The CT imaging and clinical presentation in 14 children with coexistent intracranial sepsis and sinusitis were reviewed. A routine CT head scan (10-mm thick semi-axial slices through the cranium done before and after intravenous contrast medium administration) was found to be an inadequate initial investigation as the intracranial collection was missed in four patients and the abnormal sinuses not shown in six. In half the children the diagnosis of sinusitis was unsuspected at the time of admission. The dominant clinical features were fever, intense headache and facial swelling in early adoles

Pulsatile tinnitus (PT) is characterized by the perception of rhythmic sounds synchronized with the heartbeat, with sigmoid sinus wall abnormalities (SSWA) being the most common etiology. The generation and transmission of sound in SSWA-PT are closely related to abnormal hemodynamics, elevated intracranial pressure, and anatomical abnormalities of the sigmoid plate and temporal bone pneumatization. Despite extensive research, the exact pathophysiological mechanisms of SSWA-PT remain unclear, often leading to misdiagnosis and suboptimal treatment outcomes. This narrative review synthesizes stud

We have evaluated the anatomical structure of cavernous sinuses in parasellar blocks obtained from adult cadavers to gain clinically useful information. Ten microdissectional studies of parasellar blocks (20 sides of cavernous sinus) were carried out with special emphasis on the course of paracavernous cranial nerves (3rd to 6th). These were also examined on plain radiographs. Secondly, CT investigations of the cavernous sinuses (notably as to paracavernous cranial nerves) were undertaken in twenty clinical cases having presumed parasellar lesions and in three parasellar blocks from cadavers.

Paranasal sinuses are some of the most poorly understood features of mammalian cranial anatomy. They are highly variable in presence and form among species, but their function is not well understood. The best-supported explanations for the function of sinuses is that they opportunistically fill mechanically unnecessary space, but that in some cases, sinuses in combination with the configuration of the frontal bone may improve skull performance by increasing skull strength and dissipating stresses more evenly. We used CT technology to investigate patterns in frontal sinus size and shape dispari

The paranasal sinuses, which formed in mammals as turbinates and air spaces to perform or aid olfaction, eventually came to participate in the formation and maintenance of the entire cranium and to have concomitant functions as well. Evolving through primates to humans, they were influenced by the retraction of the maxillo-facial cranium and the enlargement of the cerebral cranium and thus reached the state unique to humans. The evolutionary phenomena involved in the formation and functions of the paranasal sinuses include adaptation to olfaction, respiration, and cranial formation. These were

A neonate presented with Dandy-Walker syndrome manifesting as a large posterior cranial fossa cyst, aplasia of the lower cerebellar vermis, and elevation of the confluence of the sinuses but without hydrocephalus. A cystoperitoneal shunt was placed at one month after birth. The cyst diminished in size, and marked development of the cerebellar hemispheres and descent of the confluence of sinuses were observed, but not vermis development. The primary pathology of Dandy-Walker syndrome is posterior cranial fossa cyst formation due to passage obstruction in the fourth ventricle exit area and aplas

A 13-year-old boy was presented to the Department of Neurosurgery, Saiseikai Fukuoka General Hospital for further examinations concerning abnormal findings in the skull radiogram taken when he struck his head. His physical features showed some characteristics the same as those of pycnodysostosis as follows--proportionate dwarfism, prominent forehead, short spoon-shaped fingers, bilateral exophthalmos. A skull radiogram revealed widely open cranial sutures with no healing of the fracture and craniotomy which was performed for an acute epidural hematoma 6 years ago. Furthermore, the mandible was

Streptococcus pneumonia and Haemophilus influenzae account for more than 50% of bacterial acute sinusitis. Isolated sphenoid sinusitis is a rare disease with potentially devastating complications such as cranial nerve involvement, brain abscess, and meningitis. It occurs at an incidence of about 2.7% of all sinus infections. There have been no previous reported cases of unilateral sphenoid sinusitis presenting as septicaemia in an otherwise healthy young immunocompetent adult.

