Geospatial Reach of the Maryland COVID-19 School Meals Response: Spring 2020.

OBJECTIVE: Examine characteristics of pandemic meal site (n = 602) location and meals served per site in Maryland, Spring 2020, following federal/state waivers for local meal site placement decision-making. METHODS: Using geographic information systems, we connected meal sites to census tract-level data and generated service areas from sites and distances from population-weighted census tract centroids to the closest pandemic meal site. Regression analysis determined associations of census tract pandemic meal site count and meals served per site with socioeconomic and demographic variables.

Using Machine Learning to Identify Social Risk Factors of Hypertension and Diabetes in New York City: Evidence to Support the HealthyNYC Initiative.

BACKGROUND: New York City (NYC) launched the HealthyNYC initiative in 2025, aiming to reduce cardiovascular disease and diabetes deaths by 5% by 2030. Effective place-based interventions require evidence to address social risk factors associated with hypertension and diabetes. This study aims to identify key social determinants of health (SDoH) associated with variation in hypertension and diabetes prevalence across census tracts in NYC. METHODS: This retrospective cohort study analyzed clinical data on 3.2 million NYC residents integrated with SDoH data at the census tract level. Using Extre

Community characteristics associated with HIV risk among injection drug users in the San Francisco Bay Area: a multilevel analysis.

Community characteristics have been associated with racial and ethnic health disparities for a wide range of ailments and conditions. Previous research has found that rates of AIDS cases among injection drug users (IDUs) vary by community characteristics. However, few studies have examined whether community characteristics are associated with HIV risk behaviors among IDUs. To address this gap in the literature, we examined the associations between census-tract-level community characteristics and injection-related and sex-related HIV risk behaviors among IDUs in the San Francisco Bay Area. Indi

Influence of a Pandemic on an Epidemic: A Population-Based Ecological Study of the Association Between COVID-19 Diagnoses and Drug Overdose Deaths in Rhode Island, 2023.

OBJECTIVES: In Rhode Island, drug overdose deaths increased by 28% in the first 6 months of the COVID-19 pandemic in 2020 as compared with the previous year (2019), mirroring national trends. We explored how the spatial distribution of overdose deaths overlapped with that of COVID-19 cases to identify levels and increased prevalence of these health issues among census tracts in Rhode Island. METHODS: We used data from the Rhode Island Department of Health and the US Census Bureau to calculate annualized COVID-19 case rates (from March 20, 2020, through December 31, 2021) and unintentional ove

Identifying High-Risk Cardiac Arrest Neighborhoods Using Social Vulnerability Index and Geospatial Analysis.

BACKGROUND: Identifying high-risk locations for out-of-hospital cardiac arrest (OHCA) is essential for targeted community interventions. We aimed to use the Social Vulnerability Index (SVI) and geospatial mapping of OHCA data to locate high-risk neighborhoods in Birmingham, Alabama, for community training programs. METHODS: A retrospective observational analysis was conducted using nontraumatic OHCA cases in adults (≥18 years) in Birmingham from January 1, 2020 to December 31, 2023. Census tracts served as proxies for neighborhoods. A 5-step process identified high-risk tracts: (1) OHCA incid

Neighborhood Deprivation and Neural Tube Defects.

BACKGROUND: Individual measures of socioeconomic status (SES) have been associated with an increased risk of neural tube defects (NTDs); however, the association between neighborhood SES and NTD risk is unknown. Using data from the National Birth Defects Prevention Study (NBDPS) from 1997 to 2011, we investigated the association between measures of census tract SES and NTD risk. METHODS: The study population included 10,028 controls and 1829 NTD cases. We linked maternal addresses to census tract SES measures and used these measures to calculate the neighborhood deprivation index. We used gen

Quantification of Neighborhood-Level Social Determinants of Health in the Continental United States.

IMPORTANCE: An association between social and neighborhood characteristics and health outcomes has been reported but remains poorly understood owing to complex multidimensional factors that vary across geographic space. OBJECTIVES: To quantify social determinants of health (SDOH) as multiple dimensions across the continental United States (the 48 contiguous states and the District of Columbia) at a small-area resolution and to examine the association of SDOH with premature mortality within Chicago, Illinois. DESIGN, SETTING, AND PARTICIPANTS: In this cross-sectional study, census tracts from

Identifying Communities at Risk for Poor Health using Multidimensional vs. Unidimensional Neighborhood Disadvantage Indices.

BACKGROUND: Neighborhood disadvantage indices used in public health research and policy include multiple economic, social, and housing items. However, research has failed to question whether it is necessary to include a multitude of economic, social, and housing variables in a single index. The purpose of this work was to examine three different neighborhood indices: a multidimensional disadvantage index, a unidimensional disadvantage index, and a unidimensional affluence index, and examine their performance with respect to distinguishing between healthy and unhealthy census tract neighborhood

Understanding the role of violence as a social determinant of preterm birth.

BACKGROUND: Preterm birth is one of the leading causes of infant morbidity and mortality. Although major strides have been made in identifying risk factors for preterm birth, the complexities between social and individual risk factors are not well understood. OBJECTIVE: This study examines the association between neighborhood youth violence and preterm birth. STUDY DESIGN: A 10-year live birth registry data set (2004 through 2013) from Richmond, VA, a mid-sized, racially diverse city, was analyzed (N = 27,519). Data were geocoded and merged with census tract and police report data. Gestation

Associations of four indexes of social determinants of health and two community typologies with new onset type 2 diabetes across a diverse geography in Pennsylvania.

