Post-mortem ultrasound study of coronary circulation on ex-situ hearts.

Sudden cardiac death is a leading cause of death in the 40-65 age group in industrialized countries. In approximately 70-80% of cases, it is attributable to atherosclerotic degeneration of the coronary circulation. This experimental study aims to evaluate the suitability of ultrasound for the morpho-functional characterization of the coronary circulation in ex situ hearts. The coronaries of six human hearts were cannulated with a vascular catheter, and an aqueous contrast medium was injected using an electric pump. The cannulated vessel underwent ultrasound scanning (US) for both morphological…

Coronary Collateral Circulation and Cardiovascular Risk Factors: Is There a Paradox?

We sought to determine the association of major cardiovascular risk factors and other comorbidities with the presence or absence of coronary collateral (CC) circulation. All electronic medical records from 2010 to 2011 were retrospectively reviewed. A total of 563 patients were divided into 2 groups: CC present (180) and CC absent (383). Smoking (P = .012, odds ratio [OR] 1.58), hypercholesterolemia (P = .001, OR 2.21), and hypertension (P = .034, OR 1.75) were associated with the presence of CC. Increasing body mass index (BMI, P = .001) and decreasing estimated glomerular filtration rate (eG

Hypertension and the coronary circulation. With special attention to endothelial regulation.

Calcium channel antagonists are commonly used to treat chronic hypertension. Several studies of intact vascular tissues suggest that these a … channels in vascular endothelial cells. Chronic hypertension produces myriad adverse effects in the coronary circulation. After coronary occlusion, infarct size, expressed as a function of myocardial mass perfused, is increased by 33%, and the wavefront of infarction from subendocardium to subepicardium is hastened. Both chronic and acute hypertension produce numerous …

The Effects of Preoperative Coronary Collateral Circulation on Cardiac-Related Events after Coronary Artery Bypass Graft Surgery.

INTRODUCTION: This study aimed to evaluate the effects of coronary collateral circulation (CCC) in patients who had undergone coronary artery bypass grafting (CABG). METHODS: A total of 127 patients who had undergone CABG (2011-2013) were enrolled into this study and follow-up was obtained by phone contact. Patients were categorized into two groups according to preoperative CCC using the Rentrop method. Percutaneous coronary intervention (PCI), recurrent myocardial infarction (MI), stroke, heart failure (HF), and mortality rates were compared between groups. Clinical outcome was defined as co

Heart failure due to coronary fistulas from the right and left coronary circulation into the right atrium.

A patient with a history of hypertension presented with worsening shortness of breath, fatigue, and productive cough with blood-tinged sputum. A continuous murmur was heard at the lower sternum and chest X-ray showed mild cardiomegaly. A dilated right atrium with moderately elevated pulmonary artery pressures was detected by transthoracic echocardiogram. Stress ECG did not reveal any inducible ischemia. Cardiac catheterization demonstrated an RCA aneurysm with fistula to the right atrium and a circumflex artery fistula to the right atrium, which were corrected by surgery.

Acute effects of percutaneous coronary intervention on neutrophil activation and maturity markers in coronary and peripheral circulation.

BACKGROUND: Acute inflammatory responses during and after percutaneous coronary intervention (PCI) are associated with subsequent cardiovasc … post-PCI from the peripheral arterial sheath (peripheral circulation) and coronary guide catheter (coronary circulation). RESULTS: The primary endpoints were pre-PCI to post-PCI changes in immature neutrophils (CD10-CD16- subset as a total percentage of neutrophils) and neutrophil-specific expression of markers relevant for activation and degranulation (CD62L and CD …

Association between the pan-immune-inflammation value and coronary collateral circulation in chronic total coronary occlusive patients.

BACKGROUND: Inflammation and immunity play important roles in the formation of coronary collateral circulation (CCC). The pan-immune-inflammation value (PIV) is a novel marker for evaluating systemic inflammation and immunity. The study aimed to investigate the association between the PIV and CCC formation in patients with chronic total occlusion (CTO). METHODS: This retrospective study enrolled 1150 patients who were diagnosed with CTO through coronary angiographic (CAG) examinations from January 2013 to December 2021 in China. The Cohen-Rentrop criteria were used to catagorize CCC formation

Platelet microvesicles are associated with the severity of coronary artery disease: comparison between peripheral and coronary circulation.

