic ileus. The operation of choice is thought to be cholecystectomy, removal of the stones from the bile ducts, reestablishment of the major bile outflow, liquidation of the pathological anastomosis and suturing the hollow organ defect. Fibrogastroduodenoscopy, retrograde cholangiopancreatography and percutaneous transhe…

Endoscopic treatment of postoperative biliary fistulae.

Postoperative biliary fistulae are difficult to manage, particularly in the face of obstruction or malignancy. We used endoscopic sphincterotomy or endoprosthesis placement to aide fistula closure in 52 patients with postoperative biliary fistulae. Thirty-seven patients with a fistula were treated with endoscopic sphincterotomy alone. Twenty-four of these 37 patients had a history of lithiasis; 21 patients were treated successfully by endoscopic sphincterotomy alone. The fistula closed in 2.4 +/- 1.6 days. Among the other 13 patients without history of stone disease, the fistula closed in seve…

Endoscopic treatment of biliary tract fistulas.

Endoscopic therapy was attempted in 24 patients with spontaneous or postoperative persistent biliary fistulas. Endoscopic retrograde cholangiography demonstrated the site of the fistula in 22 cases. Sphincterotomy or biliary stent placement resulted in rapid resolution of the fistula in 16 of 24 patients. Failures were attributed to exclusion of the injured intrahepatic bile duct in two cases, insufficient dilation of a bile duct stricture in one, the large size of the bile duct defect in two, and associated lesions in three (cirrhosis, arterial trauma, subhepatic abscess). Endoscopic manageme…

Treatment of iatrogenic biliary obstruction by balloon dilatation of a biliary jejunal fistula.

Biliary obstruction and multiple hepatic abscesses occurred in a patient after ligation of a segmental branch of the right hepatic duct. The patient was successfully managed by transhepatic biliary drainage and balloon dilatation of an internal fistula that developed between the ligated duct and a Roux limb of jejunum. Internal biliary fistulas may be dilated using interventional radiologic techniques to permit nonobstructed bile flow. Implications for the nonsurgical treatment of biliary strictures are discussed.

[Colovesicular fistula after migration of a biliary stent].

BACKGROUND: In 5% of endoscopically inserted biliary stents distal migration occurs. Mostly the endoprostheses pass the intestine spontaneously. In some patients the stent does not pass and causes complications such as perforation, fistulae and abscess formation. CASE STUDY: A 76-year-old woman with choledocholithiasis received endoscopic retrograde cholangiopancreatography (ERCP). A stone was crushed and a plastic endoprosthesis inserted. After six months the patient returned with relapsing urinary tract infections. A CT scan showed the stent perforating the sigmoid and stuck in the bladder.

[Diagnosis and surgical treatment of internal biliary fistula].

Results of examination and surgical treatment of 172 patients with internal biliary fistula (IBF) are analyzed. IBF was revealed in 0.22% of hospitalized patients with cholelithiasis. There were 4 variants of fistula formation. Combined spontaneous multiple fistula were described for the first time in 8 patients. Based on assessment of IBF's clinical symptoms formula for prediction of IBF development was proposed. Diagnostic potential of instrumental, X-ray, endoscopic and ultrasonic examinations was assessed. Differential surgical policy depending on IBF variant is described.

Radiological findings in biliary fistula and gallstone ileus.

PURPOSE: Biliary fistula and gallstone ileus are rarely found. The diagnosis is difficult and may be delayed until operation. We reviewed the radiological findings in a retrospective material. MATERIAL AND METHODS: The cases of 16 patients treated for biliary fistula were analyzed with respect to findings at imaging. Ten patients had a spontaneous fistula. Nine of them had an internal bilioduodenal fistula and one had an external fistula with stones passing through a subcutaneous abscess. Five patients also had gallstone ileus and one patient a rare gastric outlet obstruction caused by a gall…
