The authors describe a case in which an elderly patient, who had previously been treated for trigeminal neuralgia, developed a subsequent fracture of the mandible at the site of a further trigger point. This presented difficulty in management of the case, and the necessary treatment is reviewed.

Mandibular flexure is a phenomenon generated by the action of the muscles of mastication and other muscles in the head and neck region which can lead to prosthetic and biological complications such as pain, material fracture, and bone loss around dental implants. To avoid such complications, dividing the mandibular prostheses into 3 short-span prostheses or sectioning at the midline has been suggested. This clinical report presents the management of an edentulous patient with clinically detectable mandibular flexure treated with a 1-piece metal-free complete arch implant-supported prosthesis.

A case is presented of fractured genial tubercles, revealed by occlusal radiography, in a 63-year-old edentulous woman. Isolated fractures are a rare event that may present with pain and edema in the floor of the mouth. It occurs mainly in patients wearing a complete denture when the mandible is atrophied and the genial tubercles are hypertrophied. Only 11 case reports have been found in the English literature.

A 89-year-old, nursing home female presented with a two-week history of drooling and a hard palpable mass in the region of the larynx. Initial fiberoptic laryngoscopy revealed a hard pinkish-white mass which was felt to represent a tumor. Computed tomography demonstrated swallowed, impacted, complete, mandibular denture. Radiologists should be aware of this problem, particularly in the elderly demented patient.

An 11-year clinical research study was conducted with both unloaded bulk hydroxyapatite implants and loaded hydroxyapatite-coated titanium implants. The acquired practical experience in the treatment of 20 in-patients is shared. All of them had advanced and severe atrophy of alveolar process and the body of mandibule. Two methods of vestibuloplastics were mainly applied: according to K. Schuchardt and the method of H. Pichler. The very good early and late results attained as regards the prosthetic field enabled the accomplishment of functionally fit prosthetic construction.
