ACR Appropriateness Criteria® Supplemental Breast Cancer Screening Based on Breast Density.

Mammography remains the only validated screening tool for breast cancer, however, there are limitations to mammography. One of the limitations of mammography is the variable sensitivity based on breast density. Supplemental screening may be considered based on the patient's risk level and breast density. For average-risk women with nondense breasts, the sensitivity of digital breast tomosynthesis (DBT) screening is high; additional supplemental screening is not warranted in this population. For average-risk women with dense breasts, given the decreased sensitivity of mammography/DBT, this popu…

Trends in Clinical Breast Density Assessment From the Breast Cancer Surveillance Consortium.

Changes to mammography practice, including revised Breast Imaging Reporting and Data System (BI-RADS) density classification guidelines and implementation of digital breast tomosynthesis (DBT), may impact clinical breast density assessment. We investigated temporal trends in clinical breast density assessment among 2 990 291 digital mammography (DM) screens and 221 063 DBT screens interpreted by 722 radiologists from 144 facilities in the Breast Cancer Surveillance Consortium. After age-standardization, 46.3% (95% CI = 44.1% to 48.6%) of DM screens were assessed as dense (heterogeneously/extre…

Combining quantitative and qualitative breast density measures to assess breast cancer risk.

BACKGROUND: Accurately identifying women with dense breasts (Breast Imaging Reporting and Data System [BI-RADS] heterogeneously or extremely dense) who are at high breast cancer risk will facilitate discussions of supplemental imaging and primary prevention. We examined the independent contribution of dense breast volume and BI-RADS breast density to predict invasive breast cancer and whether dense breast volume combined with Breast Cancer Surveillance Consortium (BCSC) risk model factors (age, race/ethnicity, family history of breast cancer, history of breast biopsy, and BI-RADS breast densit…

Plasma C-peptide, mammographic breast density, and risk of invasive breast cancer.

BACKGROUND: Insulin may promote breast cancer directly by stimulating the insulin receptor or indirectly by increasing the plasma concentration of active sex hormones. The association between insulin and breast density, a strong breast cancer risk factor, has not been thoroughly studied. We measured associations between c-peptide (a molar marker of insulin secretion), breast cancer risk, and breast density measurements in case-control studies nested within the Nurses' Health Study and Nurses' Health Study II cohorts. METHODS: Breast cancer associations were estimated with multivariate logisti…

Molecular Breast Imaging and Digital Breast Tomosynthesis for Dense Breast Screening: The Density MATTERS Trial.

Background Molecular breast imaging (MBI) relies on the functional uptake of a radiotracer, technetium 99m sestamibi, to reveal cancers that are occult on mammograms due to breast density. Purpose To assess the performance of screening MBI as a supplement to digital breast tomosynthesis (DBT) in women with dense breasts. Materials and Methods In this prospective, multiyear, multicenter trial from five sites, women with dense breasts were prospectively enrolled from 2017 to 2022 and underwent two annual screening rounds of DBT and MBI to assess the incremental cancer detection rate (CDR, report…

Preoperative Breast Magnetic Resonance Imaging Use by Breast Density and Family History of Breast Cancer.

BACKGROUND: Use of preoperative breast magnetic resonance imaging (MRI) among women with a new breast cancer has increased over the past decade. MRI use is more frequent in younger women and those with lobular carcinoma, but associations with breast density and family history of breast cancer are unknown. MATERIALS AND METHODS: Data for 3075 women ages >65 years with stage 0-III breast cancer who underwent breast conserving surgery or mastectomy from 2005 to 2010 in the Breast Cancer Surveillance Consortium were linked to administrative claims data to assess associations of preoperative MRI u…

Consumer perspective on breast density notification in Australia.

Breast density is a strong and prevalent risk factor for breast cancer and it significantly reduces the sensitivity of mammography to detect the disease. Inconsistency in breast density notification policy across screening programs in Australia has resulted in significant health promotion challenges, with low awareness and knowledge of breast density as a risk factor and confusion regarding assessment and risk management for women with dense breasts. Australian breast density notification policy has recently been revised and now recommends women be informed of their breast density; however, th…

Breast density in NF1 women: a retrospective study.

