BACKGROUND: Concern that some catheter related bloodstream infections (CRBSI) arise from dental treatment in home parenteral nutrition (HPN) patients results in recommendation of antibiotic prophylaxis. Clinical guideline 64 is widely recognised and observed. There is a lack of consistent guidance for other patient groups viewed at risk from procedural bacteraemia.
METHODS: 1. An email survey of the British Association for Parenteral and Enteral Nutrition (BAPEN) HPN group, requesting physicians' opinions, observations and practises relating to oral health and CRBSI prevention; 2. Comparison

This article reviews and analyzes the recently updated guidelines for the prevention of infective endocarditis (IE) through antibiotic prophylaxis established by the American Heart Association (AHA). Various articles studying methods of preventing IE were reviewed, primarily the AHA's revised guidelines published in April 2007. Lists of reference articles were examined to cross reference relevant information regarding antibiotic prophylaxis for the prevention of IE. A major influence for revisions to the guidelines is that studies show a very small fraction of IE cases are preventable with pro

BACKGROUND: No research has been conducted to assess whether antibiotic prophylaxis prescribing differs by dental setting. Therefore, the goal of this study was to compare the prescribing of antibiotic prophylaxis in Veterans Affairs (VA) and non-Veterans Affairs settings.
METHODS: This was a retrospective study of veteran and non-veteran dental patients with cardiac conditions or prosthetic joints between 2015-2017. Multivariable log binomial regression analysis was conducted to compare concordant prescribing by setting with a sub-analysis for errors of dosing based on antibiotic duration (i

Surgical site infections (SSIs) represent a potential complication in surgical procedures, mainly because clean/contaminated surgery involves organs that are normally colonized by bacteria. Dental, maxillo-facial and ear-nose-throat (ENT) surgeries are among those that carry a risk of SSIs because the mouth and the first respiratory tracts are normally colonized by a bacterial flora. The aim of this consensus document was to provide clinicians with recommendations on surgical antimicrobial prophylaxis in neonates (<28 days of chronological age) and pediatric patients (within the age range of 2

BACKGROUND: The necessity of antibiotic prophylaxis before dental procedures in arthroplasty patients remains unsupported by strong evidence. The aim of this study was to determine the attitudes of dentists and othopedists towards antibiotic prophylaxis.
METHODS: A 15-item questionnaire evaluating the demographic data of dentists and orthopaedic surgeons and their tendencies to administer antibiotic prophylaxis in various cases was delivered online. The data obtained were analyzed statistically using descriptive statistical methods and the Pearson Chi-square test.
RESULTS: A total of 460 res

BACKGROUND: Chronic kidney disease (CKD) still leads to high mortality rates, mainly due to cardiovascular disease. One important influencing factor is persisting low-grade chronic inflammation partly maintained by gingivitis that favors transient bacteremia during daily activities such as toothbrushing.
METHODS: To examine whether intensive dental prophylaxis can restore oral health, reduce the prevalence of bacteremia and degree of systemic inflammation indicated by CRP levels, we conducted this pilot study examining 30 CKD patients aged 6-26 years, 15 receiving intensive prophylaxis (IP),

OBJECTIVE: This study aimed to evaluate the protective capacity of the salivary pellicle in people living with HIV (PLHIV) on antiretroviral therapy (ART) and individuals on pre-exposure prophylaxis (PrEP) for HIV.
MATERIAL AND METHODS: Sixteen participants were divided into three groups: Control (n = 5; HIV-negative, not on ART), HIV (n = 7; PLHIV on ART), and PrEP (n = 4; HIV-negative, on PrEP). Stimulated saliva samples were pooled for each group. Enamel specimens (n = 45) were polished and allocated among the groups. Samples underwent five cycles of enamel pellicle formation (120 µL saliv

BACKGROUND: Subacute bacterial endocarditis (SBE) is an infection of the heart involving damaged valves or endothelium. The most common organisms causing SBE are the viridans streptococci. Viridans streptococci differ in their propensity to cause SBE, which is related to the ability to adhere to damaged heart valves and endothelium, which is a function of extracellular matrix production. Streptococcus intermedius is a member of the S. anginosus group. S. intermedius is one of the many strains of viridans streptococci and a rare cause of SBE. SBE may result following a high-grade, sustained ver

