Increasing threat to the healthcare setting: Candida auris.

PURPOSE OF REVIEW: This review summarizes the current knowledge related to infection prevention for Candida auris in the healthcare setting. Colonization, pathogenesis, and control strategies are discussed. RECENT FINDINGS: There are numerous well documented C. auris outbreaks in healthcare settings, however, evidence is lacking on how to manage these colonized and postinfection patients long term in the acute care setting. SUMMARY: C. auris is an emerging fungal pathogen with unique characteristics that make it challenging to eradicate in healthcare settings once introduced. Mitigation and …

Proposal for a screening protocol for Candida auris colonization.

BACKGROUND: Candida auris is an emerging multidrug-resistant yeast which can cause severe infection in hospitalized patients. Since its first detection in 2009, C. auris has spread globally. The control and elimination of this pathogen in a hospital setting is particularly challenging because of its ability to form biofilms, allowing for long-term patient colonization and persistence in the environment. Identification of C. auris from cultures is difficult due to the morphologic similarities to other yeasts, its slow growth, and the low culture sensitivity when using standard agars and tempera…

Emergence of Candida auris in Vietnam: A case series.

Candida auris (C. auris), a globally emerging pathogen, has posed a significant threat to hospitalized individuals during the COVID-19 in Vietnam. This case series reported (1) common patterns in five patients with non-multidrug-resistant C. auris infections (multiple comorbidities, severe-to-critical illness, use of broad-spectrum antibiotics, or history of surgery/invasive procedures) and (2) high rate of C. auris-associated mortality in this medical setting (four deaths out of five cases). Further studies are needed to (1) identify risk factors for C. auris infections and mortality and (2) …

Identification and Elimination of Antifungal Tolerance in Candida auris.

Antimicrobial resistance is a global health crisis to which pathogenic fungi make a substantial contribution. The human fungal pathogen C. auris is of particular concern due to its rapid spread across the world and its evolution of multidrug resistance. Fluconazole failure in C. auris has been recently attributed to antifungal "tolerance". Tolerance is a phenomenon whereby a slow-growing subpopulation of tolerant cells, which are genetically identical to susceptible cells, emerges during drug treatment. We use microbroth dilution and disk diffusion assays, together with image analysis, to inve

Mechanisms of pathogenicity for the emerging fungus Candida auris.

Candida auris recently emerged as an urgent public health threat, causing outbreaks of invasive infections in healthcare settings throughout the world. This fungal pathogen persists on the skin of patients and on abiotic surfaces despite antiseptic and decolonization attempts. The heightened capacity for skin colonization and environmental persistence promotes rapid nosocomial spread. Following skin colonization, C. auris can gain entrance to the bloodstream and deeper tissues, often through a wound or an inserted medical device, such as a catheter. C. auris possesses a variety of virulence tr…

Molecular Tools for Candida auris Identification and Typing.

Identification of Candida auris by conventional identification methodologies can be challenging. While whole genome sequencing is seen as the golden standard to genotype C. auris at an inter- and intraspecies level, it is costly and time-consuming. Sequencing the transcribed spacer (ITS) region and microsatellite typing provide simple, fast, and inexpensive alternatives for identification and genotyping of C. auris. Here we will describe both molecular approaches.

Molecular Diagnostics in the Times of Surveillance for Candida auris.

Recently, global health professionals have been significantly challenged by the emergence of Candida auris and its propensity to colonize human skin, persist in the healthcare environment, and cause healthcare-associated outbreaks. Additionally, C. auris isolates are often characterized by elevated minimal inhibitory concentration (MIC) values for antifungal drugs. Thus, rapid detection and accurate identification of C. auris together with an assessment of potential antifungal drug resistance has become essential for effective patient management, and infection prevention and control in healthc…

Genome architecture drives antifungal resistance in Candida auris.

Antifungal resistance often results from mutations in drug target genes. However, Narayanan et al. show that segmental duplication and supernumerary chromosomes also contribute to resistance in the emerging fungal pathogen Candida auris. The work expands the genetic basis of antifungal resistance and redefines the mutation-centric paradigm.

[Three Candida auris Case Reports from Istanbul, Turkey].

Candida auris is a fungal pathogen that was first identified in 2009. Since its definition, it has spread globally and has caused life-threatening nosocomial infections. Increases in the number of immunocompromised individuals, empirical use of broad-spectrum antimicrobials and widespread use of catheterizations are the predisposing factors in the development of infection. There are problems for the identification of C.auris with the routine methods. In this case report, infections with C.auris, isolated for the first time from three patients in our hospital's intensive care units (ICU) betwee…

Photodynamic Therapy Is Effective Against Candida auris Biofilms.

Fungal infections are increasing in prevalence worldwide. The paucity of available antifungal drug classes, combined with the increased occurrence of multidrug resistance in fungi, has led to new clinical challenges in the treatment of fungal infections. Candida auris is a recently emerged multidrug resistant human fungal pathogen that has become a worldwide public health threat. C. auris clinical isolates are often resistant to one or more antifungal drug classes, and thus, there is a high unmet medical need for the development of new therapeutic strategies effective against C. auris. Additio…

Colistin interacts synergistically with echinocandins against Candida auris.

Antifungal combination is an interesting approach for the treatment of several fungal infections but there is currently little evidence to support combined therapy in Candida auris infections. The antibacterial colistin has recently been shown to interact synergistically with antifungals against Candida spp., including azole-resistant isolates. The current study evaluated the in vitro interaction between colistin and either caspofungin or micafungin against 15 C. auris isolates by a checkerboard methodology based on the European Committee on Antimicrobial Susceptibility Testing (EUCAST) refere…

Candida auris Phenotypic Heterogeneity Determines Pathogenicity In Vitro.

Candida auris is an enigmatic yeast that provides substantial global risk in health care facilities and intensive care units. A unique phenotype exhibited by certain isolates of C. auris is their ability to form small clusters of cells known as aggregates, which have been to a limited extent described in the context of pathogenic traits. In this study, we screened several nonaggregative and aggregative C. auris isolates for biofilm formation, where we observed a level of heterogeneity among the different phenotypes. Next, we utilized an RNA sequencing approach to investigate the transcriptiona…

Candida auris: what have we learned so far?

PURPOSE OF REVIEW: The increasing prevalence of fungal infections due to Candida species has been well described in critically ill patient populations, but in recent years a new species, Candida auris has received attention from the medical community worldwide. We aim to summarize the current knowledge related to C. auris, as new identification techniques, novel antifungal agents and more experience with outbreak management have been published in the past few years. RECENT FINDINGS: C. auris has been described in several countries, arising independently in separate clades. Its resistance to m…

Experimental Mouse Models of Disseminated Candida auris Infection.

Disseminated candidiasis is a life-threatening disease and remains the most common bloodstream infection in hospitalized patients in the United States. Despite the availability of modern antifungal therapy, crude mortality in the last decade has remained unacceptably high. In particular, Candida auris is a multidrug-resistant, health care-associated fungal pathogen and has recently emerged as the first fungal pathogen to cause a global public health threat. A reliable animal model for disseminated C. auris candidiasis is therefore needed to study the unique aspects of this little-known host-pa…
