PURPOSE: To evaluate the effectiveness of high-dose clonazepam (1 mg) versus low-dose clonazepam (0.5 mg) with cognitive behavioral therapy for insomnia (CBT-i) in older adults with moderately severe insomnia.
METHODS: A prospective cohort study was conducted in patients who did not respond to low-dose clonazepam for insomnia secondary to chronic medical conditions. INTRODUCTION: For patients with schizophrenia, clozapine (CLZ) in combination with clonazepam (CLNAZ) is one of the viable therapeutic options. We successfully reduced the doses of CLZ and CLNAZ to the safe range of a polydrug abuse patient. As far as we know, this is the first case of this problem. Clonazepam has been reported to be of some value in the treatment of benign essential tremor in open trials. The efficacy of clonazepam was evaluated in a double-blind placebo-controlled study using up to 4 mg/day. Results are reported of a preliminary trial with clonazepam in 19 patients with trigeminal neuralgia refractory to carbamazepine treatment. In this series of patients, 13 (68.4%) showed excellent or good improvement. No serious side-effects were seen. The pharmacology and mechanism of action of clonazepam are discussed. It is suggested that patients with trigeminal neuralgia, who do not respond to carbamazepine, should be given clonazepam before surgery is considered.
