Using neural measures of economic value to solve the public goods free-rider problem.

Every social group needs to decide when to provide public goods and how to allocate the costs among its members. Ideally, this decision would maximize the group's net benefits while also ensuring that every individual's benefit is greater than the cost he or she has to pay. Unfortunately, the economic theory of mechanism design has shown that this ideal solution is not feasible when the group leadership does not know the values of the individual group members for the public good. We show that this impossibility result can be overcome in laboratory settings by combining technologies for obtaini

Prognosis after "high urgent" kidney transplantation might be determined by control of preexisting septic condition.

INTRODUCTION: Dialysis is the standard bridging method for patients with end-stage renal disease. In rare cases, dialysis is impossible and immediate kidney transplantation (KT) is the only option for survival. Most allocation organizations offer an immediate allocation procedure (high urgency [HU]), which focuses on immediate allocation at the cost of immunologic matching. The impossibility of dialysis is mainly caused by multiple systemic thromboses and blood stream infections. This situation creates an ethical dilemma: Accepting the HU-KT allocation potentially saves the patient's life albe

Environmental performance of sustainable supersulfated cements based on blast furnace slag: A life cycle study.

This study evaluates the environmental, economic, and mechanical performance of supersulfated cements (SSC) made with blast furnace slag (BFS) as a low-carbon alternative to Portland cement (PC). It combines Taguchi optimization and life cycle assessment (LCA) to examine the effects of slag fineness, curing conditions, and BFS allocation methods (mass, economic, and no allocation) on three previously optimized SSC compositions. Two scenarios representing different plant locations, were considered. This study presents three major contribution: (1) a quantified framework for balancing mechanical

Unintended consequences of changes to lung allocation policy.

Organ allocation for transplantation aims to balance the principles of justice and medical utility to optimally utilize a scarce resource. To address practical considerations, the United States is divided into 58 donor service areas (DSA), each constituting the first unit of allocation. In November 2017, in response to a lawsuit in New York, an emergency action change to lung allocation policy replaced the DSA level of allocation for donor lungs with a 250 nautical mile circle around the donor hospital. Similar policy changes are being implemented for other organs including heart and liver. Fi

Beyond Triage: Cognitive Profiles and ED-To-Inpatient Costs and Resource Pathways in Older Adults.

BACKGROUND: Older adults are frequent users of the Emergency Department (ED), with a significant proportion presenting with pre-existing or acute cognitive impairment. While negative post-ED outcomes associated with cognitive status are well documented, their direct impact on care processes and resource allocation within the hospital remains poorly understood. This study aims to quantify how different cognitive profiles affect costs and care needs for acutely ill older adults. METHODS: We conducted a secondary analysis of a prospective cohort study at a single, tertiary care hospital. We incl

Decision-theoretic recruitment and validation planning for depression surveys: An illustrative Chinese application.

BACKGROUND: Survey planning must balance recruitment losses against uncertainty in screening yield, diagnostic accuracy and survey design. This study examined how these considerations determine invitation requirements and diagnostic-validation allocation. METHODS: A Beta planning distribution used 170 PHQ-9-positive results among 1,045 Chinese community participants and an effective sample size of 100. Asymmetric recruitment loss selected conditional and joint-distribution invitation targets. A separate cost-plus-variance Bayes criterion jointly allocated survey clusters and independent refer

The Process Is Not the Point: Moral Uncertainty and Substantive Justice in Health Care Priority Setting.

Health actors must ration care due to resource constraints. But even when decision makers know the relevant empirical facts about an allocation decision-including how many patients will benefit from a service, which patients will benefit, how much each patient will benefit, and so on-these facts may not provide a clear answer about how to make allocation decisions. This is because competing moral principles recommend different courses of action: for instance, maximizing health benefits may involve deprioritizing more disadvantaged groups. Facing this moral uncertainty, many ethicists and polic

Needs assessment in long-term care: expression of national principles for priority setting in service allocation.

BACKGROUND: Long-term care services for older adults are characterised by increasing needs and scarce resources. Political strategies have led to the reorganisation of long-term care services, with an increased focus on "ageing in place" and efficient use of resources. There is currently limited research on the processes by which resource allocation decisions are made by service allocators of long-term care services for older adults. The aim of this study is to explore how three political principles for priority setting in long-term care, resource, severity and benefit, are expressed in servic

An Examination of Medically Necessary Diets within the Framework of the Thrifty Food Plan.

The Thrifty Food Plan (TFP) determines the maximum allotment of nutrition assistance for over forty million Americans participating in the Supplemental Nutrition Assistance Program (SNAP). This research recalculates the official TFP allocations using a more straightforward model that minimizes cost subject to nutrient constraints to establish a baseline for adult females and males. We then examine three additional diet plans to evaluate their feasibility under the current SNAP benefit allotment: a diet for lactose intolerance, for persons with type 2 diabetes, and a diet for pregnant women. Operating-Room scheduling under uncertainty: balancing elective and emergency surgeries.

