Brain TXA(2)and PGI(2)levels related to diffuse brain injury with secondary insults.

Secondary insults such as hypotension and hypoxia with head injury are associated with increased mortality and morbidity in comparison with head injury alone. In the present study the changes of brain thromboxane A(2)(TXA(2)) and prostacyclin (PGI(2)) levels in a Marmarou's rodent model of diffuse brain injury with hypotension and hypoxia were observed and the effect of diaspirin cross linked haemoglobin solution (DCLHb) were also investigated. Thirty-two male Sprague-Dawley (SD) rats were randomized into four groups: sham; head injury alone; head injury with secondary insults; and injury with

Acute diffuse axonal injury following repeated mild traumatic brain injury in juvenile rats.

Mild traumatic brain injuries (mTBIs) are caused by biomechanical forces being transmitted to the brain, causing neuronal connections to be subjected to sheering forces. The injury severity can be affected by a number of factors that include age and sex, however, there remains a paucity of data on how repeated mTBI (r-mTBI) impacts the female brain. In these studies, male and female juvenile rats [postnatal day (PND) 25-26] were administered a total of eight mTBIs over a 2-day period. Following each mTBI, rats were immediately assessed for acute neurological impairment. After eight mTBIs were

Axons-on-a-chip for mimicking non-disruptive diffuse axonal injury underlying traumatic brain injury.

Diffuse axonal injury (DAI) is the most severe pathological feature of traumatic brain injury (TBI). However, how primary axonal injury is induced by transient mechanical impacts remains unknown, mainly due to the low temporal and spatial resolution of medical imaging approaches. Here we established an axon-on-a-chip (AoC) model for mimicking DAI and monitoring instant cellular responses. Integrating computational fluid dynamics and microfluidic techniques, DAI was induced by injecting a precisely controlled micro-flux in the transverse direction. The clear correlation between the flow speed o

Evaluation of cerebrovascular CO2-reactivity and autoregulation in patients with post-traumatic diffuse brain swelling (diffuse injury III).

The present study was undertaken to elucidate the status of autoregulation and CO2-reactivity soon after injury in patients with a post-traumatic diffuse bilateral brain swelling. A prospective study was carried out in 31 consecutively admitted patients with a severe head injury and a Diffuse Brain Injury type III, following the definition stated by the Traumatic Coma Data Bank classification. To evaluate CO2-reactivity, AVDO2 was measured before and after ventilator manipulations. Assuming a constant CMRO2 during the test, changes in 1/AVDO2 reflect changes in CBF. Patients with changes in es

Diffusion-weighted imaging demonstrates transient cytotoxic edema involving the corpus callosum in a patient with diffuse brain injury.

Reversible T2 hyperintense signal abnormality in the corpus callosum, although frequently seen after diffuse brain injury, has not been well clarified. With some accumulated evidence, we report a case of diffuse brain injury in a 24-year-old man. Magnetic resonance imaging (MRI) demonstrated T2 hyperintense signals in the trunk and the splenium of the corpus callosum 12 days postinjury. Echo-planar diffusion-weighted imaging was also performed on the same day, which revealed decreased diffusion (hyperintense signals) in the same site and almost the same size as T2 hyperintense signals. T1-weig

Brain age gap as a diffusion MRI-based marker of traumatic brain injury-related brain changes and associated outcomes.

Traumatic brain injury is a common neurological disorder and a leading cause of long-term disability, presenting with heterogeneous cognitive, emotional, and functional impairments. A critical clinical challenge is the early identification of patients at risk for persistent symptoms. The Brain Age Gap (BAG), the difference between an individual's predicted brain age from imaging data and their chronological age, has emerged as a potential marker of injury-related brain changes. Here, we aim to evaluate diffusion-MRI-derived BAG for identifying traumatic brain injury patients (Glasgow Coma Scal

A differential response of diffuse brain injury on the concentrations of endothelin and nitric oxide in the plasma and brain regions in rats.

In the present study, we hypothesized that acute diffuse brain injury (DBI) in rats would produce an increase in endothelin-1 (ET-1), a potent vasoconstrictor, and/or nitric oxide (NO), a potent vasodilator, in plasma and brain areas in rats. DBI was induced in anesthetized male Sprague-Dawley rats (350-400 g) using a 350 g weight dropped from 1 meter height impact through a device designed by Marmarou et al., 1994. Blood plasma and brain tissue (cerebral cortex, diencephalon and brain stem) samples were collected for estimation of ET-1 and NO at zero or 6 h from rats (n = 6) subjected to DBI

Diffusion Tensor Imaging for Brain Injury Assessment: Methodological Foundations and Clinical Insights.

Diffusion tensor imaging (DTI) has emerged as a powerful neuroimaging modality for investigating white matter microstructure and its alterations following brain injury. This review presents a comprehensive overview of DTI, encompassing its physical principles, mathematical modeling of diffusion tensors, and known limitations of the technique. We explore key methodological considerations, including acquisition protocols, preprocessing pipelines, vendor-related variability, atlas registration, and the role of diffusion phantoms in calibration. With the rise of big data in medical imaging, we hig

Diffusion Tensor Imaging Correlates of Resilience Following Adolescent Traumatic Brain Injury.

