Twin gestations frequently involve maternal and fetal complications, and are quite often considered as "premium" pregnancies. Hence, many clinicians that follow the cliche "no high risk pregnancy should end with a high risk delivery" may deliver twins by CS for many subtle reasons, other than clear-cut, evidence-based, indications. Thus, the decision for a CS in twins, intentionally or not, is based on qualitative variables that were not quantified and on quantitative variables that suggest no advantage for CS in the majority of cases.

Performing a computerized multicenter-study of 66 clinics in the state of North-Rhine-Westfalia concerning the management of emergency caesarean-section (ECS), the resulting basic data were calculated as follows: In 207 cases of ECS studied, the time period needed from the decision for operation on to the beginning of surgery (DFOS) is on the average 20.4 +/- 12.3 minutes (Median 17 min., 90th Percentile 37 min.). A prospective multicenter study of 1,032 cesarean sections was performed to identify risk factors for postoperative wound infection. The overall rate of wound infection was 6.6% (3.8% in elective cases and 7.5% following nonelective operations), with considerable interhospital variation. It must be kept in mind that there are few situations that demand immediate surgery. Regarding indications for most caesarean sections, there is enough time for the anaesthesist to administer speedy, but nevertheless orderly premedication and judge the indications for anaesthesia. The anaesthetic technique should be determined in agreement with the obstetrician. In most hospitals, general anaesthesia can probably be performed more quickly than intrathecal or extrathecal anaesthetics. However, this article will not deal with the details involved in these methods.

An IUD (TCu220C) was inserted in 82 women during low-transverse cesarean section. No untoward effect on puerperal morbidity or lactation was observed and no serious complications occurred in this series. At 12 months the rates were zero for pregnancy and 7.7 for expulsion; the latter figure is comparable to that reported after immediate postplacental insertion of the same IUD model. Intracesarean IUD insertion is a procedure that deserves further promotion.

We had 65 Caesarean hysterectomies from 1960-1982 at our hospital. 43 patients were elected for various reasons: contraception 26, fibromatosis 13, intraepithelial neoplasia 4. 22 had vital indications: rupture of the uterus 12, placenta increta/percreta 5, abruptio placentae 3, atony 2. Cecal volvulus following cesarean section is rare, with only 10 previous cases reported in the literature. A study was undertaken to evaluate the postoperative morbidity of classical cesarean section (CCS). Eighty-nine such procedures, performed between 1970 and 1977, were reviewed. All cases in which prophylactic antibiotics were used were excluded from the study. Forty-seven patients (53%) developed standard morbidity, and 46 patients (52%) had a clinical diagnosis of infection. Total midgut volvulus and gangrene due to a congenitally elongated mesentery in a 27-year-old primipara occurred 3 days after cesarean section. All of the small bowel distal to the duodenum except for 12 inches of jejunum was resected, and the patient survived. This case and the literature concerning volvulus in pregnancy are reviewed here, and suggestions for prevention and treatment are made.