Balancing elective and emergency surgeries in operating room (OR) scheduling is challenging due to uncertainty in surgery durations and emergency arrivals. Traditional approaches, such as dedicated OR allocation and the Break-in-Moment (BIM) policy, can result in underutilised capacity and high emergency cancellation rates. This paper proposes a two-stage stochastic optimisation model that incorporates endogenous buffer allocation within the OR schedule. Rather than relying on policy-based or heuristically defined buffer times, the proposed framework determines the timing and placement of buff

Oncological resource allocation in Germany.

BACKGROUND: Oncology is a resource-intensive medical discipline where, so far, effectiveness rather than efficiency of a treatment has stood in the foreground. The aim of our study was, therefore, to determine the resource allocation and to assess the efficiency of oncology in Germany for the period of 2002-2004. MATERIALS AND METHODS: With the aid of the official German Health Report, the expenditures for health in 2004 and the gain in years of life according to ICD 10 disease categories were analyzed. Based on the incremental costs and years of life gained, the cost calculation per year of

Using Constrained Optimization for the Allocation of COVID-19 Vaccines in the Philippines.

BACKGROUND: Vaccine allocation is a national concern especially for countries such as the Philippines that have limited resources in acquiring COVID-19 vaccines. As such, certain groups are suggested to be prioritized for vaccination to protect the most vulnerable before vaccinating others. OBJECTIVE: The study aims to determine an optimal and equitable allocation of COVID-19 vaccines in the Philippines that will minimize the projected number of additional COVID-19 deaths while satisfying the priority groups for immediate vaccination. METHODS: In this study, a linear programming model is for

Provider costs of Antiretroviral therapy (ART) in Zimbabwe: The value of using time-driven activity based costing methods in a low resource setting.

Although ART has transformed HIV into a manageable chronic condition, significant cost and logistical challenges persist, threatening progress toward the UNAIDS 95-95-95 targets. Budget allocation to the health sector declined by over 30% in the last decade in Zimbabwe, attributed to donor fatigue and emergence of pandemics. The time-driven activity-based costing (TDABC) method was used to estimate the provider costs of ART and inform resource allocation for sustained ART programming. A descriptive cross-sectional study in 11 facilities across Zimbabwe's four levels of care collected data usin

Opportunity cost calculations only determine justified effort--or, what happened to the resource conservation principle?

We welcome the development of a new model on effort and performance and the critique on existing resource-based models. However, considering the vast evidence for the significant impact of experienced task demand on resource allocation, we conclude that Kurzban et al.'s opportunity cost model is only valid for one performance condition: if task demand is unknown or unspecified.

Comment on: effects of emergency department length of stay on inpatient utilization and mortality.

Emergency department (ED) crowding is a persistent global challenge with well-documented consequences for patient outcomes and healthcare resource utilization. A recent study by Ma et al. reported that prolonged ED length of stay was associated with lower inpatient hospitalization costs, a finding that merits careful methodological scrutiny. We outline three considerations that may inform the interpretation of these findings. First, the aggregate cost of an admitted population is sensitive to its case-mix; hospitalization costs vary substantially across and within clinical services, and the rel

Cost-Minimization and Procedural Outcomes of Cold Snare Versus Cold Forceps Polypectomy for Small Colorectal Polyps: A Prospective Cohort Study Using Real Institutional Pricing.

Background/Objectives: Although current guidelines (European Society of Gastrointestinal Endoscopy [ESGE] 2024, U.S. Multi-Society Task Force [USMSTF] 2020) recommend cold snare polypectomy (CSP) over cold forceps polypectomy (CFP) for small colorectal polyps, CFP remains widely used. Comparative economic data using real institutional pricing are scarce, limiting evidence for guideline-concordant resource allocation. We evaluated the healthcare economics and procedural outcomes of CSP versus CFP to assess their institutional resource implications. Methods: This prospective cohort study analyze

Lung ultrasound is cost-effective for early diagnosis of COVID-19 pneumonia in primary care.

Chest X-rays and lung ultrasounds are extensively utilized in primary care to diagnose pneumonia, a primary clinical manifestation of COVID-19. Employing chest X-rays for the diagnostic management of suspected COVID-19 pneumonia is significantly more costly than utilizing lung ultrasound. Since the pandemic swiftly overwhelmed healthcare systems globally, identifying the most cost-effective diagnostic tools is essential to optimize resource allocation.

Cost shifting or cost cutting by hospitals as a response to reimbursement reform? The case of diagnosis-related groups (DRG) scheme in China.

BACKGROUND: Diagnosis-related group (DRG) systems for healthcare reimbursement were recently introduced among hospitals in China, raising concerns about cost-shifting, where hospitals may increase charges for self-financing patients to offset reimbursement cuts by DRG. In 2018, both Nanchang and Ganzhou Cities in Jiangxi Province installed DRG information systems, but only Nanchang fully implemented the DRG system during the 2019-2020 pilot period. MATERIALS AND METHODS: Drawing from a healthcare administrative dataset of 14,310 patients' records, this study investigates the hospitalization c