Microvesicles (MVs) have recently emerged as markers of thrombosis. Furthermore, there is an unexplained residual thrombotic risk is observed in patients with acute coronary syndrome (ACS) and/or stable coronary artery disease (CAD), despite treatment. We measured platelet (PMVs) and erythrocyte (ErMVs) in patients with ACS and stable CAD, both in the peripheral and coronary circulation. We studied consecutive eligible patients during a coronary angiography. Blood samples were collected from the stem of the left coronary artery and femoral artery. PMVs were significantly increased in CAD patie…

Association between adiponectin production in coronary circulation and future cardiovascular events in patients with coronary artery disease.

Adiponectin has antiatherosclerotic properties and is also produced in the local coronary circulation. We previously reported that significantly less adiponectin was produced in the coronary circulation of patients with than without coronary artery disease (CAD). The goal of this study was to determine whether adiponectin production in the coronary circulation could predict future cardiovascular events in patients with CAD.Forty-eight CAD patients whose left anterior descending coronary arteries required percutaneous coronary intervention (PCI) were enrolled. The amount of adiponectin producti…

17beta-estradiol decreases endothelin-1 levels in the coronary circulation of postmenopausal women with coronary artery disease.

BACKGROUND: Estrogen reverses acetylcholine-induced coronary vasoconstriction via the possible facilitation of endothelium-derived NO. Estrogen also affects endothelium-derived constrictor factors. We therefore investigated the effects of 17beta-estradiol on coronary vasomotor responses to substance P (SP), and coronary sinus endothelin-1 and NO metabolite levels in postmenopausal women with coronary heart disease. METHODS AND RESULTS: We studied 20 women; 14 received estrogen (mean age 65+/-2 years) and 6 served as ethanol control subjects (age 63+/-3 years). Intracoronary infusions of papav

Total anomalous origin of the coronary circulation from the right pulmonary artery.

Origin of both coronary arteries from a branch of the pulmonary artery is rare and has not been reported as a cause of sudden unexpected death. We report autopsy findings of a 14-day male infant in previously good health who died suddenly. From the proximal right pulmonary artery arose a single coronary artery, which branched into the right and left main coronary arteries. The right proximal coronary artery coursed between the aorta and pulmonary trunk, and the left main traveled along the anterior proximal aorta. The distal course of the coronary circulation was normal. There was concentric s…

Surgical anatomy of the coronary circulation in hearts with discordant atrioventricular connections.

We examined the arrangement of the coronary arterial and cardiac venous systems in 46 specimens with discordant atrioventricular connections so as to identify any structural abnormalities and to consider their surgical implications in terms of anatomical biventricular repair. Grossly abnormal arterial courses were seen in 11 hearts (24%). A substantial branch supplying the morphologically right ventricular outflow tract, which could restrict a ventriculotomy, was found in 61% of cases. The coronary sinus received all the morphologically right ventricular veins, as well as the posterior interve

Abnormalities of the coronary circulation in pulmonary atresia and intact ventricular septum.

To identify developmental aspects and establish morphologic criteria useful for clinical recognition of sinusoids and coronary artery abnormalities in pulmonary atresia and intact ventricular septum, we studied 17 autopsy hearts with this lesion. Hearts from age- and weight-matched infants served as controls. There were two stillborn fetuses, 12 full-term neonates, and 3 infants. None of the eight hearts with normal or large-sized right ventricular chambers had sinusoids or coronary artery abnormalities. Five of nine hearts with small right ventricular chambers had sinusoids; all five lacked t

What is the role of the coronary circulation in congestive heart failure?

Manifestations of congestive heart failure are frequently observed in patients with coronary disease. However, they may be predominant in pa … rom other causes (e.g. valvular disease). However, congestive heart failure per se can also disturb coronary circulation as a result of changes in subendocardial myocardial perfusion or metabolism.
