Abstinence phenomena of chronic cannabis-addicts prospectively monitored during controlled inpatient detoxification (Part II): Psychiatric complaints and their relation to delta-9-tetrahydrocannabinol and its metabolites in serum.

OBJECTIVE: To investigate the impact of inpatient detoxification treatment on psychiatric symptoms of chronic cannabis addicts and to analyze the influence of serum cannabinoid levels on the severity of these symptoms. METHODS: Thirty five treatment-seeking, not active co-morbid chronic cannabis dependents (ICD-10) were studied on admission and on abstinence days 8 and 16, using several observational and self-report scales, such as Hamilton Depression Rating Scale (HAMD), Hamilton Anxiety Rating Scale (HAMA), Young Mania Rating Scale (YMRS) and Brief Psychiatric Rating Scale (BPRS), and the S…

Mesoridazine and thioridazine: clinical effects and blood levels in refractory schizophrenics.

Seven schizophrenic (according to DSM-III criteria) inpatients completed a two-phase study; each phase had a 1-week drug-free period followed by 6 weeks of a drug trial. The first phase uniformly involved treatment with chlorpromazine, and in the second phase patients received either mesoridazine (N = 3) or thioridazine (N = 4). Clinical ratings (Brief Psychiatric Rating Scale and Clinical Global Impressions) and neuroleptic blood levels were obtained weekly throughout the study. Whereas patients failed to respond to chlorpromazine 1800 mg/day, response to mesoridazine 400 mg/day and to thiori…

Exploration of the clinical profile of rubidium chloride in depression: a systematic open trial.

Thirty-one female inpatient depressives underwent a systematic open trial with rubidium chloride, 180 to 720 mg/day. By week 2, at least two-thirds had improved significantly (p less than 0.01) as measured by standard rating instruments such as the Brief Psychiatric Rating Scale and the Hamilton Depression Scale. Regression analysis suggested that the retarded endogenous pattern was most predictive of positive response. Treatment-emergent symptomatology, such as diarrhea, polyuria, and excitement, was generally mild and rarely necessitated interruption of the trial. The authors conclude that t…

A double-blind, placebo-controlled study of adjunctive nadolol in the management of violent psychiatric patients.

We report a controlled 3-week study (n = 30) of adjunctive use of nadolol, 80 to 120 mg per day, vs. placebo for the management of violent psychiatric patients. There were no remarkable adverse cardiac effects at this dose. The active treatment group showed lower total Brief Psychiatric Rating Scale (BPRS) scores after the first week of treatment (analysis of covariance [ANCOVA], p less than .08) and similar trends for the activation factor and the hostility scale. There were parallel findings for measures of extrapyramidal symptoms (EPS) (Simpson-Angus Neurological Rating Scale), which were r…

Types of psychopathology displayed by heroin addicts.

Symptom profiles of 157 male and 54 female heroin addicts based on Brief Psychiatric Rating Scale scores were compared with normative data for 6,000 psychiatric patients by using previously described schizo-depressive and coping-resignation contrast functions. Diagnostic types found among the addicts were as follows: anxious-depressive features, 35.1%; undifferentiated syndromes, 26.5%; hostile-depressive features, 16.6%; retarded depressive features, 9.5%; hostile suspicious traits, 6.6%; and blunted affect without significant depressive features, 5.7%. None of the addicts showed levels of th…

Resting-state fMRI reveals potential neural correlates of impaired cognition in Huntington's disease.

BACKGROUND: Huntington's Disease (HD) is characterized by motor, cognitive and psychiatric dysfunction. Functional MRI (fMRI) provides new insight into the pathologic mechanism underlying the cognitive symptoms. Previous fMRI studies of HD focused on functional synchronization of various brain areas by measuring functional connectivity, a method that is unable to identify regional intrinsic neural activity changes in the brain. To fill in this gap, we utilized amplitude of low frequency fluctuations (ALFF). OBJECTIVE: To investigate alterations in regional brain activity and their association

Treatment-Resistant Schizophrenia: Treatment Response and Resistance in Psychosis (TRRIP) Working Group Consensus Guidelines on Diagnosis and Terminology.

OBJECTIVE: Research and clinical translation in schizophrenia is limited by inconsistent definitions of treatment resistance and response. To address this issue, the authors evaluated current approaches and then developed consensus criteria and guidelines. METHOD: A systematic review of randomized antipsychotic clinical trials in treatment-resistant schizophrenia was performed, and definitions of treatment resistance were extracted. Subsequently, consensus operationalized criteria were developed through 1) a multiphase, mixed methods approach, 2) identification of key criteria via an online s

Pretreatment plasma homovanillic acid in schizophrenia and schizoaffective disorder: the influence of demographic variables and the inpatient drug-free period.

BACKGROUND: The relationship between plasma homovanillic acid (pHVA) and schizophrenic symptoms has not been conclusively determined. We ree … lness in the schizophrenic patient subsample. Plasma HVA correlations with behavior, as measured by Brief Psychiatric Rating Scale factors (anxiety/depression and hostility/suspiciousness), emerged only when considering schizophrenic patients drug-free for more than 2 weeks. No correlation was found between pHVA and the age of illness onset or the duration of the delay of treat …

Relationship between cardiovagal modulation and psychotic state in patients with paranoid schizophrenia.

