In vitro engineering of cartilage.

Because adult human cartilage shows poor capacity for repair and regeneration, innovative solutions are required for congenital and acquired degenerative cartilage lesions. Acquired lesions occur in young and old alike, the former being more at risk for sports-related injuries and the latter for age-related degenerative changes. Because cartilage is a relatively simple tissue with respect to its cellular homogeneity and avascularity, it has been a model for research of in vitro engineered tissues. Progress has been slow and obstructed on several levels. The adult chondrocyte has limited capaci…

The clones of osteoarthritic cartilage.

We studied the early cartilage changes in osteoarthritis, examining the most normal appearing articular cartilage from the hips of 17 patients. Normal appearing cartilage from five patients treated for fractures was used as control material. Two different types of clone were found. The first had increased staining for proteoglycan and was thought to have been engaged in the synthesis of matrix. The other type was associated with a severe deficiency of proteoglycan, matrix streaks and evidence of degradation and phagocytosis of matrix components. Immunohistochemistry demonstrated large amounts …

The pathology of cartilage in chondrodysplasias.

The pathology of four types of chondrodysplasias, viz., type II achondrogenesis, thanatophoric dwarfism, Saldino-Noonan syndrome, and chondrodysplasia punctata were studied. In each of these disorders, cells with features similar to the chief and dark chondrocytes of normal hyaline cartilage were seen to be altered in different ways. There was a total absence of chief cells in type II achondrogenesis. All the chondrocytes present were of one variety at different states of maturation, with the fully matured cell having features of dark chondrocytes. The absence of chief cells was associated wit…

Cartilage Homeostasis and Osteoarthritis.

Healthy limb joints are important for maintaining health and attaining longevity. Endochondral ossification (the replacement of cartilage with bone, occurring during skeletal development) is essential for bone formation, especially in long-axis bones. In contrast to endochondral ossification, chondrocyte populations in articular cartilage persist and maintain joint tissue into adulthood. Articular cartilage, a connective tissue consisting of chondrocytes and their surrounding extracellular matrices, plays an essential role in the mechanical cushioning of joints in postnatal locomotion. Osteoar…

Imaging update on cartilage.

In this update article, we present a review of the literature regarding the physiology of the articular cartilage, role of MR imaging in cartilage assessment, MRI sequences and protocols for cartilage imaging, brief overview of classifications and nomenclature for chondral and osteochondral lesions, MR imaging following cartilage repair and degenerative osteoarthritis.

Stapes stabilizing cartilage graft.

PURPOSE: This study aimed to evaluate hearing results in patients with type 3 tympanoplasty using autologous cartilage grafts. METHODS: The study included patients treated with "stapes stabilizing cartilage graft" (SSCG) and Plastipore partial ossicular replacement prosthesis (PPORP) for hearing reconstruction. Hearing results and complications were assessed and postoperative audiological tests were performed at least 6 months after surgery. RESULTS: There were 18 patients (5 men, 13 women) in the SSCG group and 12 patients (5 men, 7 women) in the PPORP group. The air conduction threshold ch…

Microfracture of Articular Cartilage.

Microfracture is a treatment option for symptomatic, full-thickness cartilage defects. Microfracture is most likely to be successful when performed in nonobese patients under the age of thirty years for small (<2 to 4-cm2) femoral condylar defects that have been symptomatic for a short time (less than twelve to twenty-four months). Microfracture has acceptable short-term clinical results, but results can be expected to decline over time. Long-term studies that compare microfracture with advanced cartilage restoration techniques are required to ascertain whether these newer techniques provide l…

Endoscopic butterfly cartilage myringoplasty.

CONCLUSION: Based on the results of this study, it is believed that, in appropriate patients with tympanic membrane perforation, the endoscopic butterfly cartilage myringoplasty can be applied, with a shorter operation time, high graft success rate, and low risk of complications. OBJECTIVE: The aim of this study was to evaluate the results of the endoscopic butterfly cartilage myringoplasty in terms of the graft success rate and hearing gain. METHODS: Forty-five ears of the 42 patients who were subjected to endoscopic butterfly cartilage myringoplasty surgery between January 2013 and Decembe…

Imaging of articular cartilage.

We tried to review the role of magnetic resonance imaging (MRI) in understanding microscopic and morphologic structure of the articular cartilage. The optimal protocols and available spin-echo sequences in present day practice are reviewed in context of common pathologies of articular cartilage. The future trends of articular cartilage imaging have been discussed with their appropriateness. In diarthrodial joints of the body, articular cartilage is functionally very important. It is frequently exposed to trauma, degeneration, and repetitive wear and tear. MRI has played a vital role in evaluat…

Pediatric cartilage interleave tympanoplasty.

OBJECTIVE: To describe a novel technique of cartilage tympanoplasty, and review surgical and hearing results in children. STUDY DESIGN AND SETTING: Retrospective chart review of all patients who had undergone tympanoplasty at a pediatric tertiary care hospital from August 2002 to July 2005. Forty-two patients were identified with a minimum follow-up time of 12 months. RESULTS: Mean preoperative perforation size was 21.3 percent (range 10%-90%), and mean patient age was 7.9 years (range 3-16 years). Median clinical follow-up was 24 months. Tympanic membrane closure and graft integration were …

[Transcanal butterfly cartilage tympanoplasty].

OBJECTIVES: This study evaluated the long-term results of transcanal butterfly cartilage tympanoplasty technique in adult patients. PATIENTS AND METHODS: Transcanal butterfly cartilage tympanoplasty was performed in 11 adult patients (7 males, 4 females; mean age 30 years; range 21 to 45 years) with chronic otitis media. The operations were performed under general or local anesthesia. Perforation diameters ranged from 2 to 6 mm. The mean follow-up period was 9.5 months. RESULTS: At the end of the follow-up period, perforations closed in 10 patients (90.9%). In one patient, reperforation occu…

Cartilage viability after trochleoplasty.

Trochleoplasty is an established and accepted technique for the treatment of patellar instability because of a missing trochlear groove. In this technique, a flap of cartilage over the trochlea is carefully removed and a new trochlear groove is created in the underlying bone before the cartilaginous flap is reattached with sutures. The mid-term clinical and radiological results of this operation are promising but no information about the viability of the reattached cartilage has been reported. To evaluate cartilage viability and quality after trochleoplasty and to verify the healing process, t…

[Meniscus and cartilage damage].

It is well accepted that those meniscus lesions causing an internal derangement of the knee joint lead to chondromalacia. An operation is necessary to prevent progressive condylar cartilage damage. Menisci with minor traumatic tears and mild degeneration protect the joint surface. A removal is rarely indicated. The complete meniscectomy leads to a degenerative arthrosis and should be avoided. Partial meniscus removal minimizes subsequent degenerative changes.

Nasal tip cartilage grafts.

Grafts of autogenous cartilage were utilized in 88 patients to augment the nasal spine, to add definition to the columella, or to support the feet of the crurae when the tip is raised. Complications were uncommon. This technique is an extremely useful adjunct when changing tip profile.
