Pediatric cardiology in the 1990s.

OBJECTIVES: In 1991, all active board-certified pediatric cardiologists were polled by questionnaire to examine the relation of subspecialty … on, clinical resources, career security and clinical autonomy. CONCLUSIONS: The field of pediatric cardiology is a subspecialty centered on patient care and performance of diagnostic and interventional techniques. Professional activities varied according to practice site. Pediatric cardiologists with basic research training and professional activity remain …

[Pediatric cardiological emergencies].

Emergencies in pediatric cardiology are heart failure, cyanosis and rhythm disturbances. The signs of heart failure are tachycardia, tachypnea and hepatomegaly. The therapy consists of oxygen, diuretics and digoxin. Occasionally, intubation with mechanical ventilation and intravenous catecholamines are needed. Cyanosis is often the only sign of a severe heart malformation, and prompt hospitalization is mandatory. Oxygen and warm environment is important during transport, correction of a possible metabolic acidosis and prostaglandin infusion are done in the hospital. Beyond the newborn period, …

[Risk of cardiological interventions].

The risk of cardiac interventions has to be assessed in relation to the possible benefit and in comparison to the natural history and to alternative interventions. The relationship between complications (mortality, morbidity) and age, severity of the disease, gender and technique on one hand, and between risk of the intervention versus risk of uninfluenced natural history on the other, are discussed. Diagnostic, therapeutic and prophylactic interventions are presented separately; the benefit needs to be defined differently for each: diagnostic accuracy or symptom relief, improved quality of li

[The value of dobutamine in cardiology].

The hemodynamic effects of dobutamine have been evaluated by catheterization for a limited duration in 17 patients. The cardiac index, the systolic index, the myocardial contractility given by the values dp/dt and Vmax, and the systolic work index have increased significantly while the increase in the mean aortic pressure and the cardiac rate were small. The left ventricular telediastolic pressure, and systemic and pulmonary arterial resistance diminished significantly. These results seen to authorise the prolonged use of this drug in the low output syndromes with left ventricular failure whet

Ultra-processed foods, lifestyle management, and cardiovascular diseases: A clinical consensus statement of the European Society of Cardiology Council for Cardiology Practice and the European Association of Preventive Cardiology of the European Society of Cardiology.

Ultra-processed foods (UPFs) have increasingly displaced traditional diets globally and have become a significant public health concern, par … esity) and adverse CV outcomes, the role of food processing in CV health remains underrecognized in cardiology. Current dietary counselling in clinical practice tends to overlook the potential adverse impact of UPFs, with patients not receiving comprehensive nutritional guidance. This European Society of Cardiology (ESC) clinical consensus statement, develop …

The cancer patient and cardiology.

Advances in cancer treatments have improved clinical outcomes, leading to an increasing population of cancer survivors. However, this success is associated with high rates of short- and long-term cardiovascular (CV) toxicities. The number and variety of cancer drugs and CV toxicity types make long-term care a complex undertaking. This requires a multidisciplinary approach that includes expertise in oncology, cardiology and other related specialties, and has led to the development of the cardio-oncology subspecialty. This paper aims to provide an overview of the main adverse events, risk assess…

[The clinical examination in cardiology].

The main manifestations of cardiac diseases are dyspnea, chest pain, palpitations, giddiness and syncope. A careful evaluation of subjective symptoms during history taking allows a rapid identification of an ischaemic heart disease, heart failure or cardiac arrhythmias. The additional clinical findings during bedside examination by inspection of the jugular veins, palpation of cardiac impulses and auscultation of heart sounds and murmurs are often sufficient to diagnose most of the frequent underlying disorders, such as valvular diseases, heart failure or pulmonary hypertension. The clinical a

A history of cardiology in Jamaica.

The history of cardiology in Jamaica is conveniently considered in decades beginning in the 1950s. The decade of the 1950s was characterized by early descriptions of the pattern of cardiac disease in adults and children in Jamaica, the establishment of a cardiac clinic at the University Hospital of the West Indies and early cardiac surgical landmarks. Extensive preparatory experimental work in the canine laboratory with respect to cardiopulmonary bypass in the early to mid-1960s culminated in the successful completion of the first open heart surgical procedure in April, 1968. Cardiac catheteri…

[Syncope as a cardiologic problem].

Syncope is a frequent clinical event. It is mainly caused by a suddenly reduced cerebral blood flow. There are two reasons for sudden cerebral underperfusion: cardiogenic - associated with cardiac disorders and neurocardiogenic - resulting from a sudden fall of arterial blood pressure due to impaired autoregulation of the circulation. Cardiogenic syncopes prevail in cardiac diseases associated with impaired blood flow and cardiac arrhythmias. They develop in aortic stenosis, hypertrophic cardiomyopathy, atrial myxoma, myocardial infarction, pulmonary embolism, cardiac tamponade. Cardiac arrhyt