Disturbed autonomic nervous system (ANS) function in schizophrenia might contribute to increased cardiovascular mortality. We obtained heart rate variability indices from 40 unmedicated schizophrenic patients and 58 matched controls. Mainly we found that patients displaying stronger psychotic symptoms as assessed by the Brief Psychiatric Rating Scale exhibit more severe cardiac ANS disturbances compared with controls.

Affective and behavioral features of DSM-III borderline personality disorder. Are they valid?

This study was designed to determine if a set of seven clinical features, closely reflecting Diagnostic and Statistical Manual III criteria, were individually and collectively more common in borderlines compared to patients with schizophrenia, affective disorders, and anxiety disorders. A 7-item borderline subscale was generated from the expanded Brief Psychiatric Rating Scale and administered to 1,762 psychiatric inpatients. Results indicated that scores on the total borderline scale were significantly higher in borderline patients compared to the control groups. Borderline patients manifeste…

Zuclopenthixol, D(1)/D(2) antagonist, for treatment of chronic aggressive schizophrenia and psychotic oligophrenic patients.

An open label trial was conducted to study the efficacy of zuclopenthixol decanoate, a D(1)/D(2) antagonist, in the treatment of chronic (> 10 years) neuroleptic-resistant aggressive schizophrenia (n = 10) and psychotic oligophrenic patients (n = 6). A significant reduction was noted in the Brief Psychiatric Rating Scale score, especially in the clusters of hostility-suspiciousness and excitement. After many years of seclusion, ten of the 16 patients were transferred to an open psychiatric department and were allowed to visit their families.

Capacity to consent to treatment in severe eating disorders.

Eating disorders represent a disabling, deadly and costly condition, whose principal treatment is constituted by weigh restoration and psychotherapy. Partial or total refuse of treatment is very common, leading some authors to question their decision-making capacity (DMC) to consent to treatment. However, very few studies have investigated treatment DMC, leading to contrasting results. Forty-five women were enrolled at the Psychiatric and Eating Disorders Unit of the University Hospital Policlinico Umberto I of Rome. Psychiatric symptoms severity (Brief Psychiatric Rating Scale Expanded, BPRS-…

Generalized anxiety disorder: practical assessment and management.

Generalized anxiety disorder is common among patients in primary care. Affected patients experience excessive chronic anxiety and worry about events and activities, such as their health, family, work, and finances. The anxiety and worry are difficult to control and often lead to physiologic symptoms, including fatigue, muscle tension, restlessness, and other somatic complaints. Other psychiatric problems (e.g., depression) and nonpsychiatric factors (e.g., endocrine disorders, medication adverse effects, withdrawal) must be considered in patients with possible generalized anxiety disorder. [The analysis of schizophrenic inpatients' escape behavior].

To study the escaping behavior of 1030 schizophrenic in-patients, "Brief Rating Scale for Dangerous Behavior of In-patient" was used to asses them within their first week hospitalization from May 1994 to March 1996. The result showed that 276 (26.8%) patients had escaping behavior. The patients with paranoid schizophrenia had higher rate of escaping behavior. There was no correlation being found between sex, age, duration of illness, and escaping behavior. The patients with the history of such behavior were more likely to escape from ward (41.9%). The suggestion for preventing such behavior we

Rapid tranquilization: a comparative study of thiothixene and haloperidol.

In a double-blind comparative clinical study, both thiothixene and haloperidol were found to be effective agents for rapid tranquilization of manic and schizophrenic patients. While no statistically significant difference in any of the psychopathological areas which may create a psychiatric emergency was found, results of this study provide further substantiation of the notion that thiothixene has favorable effects on anergia .

Program evaluation of a community crisis stabilization program.

The purpose of this program evaluation was to evaluate the outcomes of treatment for participants in a community crisis stabilization (CCS) program. This CCS incorporated a patient-centered and wellness model of treatment. A descriptive study was conducted to assess change in psychological symptoms and quality of life of participants from admission to discharge. The sample (n = 42) was evaluated on admission and at discharge with four measurements: the Brief Symptoms Inventory (BSI), the Revised 24-Item Behavior and Symptom Identification Scale (BASIS-24), the Brief Psychiatric Rating Scale (B…

A review of screening instruments for assessing cognition and mental status in older adults.

Older adults in non-psychiatric acute and long-term care settings need to be screened routinely for cognitive function and mental status by clinicians and health care providers. Screening instruments increasingly are being used in order to evaluate programs, implement clinical decisions and conduct research. The purpose, scope and depth of needed assessment guides the selection of the screening instrument. This article critically reviews 11 screening instruments used to assess cognitive function and mental status in older adults: Dementia of the Alzheimer Type Inventory, Brief Cognitive Rating

A study of clinical response to different kinds of neuroleptics in first time medicated schizophrenics.

Clinical responses to different kinds of neuroleptics in fresh schizophrenics were prospectively investigated. The scales used were the Global Assessment Scale (GAS), Brief Psychiatric Rating Scale (BPRS) and Psychiatric Evaluation Scale (PES). The 48 subjects were classified into three groups, 16 in the phenothiazine response group, 19 in the butyrophenone response group and 13 in the nonresponse group. For the schizophrenics who displayed negative symptoms such as motor retardation and blunted affect, while displaying milder positive symptoms, phenothiazine drugs were effective. Contrarily, …
