Lymphangiographic evaluation of patients with clinical lepromatous leprosy on clofazimine.

Pedal edema as a possible adverse effect of clofazimine therapy in leprosy was first reported in 1990. Raasch, et al. reported their lymphangiographic findings on ten patients who had clinical lepromatous leprosy in 1969. None of these patients had been on clofazimine therapy. Our study, therefore, was designed to assess the changes that might be seen in the lymphatic system of patients treated with clofazimine for the management of leprosy. Our findings are compared with those of Raasch and his colleagues.

Orange/black discolouration of the bowel (at laparotomy) due to clofazimine.

Clofazimine, a commonly used anti-lepromatous drug, is now being prescribed for the treatment of pyoderma gangrenosum, a complication of inflammatory bowel disease. This drug can cause an obstructive exacerbation of Crohn's disease. Surgeons should be aware of the orange/black discolouration of the bowel, which may mimic ischaemia macroscopically. A case, the first reported in Australia, is described and the literature discussed.

Clofazimine: a review of its medical uses and mechanisms of action.

Clofazimine has been in clinical use for almost 40 years, but little is known of its mechanism of action. The primary indication for clofazimine is multibacillary leprosy, but it is useful in several infectious and noninfectious diseases, such as typical myocobacterial infections, rhinoscleroma, pyoderma gangrenosum, necrobiosis lipoidica, severe acne, pustular psoriasis, and discoid lupus erythematosus. Postulated mechanisms of action include intercalation of clofazimine with bacterial DNA and increasing levels of cellular phospholipase A2. Clinical experience, possible mechanisms of action, …

Clofazimine pulmonary crystal deposition syndrome: a case of pseudo haemoptysis.

Clofazimine is an antimycobacterial, anti-inflammatory agent used in the management of leprosy and multidrug-resistant (MDR) tuberculosis. It has high oral bioavailability and poor solubility because of which prolonged administration of the drug results in its accumulation as intracellular biocrystals in tissue macrophages. We describe the case of a female patient in her early 30s who was on therapy for MDR tuberculosis. She presented with streaky haemoptysis of 6 months. Radiographic examination showed no abnormality in pulmonary vasculature and parenchyma. Bronchoscopy showed diffuse red-col…

Short-term trial of clofazimine in previously untreated lepromatous leprosy.

Forty-five previously untreated lepromatous leprosy patients were allocated randomly to three groups and treated, respectively, with Regimen A, standard dosage of clofazimine (CLO) in multidrug therapy (MDT) regimen; Regimen B, CLO 600 mg once every 4 weeks; and Regimen C, CLO 1200 mg once every 4 weeks. The duration of the trial was 24 weeks. By the end of the trial, although a few patients in each group did not improve at all clinically, the majority of patients showed clinical amelioration but the responses were slow. While the mean morphological index dropped to the baseline after 24 weeks…

Skin pigmentation from clofazimine therapy in leprosy patients: a reappraisal.

Skin biopsy specimens from two lepromatous leprosy patients with dark brown pigmentation who were receiving long-term clofazimine therapy were studied. Ceroid-lipofuscin pigment was demonstrated inside macrophages that contained numerous phagolysosomes. These contained lipids and clofazimine that appeared as electron-lucent vacuoles and a lipofuscin pigment that was electron dense, granular, and lamellated. Although the presence of the drug in tissues contributed to the skin pigmentation, the main cause was a drug-induced, reversible ceroid lipofuscinosis.

Clofazimine inhibits innate immunity against Mycobacterium tuberculosis by NF-κB.

UNLABELLED: Tuberculosis (TB) remains one of the infectious diseases with high incidence and high mortality. About a quarter of the populati … only used anti-tuberculosis drugs on innate immunity at the cellular level. The results showed that clofazimine (CFZ) has a significant innate immunosuppressive effect. CFZ significantly inhibited cytokines and type I interferons (IFNα and IFNβ) expression under both lipopolysaccharide stimulation and CFZ-resistant strain infection. In further mechanistic stu …

More about clofazimine--3 years experience and review of literature.

