Sleep problems are very common among psychiatric patients. Borderline personality disorder, as a common and severe mental disorder, is associated with different types of sleep disturbances, such as disturbances of sleep continuity, altered REM sleep regulation and nightmares. These disturbances are the result of interaction of the personality traits, concomitant and comorbid diseases and environmental factors. Despite the high prevalence of sleep related disorders in BPD patients, this aspect of BPD is still neglected in clinical and research settings. To date there has been little agreement o…

OBJECTIVE: This study examines Axis I comorbidity in patients with borderline personality disorder as compared with patients with other personality disorders and to patients without personality disorders.
METHODS: A total of 235 clinical histories were evaluated. These were divided into three groups: 92 pertaining to patients with borderline personality disorder, 69 to ones having other personality disorders and 74 to patients without personality disorders. For purposes of statistical analysis, ANOVA and chi-square were employed.
RESULTS: There were 89 (96.7%) patients with borderline person…

OBJECTIVES: Extended mind theory, a recent development in the field of cognitive philosophy, suggests that the information and processes contained in external objects such as notebooks and computers can be considered as much a part of a person's mind as the processes contained within the brain itself. This paper aims to outline extended mind theory, and to explore how it might be applied to an understanding of psychiatric conditions such as borderline personality disorder (BPD).
CONCLUSION: Those with BPD have a deficit in their ability to perform certain high-level cognitive tasks, such as r…

Borderline personality disorder (BPD) is characterized by interpersonal disturbances, but the neurocognitive aspects of these symptoms are poorly understood. We hypothesized that patients with BPD have impaired perception of emotional expressions, which are related to symptoms of interpersonal dysfunction. To control potential confounding factors, this study excluded subjects with comorbid diagnoses known to be associated with impaired affect perception. We tested 43 outpatients with BPD and 26 healthy controls on emotion recognition tasks (facial, prosodic, and integrated facial/prosodic), no…

The frequent occurrence of depressive symptoms in patients with borderline personality disorder has generated considerable interest in the nature of the relationship between borderline personality disorder and the depressive disorders. Data from the perspectives of phenomenology, biology, family history, course of illness, comorbidity patterns, and treatment response have been brought to bear on the question. Reviews based on research available by 1985 and 1991, respectively, arrived at differing conclusions: (1) that both disorders shared common but non-specific sources, and (2) that the two …

This paper reviews the literature concerning the phenomenology and duration of psychosis in borderline patients and presents empirical findings concerning a sample of eight borderline women with psychosis. Little work has yet been done to establish the duration of so called "transient psychotic episodes," but the clinical consensus up to now has been that such episodes last less than two weeks. The patients in the current study experienced psychotic symptoms lasting between three weeks and four months. They were characterized by hysterical features, visual disturbances, and a possible history

Borderline personality disorder (BPD) is a clinically significant and extensively studied personality disorder (PD), associated with substantial impairment, risk, treatment needs, and health-system burden. Contemporary debates about its classification now occur within diagnostic systems that already move beyond purely categorical models, including ICD-11 and the DSM-5 Alternative Model for Personality Disorders. The central question is therefore not simply whether dimensional assessment should be incorporated into PD diagnosis, but how general personality disorder diagnosis, named categorical …

Caregivers of young people with borderline personality disorder (BPD) or BPD features experience significant burden and distress and often lack effective coping strategies. A family environment of pervasive invalidation can contribute to the disorder and work against effective coping. Consequently, some psychotherapy interventions for young people with BPD or BPD features aim to incorporate caregivers in treatment to varying degrees. This review synthesised results of existing studies that included caregivers in psychotherapy alongside their young person and that examined caregiver outcomes us…

Borderline personality disorder (BPD) is commonly comorbid with chronic pain and associated with pain symptoms and interference. BPD features are associated with negative affect, which is associated with pain catastrophizing and/or pain anxiety, and finally pain severity or interference. We extended models of the BPD-pain associations in a chronic pain sample (N = 202), highlighting the role of negative affect (depression and anxiety) and pain catastrophizing, and exploring the potential benefit of emotional suppression/distraction as a component of these associations. BPD symptoms were positi…

BACKGROUND: Previous research has emphasized the importance of therapists giving Routine Outcome Monitoring (ROM) feedback to their patients. It has been shown that several factors influence therapists' tendency to provide ROM feedback to their patients.
METHODS: In this qualitative study, using a semi-structured interview followed by thematic analysis using Atlas.ti, we focused on experiences of therapists and patients with a disorder specific ROM instrument: the Borderline Personality Disorder Severity Index-IV (BPDSI-IV). Ten patients with a borderline personality disorder who had been in …

