Extensive brainstem infiltration, not mass effect, is a common feature of end-stage cerebral glioblastomas.

BACKGROUND: Progress in extending the survival of glioblastoma (GBM) patients has been slow. A better understanding of why patient survival remains poor is critical to developing new strategies. Postmortem studies on GBM can shed light on why patients are dying. METHODS: The brains of 33 GBM patients were autopsied and examined for gross and microscopic abnormalities. Clinical-pathologic correlations were accomplished through detailed chart reviews. Data were compared with older published autopsy GBM studies that predated newer treatment strategies, such as more extensive surgical resection a

[Peduncular hallucinosis associated with a space occupying lesion of the brain stem].

INTRODUCTION: Traditionally visual hallucinations have been divided into simple and complex. Complex hallucinations due to disorder of the brainstem have been described, and this process referred to as peduncular hallucinosis. CLINICAL CASE: A 74 year old woman with a history of endometrial carcinoma with metastases to bones and lungs, was admitted for progressive ataxia and paraesthesias of the left half of her body. During her admission to hospital she had complex visual hallucinations of cartoon animals, animals, people and geometrical figures. The patient herself realized that they were h

Microvascular anatomy of foramen caecum medullae oblongatae.

The foramen caecum (FC) is a triangular-shaped fossa situated in the midline on the base of the brain stem, at the pontomedullary junction. Although this area is known to have a very high concentration of brainstem perforating vessels, its microvascular anatomy has not been studied in detail. The purpose of this study was to detail the microvasculature of this territory. Twenty unfixed brains were injected with silicone rubber solution and dissected under a microscope equipped with a camera. The origin, course, outer diameter, and branching pattern of the perforators were examined. The total n

Surgical Approaches for Brainstem Tumors in Pediatric Patients.

The brainstem is a complex anatomical structure, and tumors in this region account for approximately 12% of all pediatric brain tumors. In this chapter, we detail both the intrinsic and extrinsic anatomy of the brainstem, propose a new redivision into eight anatomical compartments, and describe ten traditional safe entry zones-adding two novel approaches: the supratrigeminal zone for anterior-superior pontine lesions and the interpeduncular cerebellar route for lesions located between the middle and superior cerebellar peduncles. Anatomic knowledge of intrinsic and extrinsic brainstem structur

Safety and diagnostic yield of robotic-assisted stereotactic biopsy for pediatric brainstem lesions: a systematic review and single-arm meta-analysis.

Pediatric brainstem lesions pose significant diagnostic challenges due to their deep-seated location and critical neuroanatomical surroundings. Robotic-assisted stereotactic biopsy has emerged as a promising minimally invasive strategy, yet its safety and efficacy in this vulnerable population remain incompletely defined. We conducted a systematic review and single-arm meta-analysis in accordance with PRISMA guidelines (PROSPERO registration: CRD420251135743). PubMed, Embase, Web of Science, and Cochrane Library were searched from inception to May 2025. Eligible studies included pediatric pati

Pilocytic astrocytoma presenting as an intrinsic brainstem tumor: case report and review of the literature.

OBJECTIVE: Pilocytic astrocytomas are typically circumscribed, slow growing tumors found in children and young adults. This type of tumor can arise from various locations in the neuraxis. In the brainstem, pilocytic astrocytomas grow as dorsal exophytic masses. To our knowledge, pilocytic astrocytomas arising entirely within the brainstem have not been reported in the literature. CLINICAL PRESENTATION: A 22-year-old man presented with double vision for three months. A neurological exam revealed a right sixth nerve palsy. INTERVENTION: Magnetic resonance imaging revealed an intra-axial mass w

Surgical treatment and prognosis of adult patients with brainstem gliomas.

The paper presents 47 adult patients who were surgically treated due to brainstem gliomas. Thirteen patients presented with contrast-enhancing Grades III and IV gliomas, according to the WHO classification, 13 patients with contrast-enhancing tumours originating from the glial cells (Grade I; WHO classification), 9 patients with diffuse gliomas, 5 patients with tectal brainstem gliomas and 7 patients with exophytic brainstem gliomas. During the surgical procedure, neuronavigation and the diffusion tensor tractography (DTI) of the corticospinal tract were used with the examination of motor evok

[Primary tumors of the brain stem. State of the problem].

Primary brainstem gliomas are still poorly studied in neurooncology. This concept includes tumors with different histological and genetic features, as well as variable clinical course and outcomes. Nevertheless, treatment implies radiotherapy without a clear idea of morphological substrate of disease in 80% of cases. Small number of studies and insufficient data on histological and genetic nature of brainstem tumors complicate clear diagnostic and treatment algorithms. This review provides current information regarding primary glial brainstem tumors. Appropriate problems and objectives are hig

Oncologic causes of oculopalatal tremors: neurophysiology and treatment.

Oculopalatal tremor (OPT) is an acquired pathology characterized by continuous and rhythmical soft palatal movements combined with pendular nystagmus. Aside from vascular lesions, oncological masses affecting the dentatorubro-olivary pathway can impair brainstem and/or cerebellar pathways, manifesting as dyssynchronous movement. In this review, we delve into the neurophysiology of OPT along with oncological causes and treatment options based on the most recent clinical trial data. This literature review includes medication treatment data from clinical trials enrolling individuals with features

Using Epigenetic Reprogramming to Treat Pediatric Brain Cancer.

In this issue of Cancer Cell, Nagaraja et al. dissect the molecular mechanisms underlying therapeutic responses to transcriptional disruptors in the fatal pediatric brain tumor, diffuse intrinsic pontine glioma (DIPG). Moreover, they identify super-enhancers mediating these effects, highlighting how normal brain developmental programs can be hijacked in cancer.

Endoscopic Third Ventriculostomy and Pineal Biopsy from a Single Entry Point.

Pineal neoplasms have a significant impact on children although they are relatively uncommon. They account for approximately 3-11% of all childhood brain tumors, which is considerably higher than the <1% seen in adult brain tumors. These tumors can be divided into three main categories: germ cell tumors, parenchymal pineal tumors, and tumors arising from related anatomical structures. Obtaining an accurate and minimally invasive tissue diagnosis is crucial for selecting the most appropriate treatment regimen for patients with pineal gland tumors. This is due to the diverse treatment options av

PBI-05204, a supercritical CO2 extract of Nerium oleander, suppresses glioblastoma stem cells by inhibiting GRP78 and inducing programmed necroptotic cell death.

Successful treatment of glioblastoma multiforme (GBM), an aggressive form of primary brain neoplasm, mandates the need to develop new therapeutic strategies. In this study, we investigated the potential of PBI-05204 in targeting GBM stem cells (GSCs) and the underlying mechanisms. Treatment with PBI-05204 significantly reduced both the number and size of tumor spheres derived from patient-derived GSCs (GBM9, GSC28 and TS543), and suppressed the tumorigenesis of GBM9 xenografts. Moreover, PBI-05204 treatment led to a significant decrease in the expression of CD44 and NANOG, crucial markers of p