Disorders of the paranasal sinuses, particularly the sphenoid sinus, can be associated with significant disorders of the optic and other cranial nerves. We examined 100 consecutive routine CT scans, 100 posterior fossa CT scans, and 100 MR scans to look for evidence of sinus disease, especially of the sphenoid sinus. The sphenoid sinus was abnormal in 7% of scans by all methods. Other sinuses were more frequently abnormal, including maxillary (23%), ethmoid (34%), and frontal (16%). Although MR was more sensitive in detecting sinus inflammation in the ethmoid and maxillary sinuses, the frequen

Bilateral, idiopathic, essentially asymptomatic but hemodynamically significant stenosis or occlusion of the sigmoid sinuses is extremely rare. We report a child with the angiographic features of occlusion of the right sigmoid sinus and severe stenosis on the contralateral side, presenting with megalencephaly and a cranial bruit. Cranial computed tomographic scans demonstrated increased subarachnoid spaces and borderline ventricular enlargement.

A new technique in craniofacial surgery to separate the cranial and nasal cavities from each other is described. This can be achieved by preserving the complete anterior cranial fossa while simultaneously correcting the forehead, orbit, and face in craniofacial dysostosis. With this procedure, the risk of infection deriving from nasal sinuses and cavities should be minimized.

The examination of 2251 patients with paranasal sinusitis has detected various rhinosinusogenic orbital and intracranial complications in 75 of them (3.3% of overall number of sinusitis patients). The complications were caused by acute and chronic paranasal sinusitis in 49 and 26 patients, respectively. 55 patients had orbital, 20--different intracranial complications. The treatment in most cases consists of emergency operation on the paranasal sinuses, in intracranial complications the surgeon opens dura mater of the anterior cranial fossa and prescribes massive antibacterial, dehydration, de

The present work aimed to investigate the anatomical structures of the Saanen goat head and its volumetric properties using computed tomography (CT) and stereological methods. Eight adult Saanen goat heads were included in this study. The different cavities and structures of the head, including nasal cavity, paranasal sinuses, oral cavity, orbital cavity, and cranial cavity were evaluated using CT scans, cross, sagittal, and coronal sections. The volume of head cavities and structures were estimated using the Cavalieri method. The results showed that the dorsal, middle and nasal ventral concha

BACKGROUND: Transverse sinus stenosis (TSS) is commonly found in Pulsatile Tinnitus (PT) patients. Vortex flow is prominent in venous sinus with stenosis, and so it is important to determine the distribution and strength of the vortical flow to understand its influence on the occurrence of PT.
METHODS: In this study, by using computational fluid dynamics for hemodynamic analysis in patient-specific geometries based on Magnetic Resonance Imaging (MRI), we have investigated the blood flow within the venous sinus of 16 subjects with PT. We have employed both laminar and turbulent flow models for

A 77-year-old-man with arterial hypertension, diabetes mellitus type II presented at our clinic for a routine ophthalmological exam. He complained of intermittent double vision. The ophthalmic examination revealed paralysis of III (n. oculomotorius) and VI (n. abducens) cranial nerves with ptosis, deficit in elevation and abduction of the left eye. The patient underwent urgent MRI imaging of the brain/orbits and paranasal sinuses, and urgent neurological assessment. MRI revealed a volume-occupying process, starting from the posterior wall of the left maxillary sinus with perineural diffusion a

INTRODUCTION: Dural and anterior cranial base reconstruction is essential in the surgical resection of a craniofacial tumor that extends from the paranasal sinuses to the subdural space. Watertight reconstruction of vascularized tissue is essential to prevent postoperative liquorrhea, especially under conditions that prevent wound healing (e.g., postoperative irradiation therapy).
METHOD: We successfully treated two cases of olfactory neuroblastoma by endoscopic-assisted craniotomy with primary dural closure and anterior cranial base reconstruction using a multi-layered flap technique. Dural

INTRODUCTION: Dermal sinus (DS) is a congenital condition of embryonic origin that occurs when the surface ectoderm fails to fully separate from the neuroectoderm, despite the neural tube remaining intact. Its most common location is the spinal region, followed by the occipital region. Off-midline presentation is extremely rare. The aim of this study is to present an atypical case of a dermal sinus located in the pterional region, outside the midline.
CASE DESCRIPTION: A 4-year-old male patient presented with a mass in the tail region of the left eyebrow. Contrast-enhanced computed tomography
