A method is described for performing needle electromyography of the diaphragm which is safe and causes little discomfort. It provides valuable information concerning neuropathies and myopathies which may affect the diaphragm, and complements information derived from phrenic nerve conduction studies. Firing patterns of motor unit potentials during spontaneous respiration identify upper motor neurone disorders causing respiratory insufficiency.

We present our experience over five and a half years of acquired hernias of the diaphragm. Each case represents a different presentation of this condition, which may follow blunt or penetrating trauma or iatrogenic injury. Diaphragmatic hernia may present in the acute phase after injury but late presentation may occur after many years, with chronic abdominal or respiratory symptoms, or with acute intestinal obstruction. The problems in diagnosis and management are discussed.

A case history is presented of a child with bilateral eventration of the diaphragm. In view of the absence of musculature and because there are no signs of neurological disorders, we would call this a bilateral congenital diaphragmatic hernia. The patient was operated upon successfully. Pathogenesis and literature of this very rare anomaly are discussed briefly.

This paper reviews 58 patients with traumatic rupture of the diaphragm treated at the Royal Adelaide Hospital during an 11-year period. The series appears to be the largest reported from one centre. The majority of cases resulted from blunt trauma to the trunk caused by motor vehicle accidents. More than half these patients sustained multiple injuries. A smaller group of patients had penetrating injuries to the diaphragm from gunshot or stab wounds. Clinical diagnosis of the ruptured diaphragm was seldom possible; an erect X-ray film of the chest was the best clue to diagnosis. In the acute ph…

The thoracic diaphragm is a dome-shaped septum, composed of muscle surrounding a central tendon, which separates the thoracic and abdominal cavities. The function of the diaphragm is to expand the chest cavity during inspiration and to promote occlusion of the gastroesophageal junction. This article provides an overview of the normal anatomy of the diaphragm.

The diaphragm acts as a partition between the thoracic and abdominal cavities. On computed tomography, it is seen as a curved soft-tissue density with fat below and aerated lung above. The direct multiplanar capability of magnetic resonance technology can improve depiction of normal or abnormal diaphragmatic anatomy. Despite the usefulness of these imaging modalities, adequate visualization of the diaphragm can be difficult. Thoracic surgeons must be familiar with the correlative anatomy of the diaphragm because this knowledge is a prerequisite to making an accurate diagnosis of diaphragmatic …

The Authors studied 30 cases of diaphragmatic traumatisms from 1972 to 2003 to stress the difficulty to achieve an early diagnosis and the need of their immediate treatment: 26 of these patients were male and 4 female (6.5:1); the pathogenesis was in 50% of cases an open trauma and in 50% a closed trauma. The mean age was 36.6 years (33.4 in the open trauma and 41.4 in the closed). The left hemi-diaphragm was affected more frequently (63%) than the right (37%). The associated lesions were mainly of the parenchymatous abdominal organs (spleen 43.3% and liver 49%), while in the thorax lung was i…

A case of nasopharyngeal fibrous diaphragm of unknown origin is reported. In view of the scarcity of publications about this disorder, this new case is interesting. A differential diagnosis is made with choanal atresia and nasopharyngeal stenosis. Aspects of the etiology, clinical manifestations, and treatment are discussed.

The authors report their experience of 21 cases of post-traumatic hernias of the diaphragm. On the basis of the results obtained by discussion of etiologic, diagnostic and therapeutic patterns they make at the conclusion that the high mortality, associated with diaphragmatic hernias, can be reduced only with a short time of diagnosis and a prompt surgical treatment of the associated lesions.

Lipoma of the diaphragm is an extremely rare entity. A case of this disease is herein reported, together with the review of the literature. The patient described was a 70-year-old male, who was admitted to our hospital with an abnormal shadow on his chest X-ray film. The primary tumor, located above the left hemidiaphragm, was successfully resected by video-assisted thoracic surgery, and the final diagnosis of lipoma originating in the diaphragm was made.

Plication of the diaphragm in symptomatic patients with phrenic nerve paralysis provides excellent relief of excertional dyspnea and significantly increases arterial oxygen tension, and all lung volumes except residual volume. We report diaphragmatic plication using the minimally invasive technique of VATS. This procedure provides excellent relief of symptoms with minimal morbidity and short hospitalization.

Diaphragmatic plication is a safe, well tolerated procedure for congenital diaphragmatic eventration. It is the treatment of choice in neonates with respiratory distress following phrenic nerve injury. The abdominal approach is as good as the thoracic and may be safer in cases with central eventration and when the diagnosis of congenital diaphragmatic hernia cannot be excluded with confidence. However, in phrenic nerve paralysis the thoracic approach is superior in showing the anatomical distribution of the phrenic nerve. Experience with eight paediatric patients treated for diaphragmatic even…

Diaphragmatic disruption is an uncommon consequence of blunt trauma and is easily overlooked unless a high index of suspicion is maintained. The diagnosis is important because of the high incidence of associated organ damage and the complications of a missed injury. This article outlines the clinical features and management of the condition.

During a period of 4 1/2 years, 37 infants with congenital diaphragmatic hernia were treated. The overall survival rate was 68%. Survival depended more on cardiopulmonary function than the size of the diaphragmatic defect. There was little evidence that infants with agenesis of the diaphragm formed a special group with a poor prognosis, and four of the ten patients with unilateral agenesis survived. A Dacron prosthesis is recommended as a substitute for the missing diaphragm.