BACKGROUND: Dental procedures can lead to bacteremia and have been considered a potential risk factor for pyogenic vertebral osteomyelitis (PVO). However, data on the association between dental procedures and PVO are limited.
QUESTIONS/PURPOSES: (1) After controlling for relevant confounding variables, are dental procedures associated with an increased risk of PVO? (2) Does antibiotic prophylaxis before dental procedures effectively decrease the risk of PVO?
METHODS: A case-crossover study was conducted to investigate the association between dental procedures and PVO using a Japanese claims

INTRODUCTION: The profession of dental hygienists was created in 1913. However, it does not exist yet in France. Currently, an initiative has begun with creating a high-level professional upgrading of dental assistants into Oral Prophylaxis Assistant, similar to dental hygienists.
METHODS: In order to determine the limits and levers influencing this new qualification and its interests in paediatric dentistry, a mixed methods study (surveys and semi-structured interviews) was conducted.
RESULTS AND DISCUSSION: 119 dental assistants and 34 dentists responded to the survey. Among these, 7 denta

A sample of 44 adults with severe disabilities completed a randomized single-blind cross-over study testing chlorhexidine swabbing under various conditions: with/without prior dental prophylaxis, reduced frequency of application (2 vs. 5 times per week), and prolonged use (42 weeks). All subjects received therapeutic doses of 10 mL 0.12% chlorhexidine gluconate (Peridex, Procter & Gamble) and 10 mL 0.05% NaF applied with a Toothette (Sage Products). Clinical effectiveness of chlorhexidine swabbing compared with placebo was previously reported. In the present study, while initial benefits were

The overwhelming majority of orthopedists and dentists surveyed recommend antibiotic prophylaxis for dental procedures to prevent late prosthetic joint infection. It is time to stop this practice, which is not based on scientific evidence but rather on "myths" of prosthetic joint infections after dental procedures. The first myth is that there are close similarities between late prosthetic valve endocarditis and late prosthetic joint infection. The second myth is that dental treatment is the probable cause of a large percentage of prosthetic joint infections. The third myth is that results of

The aim of the investigation was to collect information from specialized hospitals regarding dental care before and after organ transplantation or replacement of prosthetic joints. 50 transplantation centres and 100 orthopaedic hospitals in Germany were chosen. A questionnaire was used to elucidate the following aspects: Is a dental examination carried out preoperatively? When the patient is discharged, is he or she recommended to have antibiotic prophylaxis before dental treatment? If so, which antibiotic is recommended? The response rate was 56% (n = 28) for transplantation centres. 89% arra

Despite the controversy about the risk of individuals developing bacterial endocarditis of oral origin, numerous Expert Committees in different countries continue to publish prophylactic regimens for the prevention of bacterial endocarditis secondary to dental procedures. In this paper, we analyze the efficacy of antibiotic prophylaxis in the prevention of bacteremia following dental manipulations and in the prevention of bacterial endocarditis (in both animal models and human studies). Antibiotic prophylaxis guidelines remain consensus-based, and there is scientific evidence of the efficacy o

BACKGROUND/AIM: Dentists are responsible for a substantial proportion of antibiotic prescriptions, and antibiotic use contributes to selection pressure for antimicrobial resistance, while inappropriate use increases this burden without corresponding patient benefit. Adequate knowledge of antibiotic indications, prophylaxis and selection is therefore essential in dental education. This study aimed to evaluate the knowledge and awareness of final-year dental students and dental interns in Poland regarding systemic antibiotic therapy and antibiotic prophylaxis in dentistry.

Revised guidelines for the prevention of infective endocarditis published by the American Heart Association in 2007 do not support the indiscriminate use of antibiotic prophylaxis for dental procedures. However, they still recommend the use of prophylaxis for high-risk patients before dental treatments likely to cause bleeding. Given the high prevalence of bacteremia of dental origin due to tooth-brushing, mastication or other daily activities, it appears unlikely that infective endocarditis from oral microorganisms can be completely prevented. A good oral health status and satisfactory level

The objective of this paper is to describe digital imaging technology: available modalities, scientific imaging process, advantages and limitations, and applications to dental hygiene practice. Advances in technology have created innovative imaging modalities for intraoral radiography that eliminate film as the traditional image receptor. Digital imaging generates instantaneous radiographic images on a display monitor following exposure. Advantages include lower patient exposure per image and elimination of film processing. Digital imaging enhances diagnostic capabilities and, therefore, treat
