Timing of percutaneous cholecystostomy tube removal: systematic review.

INTRODUCTION: Percutaneous cholecystostomy (PC) is an effective procedure to treat moderate or severe acute cholecystitis (AC) in high-risk patients. The ideal timing of the drainage removal is argued. The aim of this study is to analyze our experience and perform a systematic review about the ideal timing of a percutaneous cholecystostomy (PC) tube removal. EVIDENCE ACQUISITION: A web-based literature search was performed and studies reporting the length of the catheter maintenance were analyzed. A regression analysis between the timing of tube removal and morbidity, mortality and disease re…

Successful laparoscopic cholecystectomy after percutaneous cholecystostomy tube placement.

BACKGROUND: Interval cholecystectomy (IC) after percutaneous cholecystostomy tube (PCT) placement is the definitive treatment for cholecystitis in patients who are operative candidates after optimization of medical comorbidities. It is not clear, however, which patients will be able to have a laparoscopic IC after PCT placement. We aimed to identify factors associated with successful laparoscopic IC in these patients. METHODS: This is a retrospective review of patients who had a PCT from 2009 to 2011. Patient's baseline demographics, clinical data, and outcomes were analyzed. Univariable and …

Percutaneous cholecystostomy for acute cholecystitis: ten-year experience.

PURPOSE: To review the clinical course of patients with acute cholecystitis treated by percutaneous cholecystostomy, and to identify risk factors retrospectively that predict outcome. MATERIALS AND METHODS: A total of 106 patients diagnosed with acute cholecystitis were treated by percutaneous cholecystostomy during a 10-year period. Seventy-one (67%) presented to the emergency department (ED) specifically for acute cholecystitis, and 35 (23%) were inpatients previously admitted for other conditions. Outcomes of the two groups were compared with respect to severity of illness, leukocytosis, b…

Operative tube versus percutaneous cholecystostomy for acute cholecystitis.

The records of 26 patients who underwent cholecystostomy procedures for presumed acute cholecystitis during a 6-year period were reviewed. Nine patients had operative tube cholecystostomy (OC), and 17 patients had radiologic percutaneous cholecystostomy (PC). A correct diagnosis of acute cholecystitis was made in 22 of 26 patients (84%), including 14 of 17 PC patients and 8 of 9 in the OC group. The rate of resolution of cholecystitis was the same in each group (75% OC versus 78% PC). APACHE II scores prior to treatment were significantly higher in OC patients (20.9 OC versus 12.4 PC, p < 0.01…

Management of Percutaneous Cholecystostomy Drains: A Consensus Statement.

IMPORTANCE: Percutaneous cholecystostomy drains (PCDs) are frequently used in patients with acute cholecystitis (AC) who are poor surgical candidates. However, standardized guidance on the indications, follow-up, and management of PCDs is lacking in the literature. OBJECTIVE: To gather expert consensus recommendations and develop guidelines on the management of PCDs in patients with AC. EVIDENCE REVIEW: This Delphi consensus study was conducted between August 2024 and April 2025. Statements were generated following a structured literature review and evaluated through a 3-round Delphi process…

Percutaneous cholecystostomy: prognostic factors and comparison to cholecystectomy.

BACKGROUND: Data regarding long-term outcomes following percutaneous cholecystostomy (PC) are limited, and comparisons to cholecystectomy (CCY) are lacking. We hypothesized that chronic disease burden would predict 1-year mortality following PC, and that outcomes following PC and CCY would be similar when controlling for preprocedural risk factors. METHODS: We performed a 10-year retrospective cohort analysis of patients with acute cholecystitis managed by PC (n = 114) or CCY (n = 234). Treatment response was assessed by systemic inflammatory response syndrome (SIRS) criteria at PC/CCY and 72…

Cholecystostomy Treatment in an ICU Population: Complications and Risks.

BACKGROUND: Percutaneous cholecystostomy tube placement has widely been used as an alternative treatment to cholecystectomy, especially in advanced disease or critically ill patients. Reported postprocedural complication rates have varied significantly over the last decade. The goal of this study is to evaluate the safety of percutaneous cholecystostomy tube treatment in critically ill patients. STUDY DESIGN: We performed a retrospective chart analysis of 96 critically ill patients who underwent cholecystostomy tube placement during an intensive care unit (ICU) stay between 2005 and 2010 in a…

[Method of punction transcutaneous cholecystostomy and device for accomplishment].