Borderline personality disorder is a complex disease model as it encompasses a diversity of pathological personality traits and psychopathological symptoms. It is not surprising, therefore, that it is often manifested by personality disorders across all three clusters and accompanied by other mental (Axis I) disorders. This melange makes both psychological treatment and pharmacotherapy especially challenging, and this paper describes the case of a particularly complex case of a 33-year-old Romanian patient, who has a history of severe deprivation in childhood, mood and substance use disorder i…

Individuals with borderline personality disorder (BPD) or antisocial personality disorder (ASPD) are overrepresented in forensic settings. Yet, despite the burden these disorders place on healthcare and criminal justice systems, there remains a lack of evidence-based pharmacological treatments. Epidemiological data have shown that comorbid cannabis use disorders are common in BPD and ASPD. ∆9 -Tetrahydrocannabinol, the primary psychoactive constituent of cannabis, is an exogenous cannabinoid that stimulates the endocannabinoid system (ECS). Hence, an investigation of the ECS in these condition…

Organic personality disorder (OPD) is the traditional diagnostic category used to account for personality disturbances after traumatic brain injury (TBI). The recent use of Axis-II personality disorders, notably borderline personality disorder (BPD), has appeared in the TBI literature as an alternative to OPD. This would presumably offer a better description and understanding of the multiple clinical manifestations of these personality changes and disorders. This article offers a view that it is possible and fruitful to use both diagnoses in a complementary manner. An accurate recognition of t…

Post-traumatic stress disorder (PTSD) and borderline personality disorder (BPD) have been associated with an increased generation of false memories. We aimed to disentangle disorder-specific false memory in individuals with PTSD and BPD using the Deese-Roediger-McDermott (DRM) paradigm. It measures the tendency to mistakenly remember stimuli that are associated with actually presented material, but have not been presented. Participants with BPD without comorbid PTSD (n = 32), participants with PTSD without comorbid BPD (n = 28), and mentally healthy controls (HC, n = 30) were given a word reco…

Since both borderline personality disorder (BPD) and bipolar II disorder (BP-II) are common psychiatric diagnoses due to their co-morbidity and similar characteristics, some researchers suggested they coexist along a spectrum of cyclical affective disorders. As part of the ongoing debate regarding these central diagnoses, a few studies were recently conducted. The results showed that patients with BPD reported higher Levels of impulsiveness and hostility across Axis I diagnostic groups. Furthermore, refining the impulsivity scale revealed different types of impulsivity in the two disorders. Th…

OBJECT: To determine which constellation of clinical features constitutes the core of borderline personality disorder (BPD).
METHOD: The criterion of endurance was used to identify the constellation of features which are most basic, or core, in borderline personality disorder. Two sets of constellations of DSM-III features were tested, each consisting of three groupings. The first set of constellations was constructed according to Clarkin's factor analysis; the second was theoretically derived. Broadly speaking, the three groupings concerned 'self', 'emotional regulation', and 'impulse'. Chan…

The number of well-designed placebo-controlled studies on pharmacological treatment of borderline personality disorder has been small. We present a breakdown of results of placebo-controlled pharmacological studies, sorting target symptoms into the trait dimensions of affective instability, anxiety inhibition, cognitive-perceptual disturbances, and impulsivity-aggression. Twenty randomized placebo-controlled pharmacological trials studying typical and atypical antipsychotics, selective serotonin reuptake and monoamine oxidase inhibitors, tricyclic antidepressants, mood stabilizers, and benzodi…

ObjectiveBorderline Personality Disorder (BPD) remains one of the most challenging conditions encountered in inpatient psychiatry, particularly in regional settings where access to specialised psychological care is limited. Drawing on the personal experience of a public psychiatric specialist practicing in regional Australia, this piece reflects on diagnostic dilemmas, therapeutic boundaries, and the emergence of other challenges unique to the treatment of BPD. A large part of diagnostic complexity in BPD often arises from overlapping symptomatology of differential diagnoses such as Bipolar di…

BACKGROUND: Structural imaging studies of borderline personality disorder (BPD) have identified regions of reduced and increased cortical volume, as well as volume reductions in the hippocampus and amygdala, although with considerable variability across studies. Examining adolescent patients with the disorder can reduce potential confounding effects such as later development of affective and other comorbid disorders.
METHODS: Fifty-one adolescents (48 females, 3 males) with BPD and without comorbid disorders and with 43 matched healthy controls underwent whole-brain voxel-based morphometry (V…