Evaluation of geographic disparities in type 2 diabetes (T2D) onset requires multidimensional approaches at a relevant spatial scale to characterize community types and features that could influence this health outcome. Using Geisinger electronic health records (2008-2016), we conducted a nested case-control study of new onset T2D in a 37-county area of Pennsylvania. The study included 15,888 incident T2D cases and 79,435 controls without diabetes, frequency-matched 1:5 on age, sex, and year of diagnosis or encounter. We characterized patients' residential census tracts by four dimensions of s

Gentrification, Racial Demographics, and Indicators of Community Conditions in Allegheny County.

Gentrification is a process of neighborhood change characterized by capital investment in historically disinvested neighborhoods and an influx of residents of higher socioeconomic status. While gentrification generates neighborhood improvements that benefit newcomers, it has been recognized as a public health issue characterized by inequities in housing, economic, and health opportunities for long-term residents, specifically low-income and Black populations, who are at high risk of physical and social displacement. Thus, this cross-sectional ecological study based in Allegheny County, Pennsyl

Geographic Differences and Social Determinants of Health Among People With HIV Attributed to Injection Drug Use, United States, 2017.

OBJECTIVE: People who inject drugs are among the groups most vulnerable to HIV infection. The objective of this study was to describe differences in the geographic distribution of HIV diagnoses and social determinants of health (SDH) among people who inject drugs (PWID) who received an HIV diagnosis in 2017. METHODS: We used data from the National HIV Surveillance System (NHSS) to determine the counts and percentages of PWID aged ≥18 with HIV diagnosed in 2017. We combined these data with data from the US Census Bureau's American Community Survey at the census tract level to examine regional,

Pharmacy deserts and health inequities in St. Louis City: a geospatial analysis of access disparities and determinants.

BACKGROUND: Pharmacy deserts, or areas lacking convenient access to pharmacies, can limit access to medications and reduce adherence. In U.S. cities, pharmacy deserts are disproportionately concentrated in racial/ethnic minority and low-income neighborhoods. This study examined the demographics and spatial distribution of pharmacy deserts in St. Louis City, Missouri, and their associations with sociodemographic and healthcare access determinants. METHODS: Pharmacy deserts were defined as census tract block groups whose centroids were more than 1 mile from a pharmacy, or more than a half mile a

Electricity inaccessibility across historically redlined and present-day disadvantaged areas in New York City.

BACKGROUND: Electricity is crucial in sustaining livelihoods from turning the lights on at night, keeping the refrigerator running to avoid food spoilage, and powering electricity-dependent durable medical equipment such as nebulizers. Thus, electricity inaccessibility may result in adverse outcomes. Like other environmental burdens, electricity inaccessibility may be socially patterned, with disproportionate occurrence in racially and economically marginalized communities. OBJECTIVE: To evaluate the 2017-2019 distribution of electricity inaccessibility - defined as power outages and energy i

When more is less: Urban disparities in access to surgical care by transportation means.

BACKGROUND: Structural factors limiting access to surgical care require elucidation. We hypothesize transportation time to hospitals with surgical capacity disproportionately burdens minority populations. METHODS: We identified hospitals with surgical capacity within a 20-mile radius of our city center. Using geocoding, we estimated travel times from each census tract to the nearest facility by car or public bus. RESULTS: For 143 tracts within the county, drive time was 13 ± 4 min and bus time was 33 ± 15 min. Only 41.2% of the population had a facility within 30 min by bus; access was furth

Pharmacy access and shingles vaccinations in the US: a propensity score matching analysis.

IMPORTANCE: Community pharmacists provide many important healthcare services, including routine adult vaccinations. However, an estimated 15.8 million people in the US live in pharmacy deserts and may lack access to these services. The relationship between pharmacy deserts and adult vaccine receipt has yet to be thoroughly explored empirically. OBJECTIVE: We evaluated the relationship between census tract-level pharmacy access and shingles vaccination receipt. DESIGN, SETTING, AND PARTICIPANTS: This propensity score matched analysis used 2022 vaccination data from seven collaborating State D

The spatiotemporal distribution of pre-exposure prophylaxis accessibility in the United States, 2016-2020.

PURPOSE: Residing in areas with little spatial accessibility to HIV pre-exposure prophylaxis (PrEP) providers, or PrEP deserts, contributes to low PrEP uptake. This study examines and characterizes the spatial distribution of PrEP accessibility in the United States over time. METHODS: We conducted spatial network analyses and geographic mapping to explore the spatiotemporal distribution of persistent PrEP deserts (census tracts with suboptimal accessibility in 2016 and 2020), new PrEP deserts (tracts with suboptimal accessibility in 2020 but not 2016), new PrEP oases (tracts with suboptimal a

A comparison of cardiometabolic risk factors in households in rural Uganda with and without a resident with type 2 diabetes, 2012-2013.

INTRODUCTION: Colorectal cancer (CRC) incidence and mortality rates are disproportionately high in rural and Appalachian communities, where geographic barriers and specialist shortages may limit access to colonoscopy. METHODS: We conducted a national cross-sectional geospatial analysis of all contiguous US census tracts. We identified colonoscopy providers by using National Provider Identifier specialty designations and colonoscopy procedure indicators. Geographic access was quantified by using a 4-band enhanced 2-step floating catchment area (E2SFCA) method. We used small-area estimation for