The author has developed a mode of punctional transcutaneous cholecystostomy--under ultrasound scanning control and the device for its accomplishment. The mode and the device provide increase of safety of the operation and the possibility of usage of ultrasound scanning without special punctional transducer. The results of clinical application of the developed method and the device in patients with acute cholecystitis are effective.

The efficacy of percutaneous cholecystostomy in critically ill patients.

Percutaneous cholecystostomy (PC) has been proposed as a method of biliary decompression in critically ill patients with acute cholecystitis. We evaluated the efficacy of PC in this setting. The charts of 33 critically ill patients (mean age 52, range 5-87) who underwent PC for suspected acute cholecystitis were retrospectively examined. Univariate analysis was performed to identify which patients might benefit from PC. PC was technically successful in all patients with no direct mortality or major complications. Failure to improve within 24 hours was associated with increased mortality (P = 0…

Percutaneous cholecystostomy in the critically ill burn patient.

OBJECTIVE: We desired to demonstrate the utility of percutaneous cholecystostomy in the evaluation and management of critically ill burn patients with fever and rising cholestatic chemistries. DESIGN: Retrospective review. MATERIALS AND METHODS: Over a 2 1/2-year period there were 411 admissions to a regional adult burn until of whom six patients (1.5%) were strongly suspected of having developed acute cholecystitis. Five were managed with percutaneous cholecystostomy. MEASUREMENTS AND MAIN RESULTS: All patients had sonographic signs consistent with acute cholecystitis. One was managed with…

Percutaneous cholecystostomy treatment of acute cholecystitis in pregnancy.

BACKGROUND: Medical treatment of acute cholecystitis in pregnancy may lead to prolonged management and recurrent hospitalizations, whereas surgical management predisposes the mother and fetus to the inherent risks of surgery and general anesthesia. Although percutaneous cholecystostomy has been proven to be an efficacious treatment in critically ill and general surgery patients who are at high risk for surgery, this technique has not been used routinely as a treatment for acute cholecystitis in pregnancy. CASES: We report two women (at 30 and 32 weeks' gestation, respectively) who presented w…

Indications and limitations of percutaneous cholecystostomy for acute cholecystitis.

Percutaneous cholecystostomy (PC) was used as an alternative to operative therapy in 21 elderly or critically ill patients with suspected acute cholecystitis. All had associated disabling diseases, ten (48 percent) were older than 65 years, nine (43 percent) were in the intensive care unit and eight (38 percent) were recovering from recent operations. Among the 21 patients, 18 had cholecystitis (eight calculous and ten acalculous); in three patients, the procedure was only diagnostic. In 16 of 18 patients with acute cholecystitis, immediate relief of symptoms and significant improvement of lab…

[Percutaneous cholecystostomy guided by ultrasonography. A preliminary experience].

Surgical cholecystostomy is a palliative treatment for cholecystitis and distal biliary obstructions when the general condition of the patient does not allow complex techniques. Percutaneous cholecystostomy (PC) guided by ultrasonography is an alternative to that procedure as well as a method of direct access to the biliary tract for diagnostic examinations (bacteriologic study of bile and percutaneous cholangiography). During one year, 9 female patients, mean age 74 (49 to 90) underwent this approach; 5 patients had cholecystitis, 2 were suspicious of biliary sepsis and 2 had angiocholitis. B…

Long-term effects of cholecystostomy on gallbladder function.

In eight patients without a history of gallbladder disease, cholecystostomy was performed for acute pancreatitis (four patients) and blunt abdominal trauma (four patients). In one case only, acute cholecystitis developed after discontinuation of the cholecystostomy. Six patients were followed for a mean period of 3.9 years, after which the gallbladder function was evaluated. Cholecystography and ultrasonography demonstrated good visualisation of the gallbladder without signs of gallstones. The contraction of the gallbladder produced by cholecystokinin varied. This could be due to adhesions imp…

Cholecystostomy: Its Place in the Treatment of Acute Cholecystitis.

Conservative management of acute cholecystitis has frequently been too prolonged, particularly in the aged. Early cholecystostomy followed by interval cholecystectomy improved the results of treatment as demonstrated in a series of 41 patients at Shaughnessy Veterans Hospital, Vancouver. The following advantages were noted. Advanced disease in the elderly toxic patient was successfully treated by a minor and short surgical procedure, cholecystostomy. Per-cholecystostomy cholangiography revealed dilatation and/or stones in the common bile duct. Interval cholecystectomy was facilitated by the ab…