Neurofibromatosis type 1 (NF1) is an autosomal dominant condition caused by neurofibromin haploinsufficiency due to pathogenic variants in the NF1 gene. Tumor predisposition has long been associated with NF1, and an increased breast cancer (BC) incidence and reduced survival have been reported in recent years for women with NF1. As breast density is another known independent risk factor for BC, this study aims to evaluate the variability of breast density in patients with NF1 compared to the general population. Mammograms from 98 NF1 women affected by NF1, and enrolled onto our monocentric BC …

Lipidome of mammographic breast density in premenopausal women.

BACKGROUND: High mammographic breast density (MBD) is a strong risk factor for breast cancer development, but the biological mechanisms underlying MBD are unclear. Lipids play important roles in cell differentiation, and perturbations in lipid metabolism are implicated in cancer development. Nevertheless, no study has applied untargeted lipidomics to profile the lipidome of MBD. Through this study, our goal is to characterize the lipidome of MBD in premenopausal women. METHODS: Premenopausal women were recruited during their annual screening mammogram at the Washington University School of Me…

Mammographic breast density in infertile and parous women.

BACKGROUND: Mammographic breast density is a useful marker for breast cancer risk, as breast density is considered one of the strongest breast cancer risk factors. The study objective was to evaluate and compare mammographic breast density in infertile and parous women, as infertility may be associated with high breast density and cancer occurrence. METHODS: This study evaluated mammographic breast density using two different systems, BIRADS and Boyd. A selected patient population of 151 women with primary infertility (case group) was compared to 154 parous women who had at least one previous…

Mammographic density and survival in interval breast cancers.

INTRODUCTION: Mammographic density (MD) is the strongest risk factor for breast cancer. It is also strongly associated with interval cancers (ICs) due to decreased screening sensitivity and possibly by also giving rise to more aggressive tumors. With this information as background, we compared survival in interval and screen-detected cancers, taking MD into consideration. METHODS: The patients were postmenopausal women ages 50 to 74 years who were diagnosed with breast cancer in Sweden between 1993 and 1995. A total of 1,115 women with screen-detected cancers and 285 with ICs had available ma

Women's responses to information on mammographic breast density.

The objective of this study was to determine the negative and positive outcomes of providing mammographic breast density (MBD) information to participants of a screening program. A randomized experiment was conducted with a sample of 618 women 50 years or older with MBD greater than 50% of breast volume. The intervention consisted of reporting the presence of MBD in the screening mammography results letter that was sent along with an information pamphlet. Compared to the controls, more women in the intervention group described the term breast density correctly and recognized it as a risk facto…

Effect of danazol on the radiographic density of breast parenchyma.

We have given danazol to 25 women with mammary dysplasia. Clinical, cytologic and radiographic examinations were made before, during and after this treatment. In 23 patients breast pain disappeared completely. Danazol produced such a decrease in breast volume and density that on the mammogram the parenchyma was much more easily visualized. We suggest that danazol might be given before mammography to high risk women having dense dysplasic breasts, in order to ensure easier and more accurate interpretation.

Physician Knowledge, Attitudes, and Practices Regarding Breast Density.

Background: Many states have enacted breast density laws, requiring that women be informed of their breast density status; however there is currently no consensus for screening guidelines or recommendations for women with dense breasts. The objective of this study is to access physician views about breast density and their practices for breast cancer screening of women with dense breasts in light of breast density laws. Materials and Methods: Setting: Academic medical centers, community and private practices mostly in New York City. Participants: Primary care providers (PCPs), radiologists and…

Multiple metabolic risk factors and mammographic breast density.

PURPOSE: We examined whether obesity and a history of diabetes, hypertension, and elevated cholesterol, individually and in combination, are associated with breast density, a strong risk factor for breast cancer. METHODS: We measured percent density and dense area using a computer-assisted method (n = 191; age range = 40-61 years). We used linear regression models to examine the associations of each metabolic condition and the number of metabolic conditions (zero, one, two, and three or four conditions) with breast density. RESULTS: Among individual metabolic conditions, only high blood chol…
