IMPORTANCE: The notion that systemic isotretinoin taken within 6 to 12 months of cutaneous surgery contributes to abnormal scarring or delayed wound healing is widely taught and practiced; however, it is based on 3 small case series from the mid-1980s.
OBJECTIVE: To evaluate the body of literature to provide evidence-based recommendations regarding the safety of procedural interventions performed either concurrently with, or immediately following the cessation of systemic isotretinoin therapy.
EVIDENCE REVIEW: A panel of national experts in pediatric dermatology, procedural/cosmetic dermatol

Guidelines are systematically developed decision aids for specific medical conditions. The German Dermatological Society, together with the German Professional Association of Dermatologists, takes the lead in the development of the guidelines for dermatology in Germany. In addition to national guidelines, European and international guidelines also exist. In 2014 and 2015 German guidelines on the following topics were newly developed or updated: cutaneous larva migrans, anticoagulation during dermatosurgery, pemphigus vulgaris and bullous pemphigoids, Mohs surgery, anal dysplasia, and anal carc

Plastic reconstructions in the head and neck region are common procedures in dermatology. However, the theoretical principles underlying these, which are the basis for successful surgery, are rarely taught in dermatology training. This summary of the basic theoretical concepts for plastic reconstructive surgery is intended to close this gap. Over the past 100 years, repeated attempts have been made to improve the technique of plastic reconstructive surgery and to make it more understandable though different concepts. This includes knowledge of the timing of reconstruction, the trophism of the

BACKGROUND: Although interventions on the skin of children and adolescents often heal better than in adults, inappropriate interventions on nails can lead to avoidable trauma and irreparable damage to fingers and toes.
PROBLEMS: When are interventions on nails indicated in children? What age-related characteristics are important?
MATERIAL AND METHODS: With reference to the literature and experience from own clinical cases, the most commonly occurring constellations in children are discussed and approaches to protective treatment are demonstrated.
RESULTS: Ingrown toenails are one of the mos

Dermatologic and reconstructive surgery involves a range of techniques aimed at maximizing tissue preservation while minimizing postoperative functional impairment. Although classical reconstructive methods remain safe and effective, ongoing innovations enable refinement of established approaches and may contribute to improved tissue preservation as well as enhanced functional and aesthetic outcomes. We present two independent cases involving small-to-medium sized primary defects following surgical excision of a zygomatic cutaneous lesion and a lower lip mucosal/cutaneous lesion. Given the ana

BACKGROUND: Although office-based dermatologic procedures are generally considered safe, there is a lack of prospective data on the rate of adverse events (AEs) associated with these procedures.
OBJECTIVE: To determine the frequency of AEs after dermatologic surgery and to characterize the most commonly encountered AEs.
METHODS: A web-based interface was designed to track AEs with the input of four dermatologic surgeons. Patient demographic and operative data were collected at the time of the dermatologic surgery procedure. AEs occurring at any time during the data collection period were log

Perioperative use of anticoagulation in the context of skin surgery can be difficult. The different forms of anticoagulation act on different points in the coagulation cascade; therefore, different perioperative difficulties exist, which have to be considered preoperatively. The current guidelines "Umgang mit Antikoagulanzien und Thrombozytenaggregationshemmern bei Operationen an der Haut" provides assistance in the decision-making for the practitioner. Individual factors that influence bleeding or coagulation should be considered and evaluated preoperatively. This article gives an overview of

When medical options for axillary hyperhidrosis have failed, botulinum toxin is an effective, safe, and well-tolerated, although temporary, treatment option. For long-lasting or permanent efficacy, some patients turn to local procedures, such as superficial liposuction or manual curettage, or more invasive local surgery. Newer, minimally invasive treatments have become available, such as microwave energy thermolysis.

A pneumothorax usually occurs in young men. From the cosmetic point of view, the size of the skin incision should be considered to be minimal if surgery is applied for young pneumothorax patients. We reported a case of pneumothorax which was operated with a small longitudinal axillary skin incision, the size of which was 3.5 cm long. The patient was a 18-year-old high school student. He was admitted to our hospital with the diagnosis of spontaneous pneumothorax. His chest X-ray showed a moderate collapse of his left lung and bullae in his left apex. Tube drainage was performed and the collapse

The purpose of this article is to present a review of several of the basic bunionectomy criteria as they apply to the so-called soft-tissue bunionectomy. A variation of the McBride bunionectomy is presented using a minimal traumatic technique, and a retrospective study of 80 cases is presented. It can be concluded from this study that the soft-tissue bunionectomy is a viable procedure when the appropriate criteria are present. It does appear, however, that this procedure will be successful in only the mildest cases of deformity.

Limb replantations and revascularizations with a significant muscle bulk may be dangerous and even have life-threatening complications. This is intimately related to the period of muscle anoxia which may lead to postischemia (crush) syndrome with its associated pathological conditions. Once blood circulation is reestablished, every step must be taken to maintain optimal blood flow and to avoid secondary ischemia within the revascularized part, due to increased pressure within muscle compartments as a consequence of traumatic and postanoxic edema formation (compartment syndrome). Therefore, wou

Surgical syndactylism is reviewed as an alternative method of treatment for heloma molle of the fourth interspace. This is achieved by excision of the apposing skin surfaces between the fourth and fifth toes. By maintaining intimate apposition of the two toes, permanent syndactylism of the interdigital space is achieved, therefore preventing reformation of the heloma molle.

Frontal fibrosing alopecia (FFA) is characterized by a receding hairline in the frontotemporal region due to the gradual loss of hair follicles and then follicular stem cells and follicular fibrosis. Follicular stem cells are crucial in skin healing after chemical peeling and other resurfacing procedures. Although there is a possible association of FFA with a history of facial and scalp surgical procedures, there is no information on the safety of cosmetic procedures in patients with FFA. We report five patients with FFA who experienced unusual and complicated outcomes after undergoing a deep

BACKGROUND: As the number of cutaneous surgeries continues to increase, it is important to evaluate the safety of dermatologic surgery in the outpatient setting.
OBJECTIVE: The authors sought to determine postoperative bleeding, infection, dehiscence, and necrosis rates in office-based dermatologic surgery using large flap, large graft, and interpolation flap repairs. The authors evaluated the relationship between these complications and surgical site, closure type, repair size, antibiotic use, and antithrombotic use.
METHODS: Eligible patients were identified through searching the electroni

Dermatologic surgeons are encountering more patients on antithrombotic agents. There are no established consensus guidelines for managing antithrombotic agents in the perioperative period. We provide an updated overview of antithrombotic agents in dermatologic surgery and management of such agents in the perioperative period with additional unique perspectives from cardiology and pharmacy. A literature search of PubMed and Google Scholar was performed to review the English-language medical literature. The landscape of antithrombotic therapy is changing with a notable rise in the use of direct
