The exchange of genes between cells is known to play an important physiological and pathological role in many organisms. We show that circulating tumor DNA (ctDNA) facilitates cell-specific gene transfer between human cancer cells and explain part of the mechanisms behind this phenomenon. As ctDNA migrates into the nucleus, genetic information is transferred. Cell targeting and ctDNA integration require ERVL, SINE or LINE DNA sequences. Chemically manufactured AluSp and MER11C sequences replicated multiple myeloma (MM) ctDNA cell targeting and integration. Additionally, we found that ctDNA may…

BACKGROUND: Pancreatic ductal adenocarcinoma (PDAC) carries high mortality despite neoadjuvant chemotherapy (NAC) and surgical resection, creating an urgent need for prognostic biomarkers to guide treatment. We aimed to determine whether circulating tumor DNA (ctDNA) detected by next-generation sequencing (NGS) predicts survival in patients with localized PDAC.
METHODS: Patients with localized PDAC were prospectively enrolled at an academic institution between October 2020 and November 2024 (NCT04616131). Patients underwent next-generation sequencing (NGS), with blood samples collected at dia…

Circulating tumor DNA (ctDNA), the tumor-derived fraction of cell-free DNA, has emerged as a valuable biomarker for monitoring treatment response, detecting minimal residual disease (MRD), and identifying early cancer recurrence. While histologic tissue diagnosis remains the gold standard for confirming malignancy, guidelines from organizations such as the National Comprehensive Cancer Network (NCCN) and the European Society for Medical Oncology (ESMO) acknowledge that ctDNA may serve as a supplemental tool in rare instances where tissue is unobtainable. In such cases, results should be interp…

INTRODUCTION: In osimertinib-treated EGFR mutation (EGFRm)-positive NSCLC, resistance inevitably occurs. Early resistance mechanism (RM) detection by circulating tumor DNA (ctDNA) in plasma and consecutive targeted treatment may delay progressive disease (PD). In this multicenter prospective study, we evaluated the detection rate and time interval of RM emergence in plasma ctDNA before radiologic PD.
METHODS: Patients with EGFRm-positive NSCLC, treated with second- or third-line osimertinib, underwent computed tomography of the thorax and ctDNA analysis (Roche AVENIO expanded panel, research …

PURPOSE: KRAS is the most mutated proto-oncogene that has been identified in cancer, and treatment of patients with KRAS mutations remains an arduous challenge. Recently, KRASG12C mutation has attracted special interest because it is now considered potentially druggable with recently developed covalent small-molecule KRASG12C inhibitors. Nevertheless, to date, there have been no large-scale analyses of liquid biopsy that include testing for KRASG12C. Here, we performed a comprehensive analysis of KRASG12C mutations in multiple cancer types, as detected by circulating tumor DNA.
METHODS: We co…

Brain metastasis (BM) genetically diverges from the primary tumor in non-small-cell lung cancer (NSCLC). Hence, accurately capturing clinically relevant alterations is pivotal for the delivery of targeted therapies. Circulating tumor DNA (ctDNA) sequencing has emerged as a promising liquid biopsy in the biomarker-based clinical management of recurrent and extracranial metastatic NSCLC. However, the absence of simultaneous sequencing data from brain metastatic sites prevents the definitive evaluation of the efficacy of ctDNA in representing genetic profiles in BM. Here, we performed parallel ge…

PURPOSE: Analysis of circulating tumor DNA (ctDNA) in blood could allow noninvasive genetic analysis of primary tumors. Although there have been unmet needs for noninvasive methods in patients with primary central nervous system lymphoma (PCNSL), it is still not determined whether plasma ctDNA analysis could be useful for patients with PCNSL.
MATERIALS AND METHODS: Targeted deep sequencing of 54 genes was performed in cell-free DNA isolated from plasma samples collected pretreatment, during treatment, and at the end of treatment in 42 consecutively diagnosed PCNSL patients between January 201…

With the addition of molecular testing to the oncologist's diagnostic toolbox, patients have benefitted from the successes of gene- and immune-directed therapies. These therapies are often most effective when administered to the subset of malignancies harboring the target identified by molecular testing. An important advance in the application of molecular testing is the liquid biopsy, wherein circulating tumor DNA (ctDNA) is analyzed for point mutations, copy number alterations, and amplifications by polymerase chain reaction (PCR) and/or next-generation sequencing (NGS). The advantages of ev…

Liquid biopsies (circulating tumor DNA-ctDNA testing) are increasingly being utilized in clinical trials as well as practice for the detection of cancer, monitoring of tumor genomic abnormalities, response to treatment and early detection of relapse/recurrence. Here, we present a challenging case where liquid biopsy was used to confirm an early recurrence of pancreatic cancer where acquisition of tissue was not safe or feasible on more than one occasion.

The development of noninvasive methods to detect and monitor tumors continues to be a major challenge in oncology. We used digital polymerase chain reaction-based technologies to evaluate the ability of circulating tumor DNA (ctDNA) to detect tumors in 640 patients with various cancer types. We found that ctDNA was detectable in >75% of patients with advanced pancreatic, ovarian, colorectal, bladder, gastroesophageal, breast, melanoma, hepatocellular, and head and neck cancers, but in less than 50% of primary brain, renal, prostate, or thyroid cancers. In patients with localized tumors, ctDNA …

The application of circulating tumor DNA (ctDNA) has advanced in the treatment of multiple cancer types, including urothelial cancer (UC). While ctDNA currently plays a limited role in non-muscle-invasive bladder cancer (NMIBC), its relevance is expanding in muscle-invasive bladder cancer (MIBC), as evidenced by recent positive results from the IMvigor011 trial. This study demonstrated that a ctDNA-guided approach to adjuvant atezolizumab therapy enhances disease-free survival. Furthermore, ctDNA is well-established as a tool for monitoring treatment response and assessing minimal residual dis…

A circulating tumor DNA (ctDNA) assay is an emerging non-invasive diagnostic approach providing real-time insights into the heterogeneous tumor molecular landscape of advanced prostate cancer, overcoming the limitations of traditional tissue biopsies and PSA. Detection methods include droplet digital PCR, next-generation sequencing, and new epigenomic and fragmentomic strategies (investigational) designed to improve sensitivity in cases of low ctDNA shedding. While ctDNA's role in localized prostate cancer is limited, it offers significant prognostic value in metastatic cases, where high ctDNA…
