A 77-year-old woman presented to the emergency department complaining of a headache following a syncopal episode (while standing) earlier that day. She said that she'd lost consciousness for several minutes, and then experienced several minutes of mild confusion that resolved spontaneously. On physical exam, she was oriented to person and place, but not time. She had a contusion in her left occipitoparietal region without extensive bruising or deformity. The patient had normal cardiopulmonary, abdominal, and neurologic exams. Her past medical history included hypertension and normal pressure h…

The literature is rich in case reports of intraosseous haemangioma, although most of these are actually cases of venous or capillary malformations. To illustrate this confusion in terminology, we present three cases of slow-flow vascular malformations misnamed as intraosseous haemangioma. A retrospective study of children diagnosed with intraosseous haemangioma was conducted. Clinical and radiological data were evaluated. Histopathological examinations and immunohistochemical studies were redone by three independent pathologists to classify the lesions according to the International Society fo…

A 13-year-old girl presented with an afebrile seizure followed by prolonged confusion and visual hallucinations. Initial investigations in the form of blood tests, cerebrospinal fluid analysis and head imaging by CT, were normal. She represented with two further episodes within a period of 3 weeks. Further investigations considering infective, metabolic and some autoimmune causes of encephalopathy were negative. An MRI head scan was normal. Thyroid function testing disclosed primary hypothyroidism and elevated antithyroid antibodies. She responded well to glucocorticoid therapy for presumed Ha…

Pentamidine has a long history in the treatment of human African trypanosomiasis (HAT) and leishmaniasis. Early guidelines on the dosage of pentamidine were based on the base-moiety of the two different formulations available. Confusion on the dosage of pentamidine arose from a different labelling of the two available products, either based on the salt or base moiety available in the preparation. We provide an overview of the various guidelines concerning HAT and leishmaniasis over the past decades and show the confusion in the calculation of the dosage of pentamidine in these guidelines and t…

Tremendous confusion exists concerning the issues of physician certification and practice accreditation in obstetric and gynecologic ultrasonography. Certification speaks to physician competence. All obstetrician-gynecologists who have finished residency training since 1982 are deemed "competent" to perform obstetric and gynecologic ultrasonography by virtue of their training. Those trained before 1982 need to be able to describe their level of experience in performing obstetric and gynecologic ultrasonography. Accreditation deals with issues of patient safety and quality control, including eq…

As in many specialties, otolaryngology is not without diagnosis surrounded by controversy. This report discusses the entity known as the primary branchial cleft carcinoma, which has been argued by some to be no more than metastatic disease of the unknown primary. Although the disease is rare, the consideration of this diagnosis requires careful thought and analysis by the otolaryngologist, pathologist, and radiation oncologist so that a rational treatment plan can be offered to the patient.

Marked hyperkalaemia occurred on more than one occasion in 3 patients with thrombocytosis caused by the myeloproliferative syndrome. Initial failure to recognize that the elevation in the serum potassium level was an artefact resulted in unnecessary admissions to hospital and inappropriate investigations and treatment. Awareness of this entity and the use of suitable confirmatory tests should avoid such problems in management.

BACKGROUND: Infectious spondylitis is a rare but increasingly recognized condition involving infectious lesions of the spine. It can affect patients of all ages and is typically caused by bacterial, fungal, or other pathogens. The infection commonly targets the vertebral bodies and intervertebral discs. Early symptoms of infectious spondylitis are often nonspecific, making it challenging to differentiate from other spinal disorders such as tumors, degenerative diseases, or other types of infections. As the condition can progress rapidly, early diagnosis and intervention are crucial to improvin
