A 65-year-old man, who had been treated with dabigatran for 66 days prior to electrical cardioversion, developed extensive intestinal, renal and cerebral thromboembolism five days after cardioversion. There is limited information available on the treatment of thromboembolism in patients being treated with dabigatran. Routine biochemical monitoring is not available. As is the case for vitamin K antagonists, anticoagulation with dabigatran is not without risks.

A patient on oral anticoagulation with dabigatran presented with chest pain and dyspnoea. A CT scan of the chest revealed an intramural haematoma of the ascending aorta with a large pericardial effusion. The patient underwent a modified Bentall procedure. In the absence of a specific antidote for this novel oral anticoagulant medication, even in an emergency situation, successful surgical treatment was possible with an aggressive use of available prohaemostatic agents.

Dabigatran has been approved for prevention of thromboembolic complications in nonvalvular atrial fibrillation. We present a case of thrombosis of a mechanical prosthetic mitral valve during anticoagulation with dabigatran, switched from warfarin. The patient presented with cardiogenic shock, was treated with thrombolytic therapy and discharged with warfarin therapy. Dabigatran in patients with mechanical valve prosthesis should be avoided and echocardiography is indicated if dyspnoea develops in such patients.
