BACKGROUND: Hip revision surgery for fractured ceramic components may represent the worst-case wear scenario due to the high risks of recurrent dislocations, instability, and third body wear. The ideal bearing choice for the new prosthetic articulation is still subject of debate, while alumina matrix composite (AMC) articulations offer theoretical superior performances; the present work was designed to test the wear behaviour of ceramic on ceramic articulations (liner and head) in a worst-case scenario by adding ceramic third-body particles to the test lubricant with combined walking and sublu

Total hip arthroplasty (THA) in the adolescent patient has historically shown relatively poor survivorship. This study reports the results of THA in young patients using contemporary ceramic bearings. Twenty-four THAs were performed using ceramic bearing surfaces in patients 20 years old or younger (mean, 16.4; range, 12-20). Average follow-up was 52 months (range, 25-123 months). The survival rate was 96%, with 1 revision for a loose acetabular component. Other complications included a peroneal nerve palsy that resolved and 2 dislocations in 1 patient. Postoperatively, the Modified Harris Hip

INTRODUCTION: Ceramic-on-ceramic (COC) total hip arthroplasty (THA) was developed to reduce wear debris, and consequently the occurrence of osteolysis and aseptic loosening. The purpose of the present study was to determine the incidence and natural history of noise including squeaking after COC THA, and to clarify whether clinical outcomes and any demographic and implant-related factors are associated with noise from these ceramic bearings.
METHODS: This was a retrospective observational study of 137 primary COC THAs between 2004 and 2009 at our institute. The Mean follow-up was for 10.5 yea

BACKGROUND: Hard bearings, metal on metal (MOM) and ceramic on ceramic (COC), have been developed to overcome polyethylene wear-related osteolysis, but comparisons between these 2 hard bearings are limited. We performed a meta-analysis to determine revision rates for (1) any reason, (2) aseptic loosening, (3) periprosthetic joint infection (PJI), and (4) recurrent dislocation between MOM and COC bearings in cementless total hip arthroplasty.
METHODS: This analysis included 3 studies comparing COC and MOM bearings and 56 studies reporting outcomes of MOM or COC bearings. We conducted a compari

BACKGROUND: Ceramic-on-ceramic (CoC) total hip arthroplasty (THA) has been scrutinised for a high incidence of post-operative squeak. The psychological and social consequences, and subsequent request for revision surgery are not well documented.
METHODS: Seventy consecutive primary CoC THA operated on by a single surgeon at a single institution were enroled and followed for an average 7.1 years. Information was obtained from routine post-operative review, medical records, mailed questionnaire and periodic phone interview.
RESULTS: Of 70 THA, 69 were available for follow-up. Seventeen (24.6 %

The aim of this study was to evaluate the first 100 cementless ceramic-on-ceramic total hip arthroplasty (THA) performed at our institution with more than nine years of follow-up. Clinical evaluation was performed using Harris hip score. Radiological evaluation was performed by two surgeons. Four patients were lost to follow-up. Harris hip score significantly improved at latest follow-up. Radiological analysis showed calcar osteolysis for 75 patients and one cup loosening. One patient required a revision five years postoperatively for cup loosening. Based on these results and the routine use o

With an increase of revision total hip arthroplasty (THA), the choice of bearing surface becomes more important. Wear debris by conventional metal-on-polyethylene articulations may cause extensive osteolysis, especially in young patients. We analyzed the clinical and radiographic outcomes after revision THA using third-generation ceramic-on-ceramic bearing surfaces in 42 hips. The mean age of the patients was 48.8 years (32-59 years), and the mean duration of follow-up monitoring was 5.4 years (3.2-8.0 years). At final follow-up examination, the average Harris Hip Score was 91.3. Although mino

BACKGROUND: Total ankle arthroplasty (TAA) has become one of the standard surgical treatments for patients with end-stage ankle osteoarthritis (OA). In recent years, TAA with total talar prosthesis (combined TAA) has been used for ankle OA patients with talar osteonecrosis, with severe talar collapse, or with subtalar joint OA. The purpose of this study was to investigate the functional and clinical outcomes of combined TAA, compared with conventional TAA.
METHODS: Forty-six ankles (44 patients, 32 women/12 men) that underwent conventional TAA with mean follow-up of 42 months, and 26 ankles (

PURPOSE: It is unclear whether late THA periprosthetic femoral fractures are related to a mechanical mechanism that decreases strength of the femur (for example, loosening) or to a biological problem as osteolysis. It is also unknown if ceramic on ceramic bearing couples decrease the risk of late periprosthetic fractures as a result of the absence of wear and osteolysis.
MATERIAL AND METHODS: We therefore asked whether the cumulative long-term fractures were different according to the couple of friction ceramic on ceramic or ceramic on polyethylene in 327 patients (654 hips) with bilateral TH

INTRODUCTION: The outcomes of using a ceramic-on-ceramic (CoC) vs ceramic-on-polyethylene (CoP) bearing surface for primary total hip arthroplasty (THA) are much debated. We performed a systematic review and meta-analysis of randomised controlled trials (RCTs) to compare and evaluate the outcomes of these 2 bearing surfaces.
MATERIALS AND METHODS: Electronic databases (PubMed/Medline, CENTRAL, Embase and Web of Science) were systematically searched for RCTs investigating the association between bearing surfaces and outcomes following primary THA. Two investigators independently reviewed studi

AIMS: Ceramic-on-ceramic (CoC) articulations in total hip arthroplasty (THA) have the advantage of low wear but the unique risk of fracture. After revision for CoC-bearing fracture, third-body ceramic particles can lead to massive wear of cobalt-chrome (CoCr) bearings, causing extremely elevated blood cobalt. We present a multicentre series of five cases where patients sustained fractures of ceramic liners, were revised using a CoCr articulation, and rapidly developed severe cobalt toxicity.
METHODS: We identified five cases of cobalt toxicity after fractured CoC THA treated with revision to

BACKGROUND: The surgical strategy after fractured ceramic bearings in total hip arthroplasty is still debated. A retrospective case series of prospectively followed ceramic-on-ceramic (COC) revisions after ceramic component fracture was selected, aiming to assess: (1) complications and survival rates, (2) clinical outcomes and noise incidence, and (3) radiological outcomes.
METHODS: There were 36 patients included (all COC total hip arthroplasty). In 83.3% of the patients, an alumina (Forte) component failed: 22 liners, 12 heads (two cases with liner and head failure). In 16.7% of the patient
