Fracture-Related Infection Diagnostic Tools in the Upper Extremity: A Scoping Review.

Fracture-related infection (FRI) is a serious orthopaedic complication and its diagnosis, particularly in the upper extremity, is difficult and poorly defined in current literature. An international consensus definition of FRI was published in 2018, and our scoping review aims to investigate FRI diagnostic tools reported in the primary literature and their biostatistical utility. A review of articles generated from the PubMed/NCBI search term "fracture-related infection" was undertaken using PRISMA methodology. The included studies were published from January 2018 to June 2022 and referred to

Autosomal dominant ectrodactyly and absence of long bones of upper or lower limbs: further clinical delineation.

The association of ectrodactyly and absence of long bones of the upper or lower extremities has been recognized previously in nine families. We report 24 additional individuals in four families who are similarly affected. Two of these families manifest the unusual feature of unilateral preaxial polydactyly of a lower extremity. Data from these four families plus the nine previously reported suggest that ectrodactyly associated with absence of long bones of the upper or lower limbs is a genetically determined disorder, inherited as an autosomal dominant trait with widely variable expression or

Skeletal overgrowth with modelling error in neurofibromatosis.

Ten cases of neurofibromatosis, nine involving the lower extremity and one, the upper extremity, were analysed for growth and modelling errors. The elongated bones showed over-tubulation or under-tubulation. Long as well as short bones were affected. Modelling errors mostly affected the metaphysis but, in a few cases, especially where there was isolated involvement of the fibula, the diaphysis was also affected. In two patients the whole length of paired bones of the leg showed evidence of over-tubulation. The rule of polarity which influences growth and modelling was not applicable to these c

Geographic variation in gorilla limb bones.

Gorilla systematics has received increased attention over recent decades from primatologists, conservationists, and paleontologists. Studies of geographic variation in DNA, skulls, and teeth have led to new taxonomic proposals, such as recognition of two gorilla species, Gorilla gorilla (western gorilla) and Gorilla beringei (eastern gorilla). Postcranial differences between mountain gorillas (G. beringei beringei) and western lowland gorillas (G. g. gorilla) have a long history of study, but differences between the limb bones of the eastern and western species have not yet been examined with

Scaling effect on the mid-diaphysis properties of long bones-the case of the Cervidae (deer).

How skeletal elements scale to size is a fundamental question in biology. While the external shape of long bones was intensively studied, an important component of their organization is also found in their less accessible inner structure. Here, we studied mid-diaphyseal properties of limb long bones, characterizing notably the thickness of their cortices (bone walls), in order to test whether body size directly influences bone inner organization. Previous examinations of scaling in long bones used broad samplings to encompass a wide range of body sizes. To account for the effect of confounding

Reconstruction of the Shoulder and Humerus in Metastatic Bone Disease.

Metastatic disease involving the skeleton most often affects the spine, pelvis, and proximal long bones of the extremities. In the upper extremity, the proximal humerus is frequently affected. Although many of the principles of managing metastatic bone disease are similar, regardless of the metastatic site, the upper extremity has some unique anatomic and functional traits that warrant consideration when making management decisions. The main anatomic differences from the lower extremity pertain to the smaller bone size, rotator cuff function, and proximity to major neurologic structures, and t

Pediatric upper-extremity fractures.

Upper-extremity fractures account for more than half of childhood bony injuries. The frequency of injury increases with increasing mobility. The most common mechanism is a fall on an outstretched hand while playing. Optimal management requires knowledge of the normal anatomy and variants unique to pediatric bones. The physician needs to maintain a high level of suspicion for growth plate injuries because if unrecognized, these may result in growth arrest. Although the vast majority of pediatric upper-extremity fractures will heal rapidly with minimal intervention, physicians should be aware of

Massive Upper Extremity Hemihyperplasia Secondary to Neurofibromatosis.

CASE: A 30-year-old woman with type 1 neurofibromatosis presented with enlargement of the right upper extremity. The condition began in childhood and progressed until the limb reached a nonfunctional and burdensome size from elbow to fingertips. Radiographs showed elongated, thinned bones, and CT demonstrated diffuse soft tissue infiltration. Above-elbow amputation was performed. Pathology revealed diffuse neurofibromatosis of skin, subcutaneous tissues, and muscle without malignant change. Postoperatively, she was fitted with an aesthetic prosthesis and reported marked physical and emotional

Preiser's disease or avascular osteonecrosis of the scaphoid: An updated literature review.

Avascular necrosis of the scaphoid, often referred to as Preiser's disease, is little known despite the scaphoid being the second most frequently involved location in avascular pathology of the carpal bones after the lunate (Kienböck's disease). Nonetheless, very few cases have been reported in the medical literature over the last century. Its pathophysiology is not completely elucidated although the unique vascularization of the scaphoid and several risk factors seem to be recurrently incriminated. Two new classifications based on modern imaging have appeared in the last decade and are now co

Approaches for ultrasonographic evaluation of long bones in the dog.

The objective of this study was to identify access portals for ultrasonographic evaluation of canine long bones (humerus, radius, ulna, femur, and tibia). A 7.5 MHz linear transducer was used in five medium-sized dogs (range of 25-35 kg body weight). Good approaches could be identified for all long bones. For the proximal humerus, a cranial portal, and the distal humerus, a lateral portal is suggested. For the radius and ulna, a craniolateral approach seemed to be the best. In the hindlimb, a medial approach for the femur and a craniolateral approach for the tibia were the most effective appro

[Basic principles of surgical treatment of bone metastases].

Due to an increased life expectancy and improved oncological treatment, the necessity for surgical treatment of bone metastases has increased as well. Ideally, therapy should concentrate on the improvement of the patient's quality of life while preferably being interdisciplinary. The type and extent of surgical therapy depend on the prognosis, tumor entity, localization of the tumor as well as on the patient's general condition. The main aims of surgical treatment is to reduce pain and preserve or restore function and mobility. After the contribution on spinal metastases in the last volume, th

New evidence from China for the nature of the pterosaur evolutionary transition.

Pterosaurs are extinct flying reptiles, the first vertebrates to achieve powered flight. Our understanding of the evolutionary transition between basal, predominantly long-tailed forms to derived short-tailed pterodactyloids remained poor until the discovery of Wukongopterus and Darwinopterus in western Liaoning, China. In this paper we report on a new genus and species, Douzhanopterus zhengi, that has a reduced tail, 173% the length of the humerus, and a reduced fifth pedal digit, whose first phalange is ca. 20% the length of metatarsal III, both unique characters to Monofenestra. The morphol

Differential diagnosis of pathologically induced upper and lower limb asymmetry in a burial from late medieval Ireland (CE 1439-1511).

This paper reports a case study from 15 to 16th century Ireland of a mature adult male with upper and lower limb asymmetry. All of the bones of the right upper and lower extremity were atrophied and demonstrated limb shortening. This individual also had severe scoliosis and accessory facets on the superior aspect of the distal ends of the right metatarsals. These facets were most likely caused by the individual walking predominately on his toes to make up for the height difference between the lower limbs. Based on the patterning of affected bones and associated pathological changes, the indivi
