Delirium superimposed on dementia.

PURPOSE: Delirium superimposed on dementia (DSD) is a frequent and growing emerging clinical challenge, given the increasing prevalence of dementia. METHODS: This narrative review focuses on and discusses the current knowledge on epidemiology, pathogenesis, diagnosis and management of DSD. RESULTS: There is a close interaction between delirium and dementia since dementia is a risk factor for delirium and delirium is a known risk factor for newly developed dementia or worsening of dementia. The occurrence of DSD causes adverse clinical outcomes. However, DSD is often under-recognized or is fr…

[Sepsis masquerading as delirium].

A previously healthy 60-year-old patient presented to the emergency department with severe headache, altered personality and fever. He was treated for bacterial meningitis with delirium of unknown cause but presumed to be due to alcohol withdrawal. Despite receiving the antibiotic therapy regimen recommended for bacterial meningitis the patient's condition rapidly deteriorated with profound delirium and tachypnea. The intensivist who was consulted immediately suspected sepsis-associated organ failure and admitted the patient to the intensive care unit (ICU). The blood culture was positive for …

[Delirium: an interdisciplinary challenge].

Delirium, the acute deterioration of cognitive function and attention, is the most frequent mental disorder in elderly. Its correct diagnosis and adequate management are of crucial importance for the patient's health and functional outcome. First of all, one has to be aware of the possibilities of preventing this complex, potentially life-threatening problem, which means recognizing the patient at risk, avoiding environmental stress and causal factors (i.e., anticholinergic medication) in cognitively impaired patients, and timely reaction to prodromal symptoms. Causal therapy (i.e., treatment …

Delirium masquerading as depression.

OBJECTIVE: Despite the high prevalence of delirium in palliative care settings, this diagnosis is frequently missed, particularly in patients with hypoactive delirium. These patients are also commonly misdiagnosed with depression because of the overlap in symptoms between the two diagnoses. Failure to promptly diagnose delirium can have significant ramifications in terms of delirium reversal, subsequent patient involvement in end-of-life decision making, and the recognition and treatment of other symptoms. METHOD: We report a case of a 63-year-old French-speaking woman admitted to our inpatie…

Confusion, agitation and delirium.

Delirium is one of the most serious and common complications that up to one third of older patients admitted to hospital develop. It is characterized by a disturbance of consciousness, decreased attention, and disorganized thinking that develops over a short period of time, and fluctuates during the course of the day. Delirium post-stroke prevalence ranges from 13 to 48% in general hospitals, and from 10.1 to 28% in Stroke Units. The Confusion Assessment Method and the Delirium Rating Scale are used as delirium screening tools. The cause of delirium is likely to be multifactorial. In stroke, r…

An unusual cause of delirium.

Sleep disordered breathing (SDB) including obstructive sleep apnea (OSA), central sleep apnea (CSA), and Cheyne-Stokes breathing is common i … akenings, nocturnal hypoxia, and daytime somnolence, but only OSA has been reported to present with delirium. We present a patient with clear manifestations of CSA with frank delirium that improved only after BPAP therapy.

[Delirium in palliative care].

Delirium has an abrupt onset and represents an emergency. The symptoms of delirium include reduced consciousness, cognitive failure and altered psychomotor activity. In this paper, we present delirium in palliative care based on a literature review and clinical experience. Delirium is the second most common psychiatric condition in cancer patients in general. The exact prevalence in palliative care is unknown. Several factors can precipitate a delirium. The most common factors are probably opioids, dehydration and metabolic disturbances. The assessment must be balanced towards the patient's li…

Delirium in palliative care.

Delirium is highly prevalent in cancer patients with advanced disease. Frequently a preterminal event, the condition is a sign of significant physiologic disturbance, typically involving multiple medical etiologies including infection, organ failure, adverse medication effects, and in rare situations, paraneoplastic syndromes. Unfortunately, delirium is frequently unrecognized or misdiagnosed and, therefore, inappropriately treated or untreated in terminally ill patients. The clinical features of delirium are numerous and encompass a variety of neuropsychiatric symptoms common to other psychia…

Delirium in acute stroke.

Delirium is the presenting feature in a few stroke patients, but can complicate the clinical course of acute stroke in up to 48% of cases. Old age, extensive motor impairment, previous cognitive decline, metabolic and infectious complications, and sleep apnoea are all predisposing conditions for delirium. Patients with delirium have longer hospitalizations and a poorer prognosis, and are at increased risk of developing dementia. The identification of the patients at risk and non-pharmacological preventative interventions are the key measures in the management of delirium.

Delirium in the hospitalized elderly.

BACKGROUND: As many as one third of elderly hospitalized patients become delirious, and most do not fully recover. Delirium may impart a higher mortality rate and may be a marker for future cognitive decline. OBJECTIVE: To review the clinical features, etiology, diagnosis, and management of delirium in elderly hospitalized patients. SUMMARY: Delirium can be caused by primary intracranial disease, systemic diseases, withdrawal from alcohol or sedative hypnotic agents, or drug intoxication, the most common cause. Because delirium may present with diverse clinical features, physicians should su…

Hypopituitarism presenting as delirium.

A case of hypopituitarism following childbirth (Sheehan's Syndrome) presenting with delirium is described. Diagnosis was confirmed with pituitary stimulation tests and the patient remains symptom-free for the last three years on maintenance hormonal therapy.

[The pathomorphosis of alcoholic delirium].

The authors have examined case histories of 402 patients with delirium tremens treated in 1897-1931, in 1955-1970 and in 1971-1985. An analysis has demonstrated a reduction in the frequency of visual hallucinations of zoological contents and an increase of erotic hallucinations. The duration of the psychotic state in different time periods has been determined. Changes in manifestations of emotional disturbances are described.

Treatment of delirium--a reappraisal.

Delirium is a frequently encountered clinical syndrome which can pose serious problems for the physician and patient. Numerous etiological possibilities exist, and each case is usually associated with multiple causal factors. Although the pathophysiology is poorly understood, the clinical presentation is marked either by stupor and hypoarousal or agitation and hyperarousal. Both types of delirium must be treated by searching for and correcting reversible causative factors. In addition, medication may be quite efficacious in managing the clinical aspects of agitated delirium. Most cases of agit…

Diagnosis and treatment of delirium.

Delirium is not a clinical entity but a symptom-complex of manifold etiology. Its presence signifies acute cerebral insufficiency and often represents a medical and/or psychiatric emergency.Though some forms of delirium have distinctive features, the fundamental phenomena are common to all, with clouding of consciousness the sine qua non. The condition has two major components: (1) the basic "acute brain syndrome" and (2) associated release phenomena.Clinicians must first make the vital differentiation between delirium and "functional" mental disorder, then proceed with the elucidation of the …
