Clinical Process Modeling: An Approach for Enhancing the Assessment of Physicians' Clinical Reasoning.

To ensure the validity of test interpretation and use, educators must be able to connect the dots between the tasks examinees perform on a test and the decisions educators make based on the results of that test. Toward that end, in this article, the authors introduce an approach called clinical process modeling, which combines clinical reasoning and principled assessment design. The goal of this approach is to better align test item development with the assessment of physicians' application of knowledge. Clinical process modeling involves creating complex decision trees that can mirror clinica…

Clinical reasoning in feline spinal disease: which combination of clinical information is useful?

OBJECTIVES: The aim of this study was to evaluate if a combination of discrete clinical characteristics can be used to identify the most likely differential diagnoses in cats with spinal disease. METHODS: Two hundred and twenty-one cats referred for further evaluation of spinal disease were included and categorised as follows: non-lymphoid neoplasia (n = 44); intervertebral disc disease (n = 42); fracture/luxation (n = 34); ischaemic myelopathy (n = 22); feline infectious peritonitis virus myelitis (n = 18); lymphoma (n = 16); thoracic vertebral canal stenosis (n = 11); acute non-compressive

Creating hospital-specific customized clinical pathways by applying semantic reasoning to clinical data.

OBJECTIVE: Clinical pathways (CPs) are widely studied methods to standardize clinical intervention and improve medical quality. However, standard care plans defined in current CPs are too general to execute in a practical healthcare environment. The purpose of this study was to create hospital-specific personalized CPs by explicitly expressing and replenishing the general knowledge of CPs by applying semantic analysis and reasoning to historical clinical data. METHODS: A semantic data model was constructed to semantically store clinical data. After querying semantic clinical data, treatment p

Twelve tips for using image challenges to teach clinical reasoning.

Clinical reasoning is fundamental to safe practice, yet teaching it effectively remains one of the persistent challenges of health-professions education. Image-based challenges-visual material paired with a short clinical vignette-offer an efficient, authentic, and low-cost way to make reasoning visible and to give learners repeated, guided practice. This article offers twelve practical tips for educators who wish to teach clinical reasoning through image challenges. The first tips start from learning objectives, ground the method in reasoning theory and the evidence on observation-based learn…

Structured reasoning failures compromise LLM interpretation of clinical oncology notes.

Large language models (LLMs) show strong performance on clinical benchmarks, yet their reasoning reliability in real-world oncology care remains unclear. We evaluated LLM reasoning on authentic oncology notes using a novel hierarchical error taxonomy across two retrospective cohorts spanning breast, pancreatic, and prostate cancer. GPT-4 produced reasoning errors in 23.1% of note interpretations, the majority reflecting cognitive bias patterns. Errors were more frequent in recommendation tasks and were strongly associated with guideline-discordant recommendations and lower clinician-rated clin

Learning Ophthalmologist Clinical Reasoning for Glaucoma Diagnosis from Fundus Images.

Glaucoma is a leading cause of irreversible blindness worldwide. Ophthalmologists diagnose glaucoma through a structured reasoning process b … framework that explicitly models the ophthalmologist's diagnostic workflow by generating structured clinical reasoning prior to diagnosis. The generated reports are clinically validated, achieving the best performance across all evaluated clinical findings, including a cup-to-disc ratio mean absolute error of 0.070, an ISNT Kendall distance of 1.73, and the highest …

Clinical Reasoning: An Immunocompetent Young Adult With Acute Vision Loss.

A 23-year-old male patient presented to the emergency department with several days of exacerbating headaches, bilateral retrobulbar pain, and blurred vision. Ophthalmologic evaluation was consistent with bilateral optic neuritis (ON). MRI, CSF analysis, and blood tests were normal except for elevated liver enzymes and elevated cytomegalovirus-immunoglobulin G titers. Prompt treatment with intravenous methylprednisolone for acute ON was initiated, despite the concern for an acute systemic viral infection, resulting in rapid improvement in vision. This case report highlights the key differentiat

Clinical Reasoning: Juvenile-Onset Dopa-Responsive Dystonia-Until It Isn't.

Dopa-responsive dystonia (DRD) encompasses a broad spectrum of phenotypically diverse conditions, offering unique diagnostic challenges. We describe the case of a 24-year-old woman with dystonic gait and jerky position-sensitive limb tremor that initially responded exquisitely well to low-dose levodopa, suggesting DRD. Two years later, she developed parkinsonian features, ultimately leading to a diagnostic reclassification. Despite extensive investigations, the underlying etiology remained elusive until 10 years after symptom onset. We discuss this DRD mimic and briefly review the disorders fr

Oropharyngeal Lymphogranuloma Venereum: A Clinical Reasoning Challenge and Literature Review.

Lymphogranuloma venereum (LGV) is a sexually transmitted infection typically caused by serovars L1-L3 of Chlamydia trachomatis. These serovars are tissue-invasive with a preponderance for lymphatic spread and can be acquired via unprotected oral, anal, or vaginal sex. We present the case of a 23-year-old with a prior history of syphilis admitted with four weeks of progressively enlarging painful right cervical lymphadenopathy. Extensive testing, including oropharyngeal swabs and microbiological testing, did not yield a diagnosis, nor did extensive pathological and microbial testing of the lymp

Clinical Reasoning: A Septuagenarian With Painless Ulceration on the Fingertip.

A 78-year-old nondiabetic nonsmoking man presented with a painless nonhealing ulceration on the middle fingertip of the left hand. Similar episodes occurred in the past involving the left middle and index fingers resulting in amputation of the tip of index finger. Diagnosis was deduced by clinical reasoning and confirmed by additional investigations. Readers are presented with a rational step by step diagnostic paradigm in a very rare presentation of a common neurologic disorder.

Emulating Clinical Diagnostic Reasoning for Jaw Cysts with Machine Learning.

The detection and classification of cystic lesions of the jaw is of high clinical relevance and represents a topic of interest in medical artificial intelligence research. The human clinical diagnostic reasoning process uses contextual information, including the spatial relation of the detected lesion to other anatomical structures, to establish a preliminary classification. Here, we aimed to emulate clinical diagnostic reasoning step by step by using a combined object detection and image segmentation approach on panoramic radiographs (OPGs). We used a multicenter training dataset of 855 OPGs

Clinical Reasoning: A Middle-aged Man With Progressive Gait Abnormalities.

Progressive spastic paraplegia is the core symptom of hereditary spastic paraplegias (HSPs), a group of monogenic disorders characterized pathologically by degeneration of the corticospinal tract and dorsal column and leading to irreversible neurologic deficits. However, acquired causes, such as structural, vascular, inflammatory, infectious, metabolic, toxic, neurodegenerative, and iatrogenic causes, can also cause acquired spastic paraplegia. We describe the case of a middle-aged man presenting with progressive spastic paraplegia combined with ataxia and parkinsonism. No mutation of HSP gene
