Renovascular disease is a common cause of secondary hypertension. Renal artery stenosis is present in up to one third of patients with clini … nce of renal artery stenosis, glomerular filtration is maintained by angiotensin. Administration of captopril in renal scintigraphy removes this compensatory mechanism and causes a temporary impairment of renal function in the affected kidney. Nuclear tracers can visualize this impairment, thus allowing assessment of the physiologic significance of a renal …

Most forms of hypertension require life-long treatment; thus, it is important to determine the continuing effectiveness and safety of any new therapeutic agent. While participating in various investigational studies, 7103 hypertensive patients received captopril, of whom 4397 were treated for 3 months to 4 years. The 4-year patients included 2498 with mild or moderate essential hypertension (diastolic pressure less than 120 mm Hg), 893 with severe essential hypertension, and 517 with renovascular hypertension. Repeated examinations of these long-term therapy patients, the majority of whom also…

The immediate and sustained haemodynamic effects of Captopril (CPT), an oral inhibitor of angiotensin converting enzyme, were studied in six patients (pts) with severe pulmonary hypertension (PH) (pulmonary artery pressure: mean +/- SD value = 57 +/- 20 mmHg). Two pts had primary PH, 2 embolic PH and 2 Eisenmenger Physiology (EP). Administration of 100 mg of CPT in a single oral dose produced a significant decrease only in systemic arterial pressure (SAP) (p less than 0.025) and systemic vascular resistance (SVR) (p less than 0.05) in 5 of 6 pts. Heart rate (HR), cardiac index (CI), pulmonary …

Captopril, an orally active dipeptidylcarboxypeptidase inhibitor, is a promising new antihypertensive agent. Cutaneous reactions, including (rarely) a pityriasis rosea-like eruption, are frequently associated with this therapy. Two new cases of a pityriasis rosea-like captopril-induced eruption support a pharmacologic mechanism for the eruption, since it resolved after the dosage of captopril was lowered in one patient and continued when the dosage of captopril remained unchanged in the other patient. However, the eruption later responded to therapy while use of the drug was continued. Captopr…

Proteinuria developed in six of 81 hypertensive patients given captopril for at least four months (protein excretion, greater than 200 mg/24 hr). Two had previously elevated protein excretion. In all patients the increased protein excretion occurred by the fourth month of treatment. It subsided in four after two to nine months, despite continued therapy. In two of the four, proteinuria cleared completely within seven months after onset, while in the other two it subsided to the range of 600 mg/24 hr. However, in the remaining two patients proteinuria persisted during captopril therapy and was …

OBJECTIVE: The objective of our study was to compare the value of captopril-enhanced Doppler sonography, captopril-enhanced renal scintigraphy, and gadolinium-enhanced MR angiography for detecting renal artery stenosis.
SUBJECTS AND METHODS: Forty-one patients with suspected renovascular hypertension were prospectively examined with captopril-enhanced Doppler sonography, captopril-enhanced renal scintigraphy, gadolinium-enhanced MR angiography, and catheter angiography. The sensitivity and specificity of each technique for detecting renal artery stenosis measuring 50% or greater and 70% or gr…

A preterm infant with renovascular hypertension who developed significant trilineage bone marrow suppression after receiving captopril is reported. Captopril-associated pancytopenia is a very rare complication that was thought to be dose-related and usually caused by accumulation of the drug through defective renal tubular excretion. In our patient, it appears that the combination of renal artery stenosis and renal tubular dysfunction of prematurity might have led to pancytopenia. Captopril should be used with caution especially in premature babies and newborns with underlying renal or renovas…

The value of renography before and after angiotensin converting enzyme inhibition with captopril (captopril renography) as a test for renovascular hypertension was studied in fourteen hypertensive patients. The captopril renography was performed with 99mTc-DTPA by means of a gammacamera, allowing determination of single kidney glomerular filtration rate (SKGFR). In all patients determination of renal vein plasma renin concentration and renal angiography were carried out. Eleven patients showed an elevated unilateral renin secretion. All of these had a significant decrease of SKGFR in one or bo…

Chronic effects of captopril were studied in 29 patients (age, 4 months to 16 years; mean, 6.9 years) suffering from digitalis and diuretic resistant congestive heart failure (CHF) or hypertension of different etiology. Twenty two patients with CHF (13 dilated, 4 restrictive cardiomyopathy, 5 congenital heart defects) and 7 cases with hypertension were treated for 1 to 31 months (mean, 9 months). The dose of captopril varied from 1 to 3 mg/kg/day (mean, 2.2 mg) in CHF and from 1.1 to 6.8 mg/kg/day (mean, 3.7 mg) in hypertension. In CHF digoxin therapy was maintained while the dose of diuretics…

Six patients maintained on 50-100 mg captopril, for 2-25 months, were administered captopril 50 mg orally, together with either naloxone or 0.9% saline vehicle (placebo) given intravenously, in a double-blind crossover study. Naloxone did not appear to modify the circulatory effects of captopril in these patients, in contrast to earlier findings after acute captopril administration in normotensives. The results do not support an important endogenous opioid role in the chronic antihypertensive effect of captopril, but provide evidence that different mechanisms may contribute to the early short …

Three patients with Takayasu's disease (TD), initially observed with hypertension and/or congestive heart failure, were treated with captopril, an orally active inhibitor of converting enzyme. As arteritis in this disease is diffuse and extensive, surgical correction of renovascular hypertension is often delayed or impossible. Our experience enables us to recommend captopril for conservative treatment in TD.

The fall in blood pressure produced by captopril is not accompanied by a compensatory tachycardia. Angiotensin converting enzyme may participate in the metabolism of endogenous opioids and these substances can lower blood pressure without reflex tachycardia. It is therefore possible that the hypotensive effect of captopril involves an action on endogenous opioid metabolism. The influence of naloxone, an opiate receptor antagonist, on the cardiovascular effects of captopril has been studied in nine normal volunteers. Mean blood pressure following a 70 degrees head up tilt 1.5 h after captopril …

Several vasodilating agents have been used for the treatment of primary pulmonary hypertension (PPH). However, no effective therapy is currently available for this distressful disease. We tried to use an angiotensin-I converting enzyme inhibitor, captopril, for one woman suffering from PPH. Her initial treatment with furosemide, digoxin and spironolactone had shown little effect. Further administration of captopril improved her clinical condition from NYHA IV to II and made it possible to treat her at our out-clinic. No adverse effect has been found. We would like to appreciate captopril as on…

1. The effects of a single 25 mg oral dose of captopril on blood pressure, heart rate and circulating renin, angiotensin I, angiotensin II, bradykinin and catecholamine levels were examined in untreated patients with essential (n = 10, Group I), accelerated (n = 6, Group II) and renal hypertension (n = 8, Group III) studied on a normal sodium diet. 2. Mean blood pressure fell only slightly in Group I patients, (113 +/- 3 to 109 +/- 3 mmHg at 60 minutes) but a greater fall was observed in Group II (153 +/- 8 to 135 +/- 11 mmHg) and a marked fall in Group III, (136 +/- 3 to 114 +/- 5 mmHg). Ther…