BACKGROUND: Traumatic brain injury (TBI) is associated with considerable mortality and morbidity in adolescents, but positive outcomes are possible. Resilience is the concept that some individuals flourish despite significant adversity. OBJECTIVE: To determine if there is a relationship between resilience-promoting factors that are known to promote resilience and white matter (WM) microstructure 1 year after complicated mild TBI or moderate or severe TBI that is sustained by adolescents. METHOD: We examined the relationship between performance on a self-report measure of resilience-promoting

Pathology of fatal diffuse brain injury in severe non-penetrating head trauma.

Traumatic brain injury (TBI) is recognised as a serious global public health problem that imposes a heavy socioeconomic burden on society. The vast majority of cases result from road traffic accidents and falls, and the injuries are mainly attributed to velocity-related mechanisms. Lethal cases are mostly found to suffer from severe diffuse brain injuries (DBI), comprising diffuse vascular injury, diffuse axonal injury (DAI), generalized cerebral edema and ischemic-hypoxic injury. Coup and contrecoup brain contusions may also occur. This study set out to describe the pathological findings of s

Emotional prosody and diffusion tensor imaging in children after traumatic brain injury.

PRIMARY OBJECTIVE: Brain structures and their white matter connections that may contribute to emotion processing and may be vulnerable to disruption by a traumatic brain injury (TBI) occurring in childhood have not been thoroughly explored. RESEARCH DESIGN AND METHODS: The current investigation examines the relationship between diffusion tensor imaging (DTI) metrics, including fractional anisotropy (FA) and apparent diffusion coefficient (ADC), and 3-month post-injury performance on a task of emotion prosody recognition and a control task of phonological discrimination in a group of 91 childr

Substantia nigra vulnerability after a single moderate diffuse brain injury in the rat.

Dementia and parkinsonism are late-onset symptoms associated with repetitive head injury, as documented in multiple contact-sport athletes. Clinical symptomatology is the likely phenotype of chronic degeneration and circuit disruption in the substantia nigra (SN). To investigate the initiating neuropathology, we hypothesize that a single diffuse brain injury is sufficient to initiate SN neuropathology including neuronal loss, vascular disruption and microglial activation, contributing to neurodegeneration and altered dopamine regulation. Adult, male Sprague-Dawley rats were subjected to sham o

A finite element model of region-specific response for mild diffuse brain injury.

It is well known that rotational loading is responsible for a spectrum of diffuse brain injuries spanning from concussion to diffuse axonal trauma. Many experimental studies have been performed to understand the pathological and biomechanical factors associated with diffuse brain injuries. Finite element models have also been developed to correlate experimental findings with intrinsic variables such as strain. However, a paucity of studies exist examining the combined role of the strain-time parameter. Consequently, using the principles of finite element analysis, the present study introduced

A preliminary validation study of diffusion tensor imaging as a measure of functional brain injury.

BACKGROUND: Diffusion tensor imaging (DTI) characterizes multiple sclerosis (MS) tissue injury, although it has remained unproven whether DTI changes in disease have functional consequences. The medial longitudinal fasciculus (MLF) is a key brainstem pathway for ocular adduction and is commonly injured in patients with MS, typically resulting in internuclear ophthalmoparesis. OBJECTIVE: To validate DTI as a physiologically relevant measure of brain tissue integrity. DESIGN: A correlation study of ocular dysmotility and DTI conducted between January 2004 and September 2004. SETTING: Multiple

Metabotropic glutamate receptor antagonists and agonists: potential neuroprotectors in diffuse brain injury.

Our previous study has suggested that metabotropic glutamate receptors (mGluRs) were significantly involved in the secondary processes after diffuse brain injury (DBI) and that mGluRs antagonists or agonists may be used for the treatment of DBI. In the present study, the neuroprotective effects of antagonists or agonists of mGluRs on DBI were further investigated. Sprague-Dawly rats were randomized into the following six groups: (i) normal control; (ii) sham-operated control; (iii) DBI; (iv) DBI treated with normal saline (NS); (v) DBI treated with alpha-methyl-4-carboxy-phenylglycine (MCPG);

Influence of oxygen therapy on glucose-lactate metabolism after diffuse brain injury.

OBJECT: Severe traumatic brain injury (TBI) imposes a huge metabolic load on brain tissue, which can be summarized initially as a state of hypermetabolism and hyperglycolysis. In experiments O2 consumption has been shown to increase early after trauma, especially in the presence of high lactate levels and forced O2 availability. In recent clinical studies the effect of increasing O2 availability on brain metabolism has been analyzed. By their nature, however, clinical trauma models suffer from a heterogeneous injury distribution. The aim of this study was to analyze, in a standardized diffuse

Detrimental role of bradykinin B2 receptor in a murine model of diffuse brain injury.

Inhibition of the bradykinin B2 receptor type (B2R) has been shown to improve neurological outcome in models of focal traumatic brain injury. However, the involvement of B2R in trauma-induced diffuse injury has not yet been explored. This is an important point, since in humans a pattern of diffuse injury is commonly found in severely injured patients and has been associated with a poor neurological outcome and prognosis. Using the non-peptide B2R antagonist LF 16-0687 Ms and B2R null (B2R-/-) mice, we investigated the role of B2R in a model of closed head trauma (CHT). LF 16-0687 Ms given 30 m