Local and systemic side effects of clofazimine in 514 Leprosy and 26 vitiligo patients who had taken the drug in different doses (100 mg to 300 mg daily) for variable periods of time. The commonest side effect noted was reddish brown pigmentation of skin in 77.8% patients. In an equal number of patients, ichthyotic changes on the peripheral parts of the body were noticed. GIT symptoms occurred only in 0.04% patients in the form of abdominal pain, epigastric distress, mild transient nausea and anorexia. Other minor side effects noted were reddish coloration of sweat, urine and tears. Schilling'…

Clofazimine Inhibits the Growth of Babesia and Theileria Parasites In Vitro and In Vivo.

The present study evaluated the growth-inhibitory effects of clofazimine, currently used for treating leprosy, against Babesia bovis, B. bigemina, B. caballi, and Theileria equi in in vitro culture and against Babesia microti in mice. The 50% inhibitory concentrations (IC50s) of clofazimine against the in vitro growth of B. bovis, B. bigemina, B. caballi, and T. equi were 4.5, 3, 4.3, and 0.29 μM, respectively. In mice infected with B. microti, treatment with 20 mg/kg of body weight of clofazimine administered orally resulted in a significantly lower peak parasitemia (5.3%) than that in the co…

Tissue concentration, systemic distribution and toxicity of clofazimine--an autopsy study.

There are very few autopsy studies available on systemic distribution of clofazimine, a drug with anti-mycobacterial activity, used in multidrug therapy (MDT) regimen of leprosy and in erythema nodosum leprosum (ENL). An autopsy study was done on a 45 year old female of lepromatous leprosy (LL) on MDT and long term high dosage of clofazimine. Patient succumbed to intractable abdominal pain, diarrhoea, hypokalemia following clofazimine treatment. Autopsy study revealed yellowish brown discoloration of skin, viscera and body fluids. Chemical extraction of the drug revealed the highest concentrat…

Critical comments on the treatment of leprosy and other mycobacterial infections with clofazimine.

The usefulness of clofazimine (CLO, CAS 2030-63-9) in the treatment of mycobacterial infections with special emphasis on treatment of leprosy is critically discussed. Skin discolouration which decreases compliance, placenta passage, excretion in mother's milk which endanger the embryo or baby respectively, saturation kinetics in absorption and difficulties to determine free drug concentration are severe problems. The observed antagonism in the combination of CLO with other drugs, especially with dapsone, is another argument against its application in the therapy of mycobacterial infections. In…

[Enteropathy due to clofazimine treatment. A case report with an ultrastructural study].

The authors report a case of enteropathy due to clofazimine treatment for prurigo nodularis. The drug usually restricted to the treatment of leprosy has sometime been given to patients suffering from skin diseases because of its assumed anti-inflammatory effects. When high dosage of clofazimine treatment is carried on during a long period, and drug accumulates in the tissues and precipitates as a solid. This crystal storage in the lamina propria of the jejunal mucosa and in the mesenteric lymph nodes accounts for an enteropathy, sometime severe but slowly regressing after drug withdrawal. The …

Drug interactions -- the influence of rifampicin and clofazimine on the urinary excretion of DDS.

Indication of an enhancing effect of Rifampicin on the urinary excretion of DDs has already been reported from this institution. Further obs … ater phases of the drug administration. As part of the study of drug interactions, the influence of Clofazimine administration on DDS metabolism was also studied. The findings indicate that Clofazimine does not exert any influence on DDs excretion by leprosy patients. The findings and their implications are discussed.

Exposure-QTc modeling of bedaquiline, pretomanid, and clofazimine in adults with tuberculosis.

Drug resistance (DR) poses a critical challenge to global efforts to manage tuberculosis (TB). Limited information is available on the combined cardiotoxicity of bedaquiline (BDQ), pretomanid (Pa), and clofazimine (CFZ), key drugs in current DR-TB treatment regimens. All of those prolong the QT interval. We aimed to describe this interaction to predict possible toxicities with established and novel dosing regimens to identify patients at higher risk of QT prolongation. The data for this analysis were obtained from an early-bactericidal activity study in drug-susceptible-TB of several TB drugs.…
