Clostridium botulinum is a gram-positive anaerobic rod that forms endospores. This bacterium produces large molecular toxin complexes, namely botulinum toxin complexes (progenitor toxins). It (L toxin complex) is composed of a single neurotoxin molecule (BoNT with a molecular weight of 150 kDa), a single nontoxic nonhemagglutinin molecule (NTNHA), and a hemagglutinin complex (HA). On food-borne botulism, nontoxic components have the roles of protecting toxin protein from the degeneration and degradation action of acids and proteases existing in the gastrointestinal tract. The HA facilitates tr…

Clostridium difficile is a Gram-positive, spore-forming, anaerobe and is the leading cause of antibiotic-associated diarrhea, pseudomembranous colitis, and toxic megacolon. Essential to the lifestyle of C. difficile is the ability to form a metabolically dormant spore, germinate, and grow out upon appropriate signals and elicit disease with the secretion of two toxins. To aid in the study of this organism, this unit describes the growth and maintenance of C. difficile. Included are methods to isolate C. difficile from environmental samples, grow in laboratory medium, and produce and purify spo…

Clostridium perfringens is responsible for a number of clinical conditions ranging from relatively mild food poisoning to the potentially life-threatening gas gangrene. Fortunately, C. perfringens has remained relatively susceptible to first line antibiotics in the treatment of soft tissue infection, however, the prevalence of antibiotic resistance is increasing amongst other anaerobic organisms. A case of anaerobic cellulitis caused by a clindamycin-resistant C. perfringens is described here, emphasising the emerging problem of antimicrobial resistance.

Extraintestinal infections of Clostridium difficile are rare and often associated with underlying disorders. A case of empyema caused by aspiration of C difficile in a patient with carcinoid syndrome and C difficile colitis is described.

The gene encoding the alpha-(cpa) is present in all strains of Clostridium perfringens, and the purified alpha-toxin has been shown to be a zinc-containing phospholipase C enzyme, which is preferentially active towards phosphatidylcholine and sphingomyelin. The alpha-toxin is haemolytic as a result if its ability to hydrolyse cell membrane phospholipids and this activity distinguishes it from many other related zinc-metallophospholipases C. Recent studies have shown that the alpha-toxin is the major virulence determinant in cases of gas gangrene, and the toxin might play a role in several othe…

Clostridium difficile-associated disease (CDAD) is primarily a nosocomial condition. Community-acquired disease has been reported but the incidence is felt to be low and the rate of disease resulting in hospitalization is reported as negligible. We recently experienced a 6-month outbreak of CDAD (January to June 1995): 139 patients were involved and four deaths were attributable to pseudomembranous colitis. Early in the outbreak period we were aware that many new admissions presented with C. difficile cytotoxin B positive diarrhoea: in some cases this was the sole reason for hospitalization. T…

A Clostridium perfringens-like strain was isolated from a case of gas gangrene. The morphological properties and the lecithinase reaction of the isolate were very similar to those of C. perfringens; however, the lecithinase reaction was only slightly suppressed by C. perfringens alpha-antitoxin serum and the organism was identified as Clostridium absonum from its biochemical properties.

Nontraumatic clostridial infections are rare, but need to be diagnosed and treated early or high morbidity and mortality rates result. We believe that this is the first reported case of metastatic nontraumatic Clostridium septicum osteomyelitis. Early treatment with surgical debridement and parenteral antibiotics without hyperbaric oxygen was used. An associated occult rectal malignancy was discovered and treated.

Clostridium difficile has been detected in the stools of some patients with relapse of Crohn's disease. The authors looked prospectively for present or previous exposure to C. difficile cytotoxin in 10 patients with mild to severe Crohn's disease. None of 25 stool samples from these 10 patients was positive for C. difficile cytotoxin. These negative stool ultrafiltrates had mild cytotoxin neutralizing activity, but this finding did not differ from that in 30 cytotoxin-negative stools from patients with other diarrheal diseases. Serum from these patients also showed no cytotoxin neutralizing ac…

Thirteen of 280 (4.6%) blood cultures collected over a 12-day period were positive for Clostridium sordellii, a spore-forming anaerobe, rarely considered a human pathogen. Nosocomial bacteremia and intrinsic contamination of material used to culture blood were excluded as the source of the organism. Contaminated tincture of thimerosal used to swab the rubber stoppers of blood culture bottles prior to venting (aerobic) or during blind subculturing after 24 hours of incubation (anaerobic) in the clinical microbiology laboratory was determined to be the cause of the pseudobacteremia. After approp…

Clostridium botulinum is not a well-defined species of bacterium. Instead, it is a conglomerate of four culturally distinct groups of organisms that, among them, produce seven serologically distinct toxins, all with similar pharmacological action. The principal habitat of C. botulinum is the soil, although its distribution in the soil is sometimes highly regional. Infant botulism is caused by two types of C. botulinum: type A and the proteolytic strains of type B. Type A strains, to whose toxin humans seem most susceptible, are found most frequently in the soil of the western United States; ty…

Clostridium difficile infection (CDI) is classified as non-severe, severe, or fulminant based on clinical severity and presence of complications. Clinical manifestations span a broad spectrum, ranging from mild self-limited diarrhea to severe colonic inflammation with potentially fatal outcomes. Management strategies for CDI are guided by this severity-based classification. In this article, the authors focus on non-severe CDI, reviewing its definition, diagnostic approach, prevention strategies, treatment options, and predictors of treatment failure. They conclude by highlighting emerging area…

BACKGROUND: For surgeons the early identification of patients with clostridium difficile infections (CDI) is important, because the incidence and virulence of this potentially life-threatening disease are increasing.
OBJECTIVES: The aim of this study was to describe the frequency of CDI among surgical patients, to analyze which treatment was successful and to define which factors were associated with mortality.
METHODS: A retrospective analysis of patients with CDI was performed.
RESULTS: From January 2004 to June 2012 the overall incidence of CDI among all departments at the St. Josef Hosp…
