Osteoarthrosis as the cause of craniomandibular pain and dysfunction: a unifying concept.

It has been demonstrated that osteoarthrotic changes in the temporomandibular joint (TMJ) and in other synovial joints show a similar course, both clinically and (ultra)microscopically. Initially, cartilage changes and possibly also changes in the synovial membrane set up a vicious cycle of cartilage breakdown accompanied by attempts at repair. When the degradative process exceeds the response of repair, the osteoarthrotic disorder progresses into clinically detectable stages. Frequently, the gliding capacity of the articular disc is also impaired, giving rise to an internal derangement. In th

Craniofacial allometry in the OIM-/- mouse model of osteogenesis imperfecta.

Osteogenesis imperfecta (OI) is a skeletal disorder characterized by the impaired synthesis of type I collagen (Col1). This study tests the hypothesis that the craniofacial phenotype of severe OI is linked to an overall reduction in body size. 3D landmark data were collected from µCT scans of adult OIM-/- and wild-type (WT) mice and used to calculate centroid sizes (CS) and interlandmark distances (ILDs). To remove the effect of body size, ILDs were scaled against craniomandibular lengths and CS. Mann-Whitney U tests were used to compare CS and absolute/scaled ILDs between genotypes. OIM-/- mi

Predictors of pain reduction following manual therapy in patients with temporomandibular disorders: a protocol for a prospective observational study.

INTRODUCTION: Temporomandibular disorders (TMDs) are principally characterised by pain in the craniomandibular area and probable limitations of jaw opening. Manual therapy, like other recommended conservative treatments included in clinical guidelines, is commonly used to treat patients with TMD to reduce pain and improve function. However, outcomes may be variable. The aim of this study is to identify predictors associated with pain reduction in patients with TMD following manual therapy by analysing a combination of patient-reported outcome measures and clinical tests. Such knowledge will su

Documented instance of restored conductive hearing loss.

The relationship between temporomandibular joint dysfunction and hearing disorders has long been recognized by some healthcare providers (1,2). Fonder reports that "chronic low-grade otitis media is a constant finding in patients who have a disturbance of the stomatognathical structures due to malocclusion" (3). Fingeroth stated that "a constricted maxillary dental arch frequently results in a decrease in nasal permeability...and within this environment a conductive hearing loss may be present" (4). Histological studies confirm the intimate relationship between the TMJ, the tympanic cavity and

Dental perspective on face pain.

The majority of patients with chronic pain disorders involving the face and craniomandibular mechanism have so-called temporomandibular joint (TMJ) syndrome, atypical face pain, trigeminal neuralgia or neuropathy, or iatrogenic pain. The term "TMJ disorder" is often a misnomer, because the joint itself frequently is not implicated. Diagnosis requires persistent multidisciplined investigation. Initial treatment is aimed at relief of pain and restoration of function. Treatment may then be directed at the source of the problem. For patients with atypical face pain, a complete and detailed history

Review of Various Clinical Assessment Indices and Orthodontic Management for Temporomandibular Joint Disorders.

The term "temporomandibular disorders" (TMDs) refers to a variety of problems involving the muscles of the masticatory system and the jaw. The most common symptoms of TMD are pain in the face, headaches, clicking or popping in the joints, and difficulties with jaw function. The severity of TMD can be measured with a number of different scales, including the Helkimo, Craniomandibular Index (CMI), Mandibular Functional Impairment Questionnaire (MFIQ), Fonseca scale and Diagnostic Criteria for Temporomandibular Disorders (DC/TMD) scales. The former focuses on the patient's chief complaint, while

Craniomandibular Osteopathy in a Newfoundland Dog.

A 5-month-old Newfoundland dog was presented with lethargy and bilateral enlargement of the mandibles and maxillae. A diagnosis of craniomandibular osteopathy was made based on clinical signs, physical examination, and computed tomography findings. Improved mentation and comfort levels were achieved with pain management using meloxicam and paracetamol. Follow-up examinations were performed at 1, 2, 6, and 16 weeks after the first consultation. Stabilization of the condition was noted initially, and by 16 weeks, clinical symptoms had markedly improved. Protuberances of the maxillae were markedl

Craniomandibular/temporomandibular/cervical implications of a forced hyper-extension/hyper-flexion episode (i.e., whiplash).

Clinicians now appreciate the full chain of tissue damage of the interconnecting muscles, tendons, ligaments and fascia: this compromised linkage from the skull through the suboccipital musculature to the cervical spine and anterior/posterior cervical muscles, from the check-rein ligaments and muscles extending from the skull and maxilla to the mandible, from the suprahyoid musculature connecting the mandible through to the hyoid bone, from the hyoid through the infrahyoid to the supporting shoulder girdle, all contribute to damaged interconnecting matrices. Consequently, unresolved tissue dam

Changes in function of the masticatory system from 7 to 10 years of age in relation to articulatory speech disorders.

The aim of the present study was to determine changes in function of the masticatory system in children with articulatory speech disorders and in the controls by re-examining a sample of 287 previously studied children from 7 to 10 years of age. Articulatory speech disorders were diagnosed by the same speech therapist during spontaneous speech and test words using the Remes Articulation Test. Mandibular movement capacity, occlusal interferences, slide between retruded and intercuspal positions, and signs and symptoms of the craniomandibular disorders (CMD) were recorded by a dentist. The follo…

[Short-term effects of the Michigan splint on muscular and joint pain].

BACKGROUND: In 1966 Ramfjord and Ash proposed the Michigan's splint that, at present, is one of the splints widely accepted for the treatment of craniomandibular disorders (CMD). METHODS: For this study, thirty-one subjects were examined according to the protocol of the European Academy of CMD; each patient indicated the intensity of pain using a Visual Analogue Scale (VAS). After therapy with Michigan's splint, the visit was repeated and the patient indicated again the VAS. RESULTS: The results showed that the splint has short-term benefit effects on the muscular and articular pain, even if…

[Clinical evaluation and psychological aspects of temporomandibular joint disorders].

Establishing the patient's clinical diagnosis depends on gathering as much information of the patient and his or her signs and symptoms as possible. This information can be gathered from history, physical and psychological examination, diagnostic analysis. It is also important to look upon pain as a disorder and to consider the relationship between pain and psychological factors. The differential diagnosis is constructed through a biopsychological model of illness rather than through a more traditional biomedical model of disease. To arrive at a consistently accurate clinical diagnosis in pati

[Patient careers. Facial pain and neuralgias].

The correct diagnosis in patients with facial pain is essential to avoid erroneous diagnostic and therapeutic measures. Trigeminal neuralgia is easily recognized by the typical history given by the patient. Treatment of first choice is carbamazepine. When pharmacological treatment fails, surgical options are available. Tolosa Hunt syndrome is judged to be caused by a granulomatous infiltration in the cavernous sinus, treatment consists in corticosteroids. Painful craniomandibular disorders are often mistaken for 'atypical facial pain'. Atypical facial pain, now called 'persistent idiopathic fa…
