The aim of this review is to elucidate the communications between the facial nerves or facial nerve and neighboring nerves: the vestibulocochlear nerve, the glossopharyngeal nerve, and the cervical plexus. In a PubMed search, 832 articles were searched using the terms "facial nerve and communication." Sixty-two abstracts were read and 16 full-text articles were reviewed. Among them, 8 articles were analyzed. The frequency of communication between the facial nerve and the vestibulocochlear nerve was the highest (82.3%) and the frequency of communication between the facial nerve and the glossophar…

Cervical ribs are uncommon and occur in approximately 0.5-1% of the population. The authors describe a cadaver found to have a cervical rib with further compression by the dorsal scapular artery. This vessel originated from the third portion of the subclavian artery. No other anomalies were found in this specimen. The authors speculate that some cases of symptomatic cervical rib may have an additional vector of compression from a dorsal scapular artery when it arises from the third portion of the subclavian artery. Additional studies aimed at the vascular system may be useful in diagnosis and

The aim of our study was to evaluate the relationship between shoulder pain and damage to the cervical plexus after neck dissection. The study was performed prospectively on 34 neck sides of 17 patients with laryngeal cancer who underwent laryngectomy plus bilateral selective neck dissection (II, III, IV, +/- VI) at the Department of Otorhinolaryngology of Uludag University between December 2003 and October 2004. The cervical plexus was protected on one side of the neck and sacrificed on the other, while the accessory nerve was spared on both sides. The degree of sensorial innervation of the c…

Ludwig's angina is defined as a potentially lethal, rapidly spreading cellulitis, involving the sublingual and submandibular spaces, and is manifested by a brawny suprahyoid induration, tender swelling in the floor of the mouth, and elevation and posterior displacement of the tongue. During a life-threatening infection such as Ludwig's angina, the mother and foetus are vulnerable to septicemia and asphyxia. We describe a case of decompression of Ludwig's angina in a 28 weeks pregnant patient under bilateral superficial cervical plexus block. The block, coupled with bilateral mandibular nerve b…

OBJECTIVE: To calculate dose delivered to the lumbosacral plexus (LSP) with cervical brachytherapy using 3-dimensional imaging, and to compare this with the position of the tandem in the pelvis using bony landmarks. We also report long-term LSP toxicity outcomes.
METHODS AND MATERIALS: Treatment planning images from 55 patients treated with tandem and ring brachytherapy from October 2009 through November 2012 were reviewed. The LSP was contoured on planning computed tomographic scans to calculate dose received. Lumbosacral plexus dose was studied as a function of tandem distance from the sacr

BACKGROUND: Whether adding a bilateral superficial cervical plexus block to a thyroidectomy enhanced recovery pathway improves postoperative quality of recovery remains uncertain.
METHODS: In a single-center prospective, randomized, double-blind, placebo-controlled trial in adults undergoing thyroidectomy with general anesthesia, participants were randomized to bilateral superficial cervical plexus blocks with 0.25% bupivacaine or saline. All participants received multimodal analgesia with dexamethasone, acetaminophen, nonsteroidal anti-inflammatory drugs, and incisional local anesthetic. The…

Post-craniotomy pain is common yet often sub-optimally managed because systemic opioids can obscure postoperative neurologic examinations. The superficial cervical plexus block (SCPB) has, therefore, emerged as a targeted regional anesthesia option for occipital craniotomies. The SCPB targets the C2-C4 nerves to anesthetize the occipital scalp region, covering the lesser occipital nerve territory that lies within typical posterior scalp incisions. Clinical evidence shows the block is effective in reducing acute postoperative pain after occipital craniotomy and diminishes opioid requirements. S…

BACKGROUND: The ultrasound-guided superficial cervical plexus nerve block (SCPNB) is a well-established technique for analgesia for surgical procedures involving the anterolateral neck and it has been increasingly used in the emergency department (ED) for several indications such as neck abscesses, clavicle fractures, and auricular laceration repair. There is, however, limited evidence for its use in patients presenting to the ED with musculoskeletal neck pain.
METHODS: This is a case series of 5 patients who presented to the ED with neck pain and were diagnosed with either musculoskeletal ne…

