BACKGROUND: Livedoid vasculitis is characterized clinically by smooth or depressed ivory-white scars surrounded by hyperpigmentation and telangiectasia with or without preceding purpuric infiltrated papules and plaques and histologically by intravascular deposition of fibrin. DESIGN: Prospective, randomized double-blind controlled study. The hormonal effects of danazol and of a long-acting gonadotropin-releasing hormone analogue (GnRH-a) were studied in women with endometriosis and those with oophorectomy. We report 2 successfully treated cases of unilateral obstruction owing to endometriosis. Treatment consisted of danazol alone in 1 case and surgical freeing of the obstructed ureter in the other. Danazol alone can reverse completely ureteral obstruction caused by endometriosis but if secondary fibrosis has occurred surgical treatment may be necessary. This is the first case reported of complete resolution of unilateral ureteral obstruction owing to endometriosis using danazol therapy alone.

We treated 69 patients who had hereditary angioedema with danazol to alleviate attacks of mucocutaneous angioedema involving the skin, oropharynx, and gastrointestinal tract, and we documented the continued efficacy of danazol for long-term treatment (1 to 6 years) of hereditary angioedema. Danazol was administered orally at a dosage of 50 mg/day to 17 patients (11 females and 6 males) with idiopathic thrombocytopenic purpura refractory to steroids and/or splenectomy. Although danazol is effective in the treatment of idiopathic thrombocytopenic purpura, its long-term safety and optimal dosage are not well established. Danazol, an attenuated androgen, was administered to four patients with hereditary angioneurotic oedema, with rapid and complete response without side-effects. The follow-up period has now been up to 17 months. In all patients there was an indirect indication that their hormonal state influenced the course of the disease.

Report on the successful treatment of pubertal gynecomastia with the synthetic steroid preparation Danazol. In 15 male adolescents, 11 to 18 years old, Danazol in an average dose of 8 to 10 mg/kg/day, administered orally over 2 to 3 months, achieved a very good or good reduction of breast tissue. 4 patients showed a palpable but insufficient regression. Side-effects of the treatment were rare and clinically negligible. The mechanism of the hormonal action of the gonadotropin inhibiting steroid Danazol in gynecomastia cannot be explained by the hormone findings available so far.
