Contact dermatitis considerations in atopic dermatitis.

Complex immunologic pathways, influenced by both genetic and environment triggers, contribute to the development of atopic dermatitis and allergic contact dermatitis. Suppressing mechanisms between the Th1-driven allergic contact dermatitis and the Th2-driven atopic dermatitis conditions were thought to reduce the simultaneous expression of both; however, recent evidence indicates that pediatric patients with atopic dermatitis are likely to develop clinically relevant positive patch tests and more likely to react to specific allergens, such as lanolin. We review the potential role of skin barr

Alcohol dermatitis: allergic contact dermatitis and contact urticaria syndrome. A review.

In spite of not knowing the numbers of people exposed and obviously incomplete reporting, the known mechanisms of alcohol cutaneous intolerance and the literature classified according to mechanisms are listed. Testing techniques for delayed- and immediate-types are proposed.

Shipboard Contact Dermatitis: An Understudied and Impactful Cause of Occupational Contact Dermatitis.

Occupational contact dermatitis represents a significant concern within the United States Navy. Specifically, contact dermatitis that develops in a shipboard environment represents a unique concern, often leading to unplanned personnel losses. By identifying substances responsible for allergic and irritant contact dermatitis, dermatologists, through performing expanded patch testing and making recommendations to military commanders, play an important role in managing these patients, via ordering expanded patch testing panels and making recommendations to workplace commanders. This article aims

Follicular contact dermatitis revisited: A review emphasizing neomycin-associated follicular contact dermatitis.

Follicular contact dermatitis clinically presents as individual papules that include a central hair follicle. Pathologic features involve the follicle and the surrounding dermis: spongiosis and vesicle formation of the follicular epithelium associated with perifollicular and perivascular lymphocytic inflammation. Using the PubMed database, an extensive literature search was performed on follicular contact dermatitis and neomycin. Relevant papers were reviewed and the clinical and pathologic features, the associated chemicals (including a more detailed description of neomycin), the hypothesized

Occupational contact urticaria and protein contact dermatitis.

Irritant dermatitis and eczema are the most prevalent occupational skin diseases. Less common are immediate contact reactions such as contact urticaria and protein contact dermatitis. Occupational contact urticaria can be subdivided into two categories, immunological and non immunological. However, some agents can induce these two types of reactions. Contact urticaria to natural rubber latex is particularly frequent among health care personnel, but contact urticaria to a wide variety of other substances occurs in many other occupations. Among those at risk are cooks, bakers, butchers, restaura

Black-spot poison ivy dermatitis. An acute irritant contact dermatitis superimposed upon an allergic contact dermatitis.

A black spot in the epidermis over a blister of poison ivy dermatitis is an uncommon finding. Four patients with the phenomenon are described. Histologic and histochemical studies were made on biopsy material and the blackish deposit on the skin surface was compared with black deposits in and on leaves of the species of poison ivy. This examination revealed a yellow, amorphous substance on the stratum corneum of the lesions and a similar substance in and on leaves of the poison ivy plant, Toxicodendron radicans ssp. negundo. Associated with the pigmentary deposits there were distinct changes o

Contact Dermatitis in the Patient with Atopic Dermatitis.

Atopic dermatitis (AD) and allergic contact dermatitis (ACD) are common T-cell-mediated inflammatory skin conditions that can share clinical presentations. The variable expression of ACD among patients with AD represents the shades of gray of the multifaceted relationship between the 2 disorders, where increased allergen permeation in compromised epidermal barriers augments antigen presentation and sensitization, with subsequent immune dysregulation. Further studies are needed to define the relationship and immunologic intersection points of these 2 conditions.

[Contact dermatitis and atopic dermatitis--rare comorbidity?].

Allergic contact dermatitis and atopic dermatitis represent two clinical entities resulting from the activation of two mutually exclusive subsets of T lymphocytes. Therefore, one might expect that the ability of atopic patients to evolve allergic contact dermatitis would be diminished. However, results of recent studies indicate, that similar cytokines' spectra play role in the pathogenesis of both diseases. Similarly, the clinical epidemiological studies reveal comparable prevalence rates of contact hypersensitivity in atopic dermatitis patients and in non-atopics patients. Patch testing with

Ulcerative contact dermatitis caused by sodium silicate. Coexistence of primary irritant contact dermatitis and contact urticaria.

A 57-year-old man had had recurrent ulcerative lesions on his left hand for two years. The ulcers were associated with chronic eczematous changes resulting from primary irritant contact dermatitis to sodium silicate, as indicated by positive patch tests. The patient also had another type of cutaneous reaction to sodium silicate, ie, contact urticaria. An immediate wheal and flare reaction was found 15 minutes after application of sodium silicate to a scratch test site. This response was not seen in healthy control subjects. To our knowledge, the coexistence of primary irritant contact dermatit
