Isotretinoin and night blindness.

Isotretinoin is an effective and increasingly popular treatment for acne vulgaris. There have been reports of night blindness as a side-effect of treatment although the evidence does not demonstrate a clear causal relationship between isotretinoin therapy and the risk of night blindness. Nevertheless, considering the lack of evidence in this area, it is important to educate patients about this potential consequence, which may become longstanding and even irreversible, and encourage them to promptly report changes in their night vision.

Blindness, weakness, and tingling.

A previously healthy 50-year-old man complained of acute bilateral loss of vision over 24 hours associated with tingling of his hands and feet. During the first 48 hours of hospitalization, he evolved quadriplegia, facial diplegia, bulbar palsy, and respiratory distress requiring ventilator assistance. At the same time vision deteriorated to hand motions. Lumbar puncture, electromyography, and clinical findings were all consistent with Guillain-Barré syndrome. The diagnosis of concomitant bilateral optic neuritis was made. Tests for immunological, inflammatory, infectious, or genetic diseases

Spotlight on childhood blindness.

AIM: Regular vaccination against mumps resulted in a significant reduction in epidemic mumps in the Czech Republic. However, mumps cases have recently shown an upward trend, even in the vaccinated population where a considerable proportion of cases have occurred. The aim of this study was to find out, by mumps virus IgG antibody avidity testing, whether the high incidence of mumps in the vaccinated population is a result of primary or secondary vaccine failure and whether the vaccinated differ from the naturally immunised in anamnestic antibody avidity. Given the problematic laboratory diagnos

[Congenital stationary night blindness].

Authors present diagnostic procedures and dinical features of the congenital stationary night blindness (CSNB) in 19 patients. Effective therapy of the concomitant ophthalmological pathologies usually improves patient's vision ability.

HIV/AIDS and blindness.

Nearly 34 million people are currently living with HIV/AIDS: ocular complications are common, affecting 50% to 75% of all such patients at s … viral therapy makes its way to these countries, however, both life expectancy and the prevalence of blindness related to HIV/AIDS can be expected to increase dramatically.

Blindness following gastrointestinal haemorrhage.

Loss of vision is a rare but well known complication of distant and recurrent haemorrhage. It shares a poor prognosis, with only 10-14% of cases likely to make a complete recovery. Visual symptoms, due to ischaemic anterior optic neuropathy, vary from blurred vision to complete loss of vision in one or both eyes. The pathogenesis of such ischaemia remains unclear. Gastrointestinal bleeding seems to be the leading cause of loss of vision secondary to haemorrhage. However, complete and permanent blindness following gastrointestinal bleeding has rarely been reported. We report the case of a 51 -y…

Autism and congenital blindness.

The nature of autism in congenitally blind children has long been a source of interest and perplexity. A group of nine congenitally blind children with an autism-like syndrome were closely matched on chronological age and verbal mental age with nine sighted autistic children, and were compared on the Childhood Autism Rating Scale (Schopler, Reichler, & Renner, 1986) and the Behavior Checklist for Disordered Preschoolers, together with the Play Items for Disordered Preschoolers (Sherman, Shapiro, & Glassman, 1983). A checklist of clinical features characteristic of autism (derived from DSM-III-

Blindness after facial trauma.

Blindness after facial fractures has been reported to occur with an incidence that ranges between 0.67% and 3% depending on the reporting institution. To verify this finding we undertook a retrospective chart review of 5936 patients with facial fractures that occurred over a 12 1/2-year period. We found that vision in 19 eyes were lost in 18 patients. Vision loss was more frequently encountered in Le Fort III level fractures (2.2%) followed distantly by Le Fort II level fractures (0.64%), and zygomatic fractures (0.45%). The cause of blindness was most frequently associated with motor vehicle …

Global data on blindness.

Globally, it is estimated that there are 38 million persons who are blind. Moreover, a further 110 million people have low vision and are at great risk of becoming blind. The main causes of blindness and low vision are cataract, trachoma, glaucoma, onchocerciasis, and xerophthalmia; however, insufficient data on blindness from causes such as diabetic retinopathy and age-related macular degeneration preclude specific estimations of their global prevalence. The age-specific prevalences of the major causes of blindness that are related to age indicate that the trend will be for an increase in suc…

Blindness in the tropics.

Blindness is a major problem in most developing countries. It occurs at ten times the rate seen in the developed countries and in over 80% of cases is either preventable or curable. The four main causes are cataract, trachoma, onchocerciasis and xerophthalmia. Cataract, which is responsible for half the blindness, can be effectively cured with modern cataract surgery. Trachomatous blindness follows frequent episodes of reinfection, which can be prevented by simple hygienic measures. Onchocerciasis can be halted by the simple administration of a once-a-year dose of ivermectin, a drug which is c…

Interferon-related cortical blindness.

The term cortical blindness indicates loss of sight due to bilateral lesions in the occipital lobes. It is a rare but severe side effect produced by chemotherapeutic agents. Cortical blindness was diagnosed in a 75-year-old man who had been treated with alpha-interferon for metastatic renal-cell carcinoma. The absence of focal neurological signs and of abnormal findings as determined by two repeated computed tomography (CT) scans of the brain, which excluded structural damage to the occipital lobes, suggest that metabolic or toxic reactions may have caused the cortical blindness diagnosed in o…

Childhood blindness: dateline Africa.

Prevalence rates of childhood blindness and infant and childhood mortality in Africa are the highest in the world. Major factors contributing to these high rates are similar for both blindness and mortality: malnutrition, measles and other acute febrile infections, malnutrition with xerophthalmia, and limited or poor access to preventive and curative health services. Because of high national growth rates, stagnant economies, and the failure of national health care systems to expand with rapidly growing populations, childhood blindness in Africa likely will remain a significant international pu…

Childhood blindness in Jamaica.

Examinations were performed on the 108 blind Jamaican children (VA less than 6/60 in the better eye) in residential care. The congenital rubella syndrome (CRS) was the leading preventable cause of childhood blindness, accounting for 22% of children examined. Improvement of the rubella immunisation programme and the introduction of appropriate surgical procedures constitute recent attempts to combat childhood blindness.

Congenital stationary night blindness.

Congenital stationary night blindness (CSNB) seems to be a very rare condition in Scandinavia. From Denmark a 7-generation family with the dominant form was published in 1909, and one family with the X-linked recessive form was reported from Norway. On going through the files of the National Eye Clinic for Visually Impaired, 7 patients were found (1 dominant, 4 X-linked recessive, 1 simplex case and 1 autosomal recessive). Including anamnestic information on relatives, 17 patients had a diagnosis of CSNB. The clinical findings in these cases are reported with stress on alteration in ERG, dark …