BACKGROUND: The brachial plexus block is conducive to providing postoperative analgesia for patients with humeral fractures. The commonly used brachial plexus block techniques have a high incidence rate of hemidiaphragmatic paralysis (HDP), which may lead to respiratory problems. The combined costoclavicular brachial plexus - cervical plexus blocks (CCB-CPBs) had demonstrated favorable analgesic effects and had reduced the incidence of HDP in shoulder surgeries. However, the clinical experience regarding CCB-CPBs is rather limited. Consequently, our study aims to evaluate its various effects, …

BACKGROUND: Anterior cervical discectomy and fusion (ACDF) is a commonly performed procedure for cervical disc disease; however, significant postoperative pain may hinder recovery and increase opioid consumption. This study compared the efficacy of cervical erector spinae plane block (cESPB) and superficial cervical plexus block (sCPB) techniques in ACDF.
METHODS: This prospective, randomized study included American Society of Anesthesiologists (ASA) class I-II patients aged 18 to 65 years who underwent ACDF surgery for cervical (C)5-C6 or C6-C7 disc disease. Patients were randomly assigned t…

BACKGROUND: The role of type I thyroplasty (TIP) is well established as the treatment for glottal insufficiency due to vocal fold paralysis, but the ideal anesthetic management for this procedure is still largely debated. We present the case of a novel anesthetic approach for TIP using combined intermediate and superficial Cervical Plexus Block (CPB) and intermittent mild sedation analgesia.
CASE REPORT: A 51-year-old presenting with left vocal fold paralysis and obstructive sleep apnea was scheduled for TIP. An ultrasound-guided intermediate CPB was performed using the posterior approach, an…

RATIONALE: Bilateral brachial plexus block (BPB) generally requires a relatively large dose of local anesthetic for a successful block, resulting in a high risk of local anesthetic systemic toxicity. It can also result in inadvertent bilateral phrenic nerve palsy, leading to respiratory failure. Hence, it has not been widely used. However, it can be performed in selected patients. In this report, we present a case of ultrasound-guided BPB for bilateral upper extremity surgery in a patient with cervical spinal cord injury (SCI).
PATIENT CONCERNS: A 25-year-old woman with SCI secondary to traum

INTRODUCTION: The cervical surgery can be complicated of postoperative facial sensory deficits, in particular in using the anterior presternocleidomastoid approach. The purpose of this study was to specify the routes and the links of nerves involved in these sensory deficits (great auricular nerve and transverse cervical nerve with the goal, to deducing, if possible, some modifications of the surgical practices to prevent the deficits.
PATIENTS: Ten dissections of the superficial cervical plexus, on preserved anatomical subjects, were made from February till May 2009. Nerves and whole superfi…

This case illustrates the rare occurrence of a cystic schwannoma of the neck, presenting clinically and ultrasonographically as a type II second branchial cleft cyst. Histology demonstrated that it was in fact a rare cystic schwannoma, most likely arising from the cervical plexus. This is the first documented case of a cystic schwannoma of the neck being mistaken for a type II second branchial cleft cyst based on clinical and ultrasound findings alone. It suggests that more sophisticated radiological investigations, such as magnetic resonance imaging, may enable accurate diagnosis of a cystic …

AIM: To determine the expression of c-fos in gastric myenteric plexus and spinal cord of rats with cervical spondylosis and its clinical significance.
METHODS: A cervical spondylosis model was established in rats by destroying the stability of cervical posterior column, and the cord segments C(4-6) and gastric antrum were collected 3, 4 and 5 mo after the operation. Rats with sham operation were used as controls. c-fos neuronal counter-staining was performed with an immunohistochemistry method. Every third sections from C(4-6) segments were drawn. The 10 most labeled c-fos-immunoreactive (Fos
