[Complicated small intestine diverticulitis].

Complicated small intestine diverticulitis Background Diverticula do not only occur in the colon but also in the small intestine albeit less frequently. Small intestine diverticula are usually asymptomatic. Acute complications, such as intestinal obstruction, bleeding or diverticulitis with or without abscess or perforation, are rare. Case description An 85-year-old male presented to the emergency department with abdominal pain since two weeks and a palpable mass in the upper left abdomen. Laboratory tests revealed significantly elevated infection parameters. Computed tomography imaging of the…

[Recurrent diverticulitis - risk factors].

INTRODUCTION: Colonic diverticular disease is a common illness, especially in the elderly population. It may be asymptomatic and cause chronic intestinal problems, colonic diverticular bleeding or inflammatory complications with considerable morbidity and mortality. We have attempted at finding factors that would help us identify patients with a higher risk of diverticulitis recurrence as well as patients with a higher likelihood of perforated diverticulitis. MATERIAL AND METHODS: This retrospective study included all patients admitted to our surgical ward for inflammatory complications of di…

Perforated diverticulitis after surgery.

The authors report on three patients who had a perforated sigmoid diverticulum after unrelated surgery. If the preceding operation is remote in time from the perforation, detection is relatively easy, but if the perforation occurs shortly after major abdominal surgery, its presentation may be totally masked by the postoperative state. Awareness of this possibility may help alert the clinician to the presence of perforation of a sigmoid diverticulum in unexplained postoperative collapse. Although discovery of these three cases suggests that diverticular perforation is not uncommon after surgery

Diverticulitis: selective surgical management.

The surgical treatment of complications of diverticulitis remains most challenging. A review of twenty years' experience with one hundred fifteen cases is presented with one proved anastomotic leak and no deaths. Interval primary resection with anastomosis for chronic recurrent disease including colovesical fistula and mesocolic abscess was proved sate with low morbidity. The three-stage procedure for perforated diverticulitis with spreading peritonitis or pericolic abscess was associated with a high rate of complications and morbidity. An aggressive approach with resection without anastomosis…

The Microbiome and the Etiology of Diverticulitis.

This article provides a review of the existing literature on the microbiome and metabolome of diverticulosis and diverticular disease. Existing studies indicate the microbiome is less likely a cause in the development of diverticulosis. Current evidence highlighting likely etiologic microbes and metabolites in the pathogenesis of diverticular disease is presented. As most data at this time is descriptive and lacks rigorous testing in an animal model of diverticular disease, these findings are extrapolated from the existing understanding of the microbiome in inflammatory bowel disease.

Contemporary management of diverticulitis.

The treatment of diverticulitis is experiencing a shift in management due to a number of large scale clinical trials. For instance, clinicians are beginning to recognize that avoidance of antibiotics in uncomplicated diverticulitis is not associated with worse outcomes. Additionally, while the decision to proceed with elective surgical resection for recurrent uncomplicated disease is less conclusive and favors a patient-centric approach, complicated disease with a large abscess denotes more aggressive disease and would likely benefit from elective surgical resection. Lastly, in patient with ac…

Anastomotic Considerations in Diverticulitis.

Diverticulitis is a common indication for colorectal surgery, both in the acute and the elective setting. The anastomosis between the colon and rectum is a critical component of colectomy for diverticular disease and should be approached thoughtfully. This article reviews important surgical considerations when creating a colorectal anastomosis in the setting of diverticular disease, whether following the reversal of an end colostomy, during an acute episode of diverticulitis, or electively for chronic or complicated disease. Timing of surgery and preoperative assessment, minimally invasive app…

Emergency Presentations of Diverticulitis.

Acute diverticulitis is a common condition that has been increasing in incidence in the United States. It is associated with increasing age, but the pathophysiology of acute diverticulitis is still being elucidated. It is now believed to have a significant contribution from inflammatory processes rather than being a strictly infectious process. There are still many questions to be answered regarding the optimal management of acute diverticulitis because recent studies have challenged traditional practices, such as the routine use of antibiotics, surgical technique, and dietary restrictions for…

Ultrasound of colon diverticulitis.

Overall, the diagnosis of diverticulitis is more reliably made by computed tomography (CT) than by ultrasound (US). However, since US is often used as a first modality in acute abdomen, it is important to be aware of the US signs of diverticulitis. Besides, in not too obese patients, US may be superior to CT. US is most useful in early, uncomplicated diverticulitis. Daily, repeated US examinations in patients with diverticulitis have taught that diverticulitis, in the majority of cases, runs a predictable and benign course. Initially, there is local wall thickening of the colon with preservati…

Diverticulosis and diverticulitis of the appendix.

Diverticulosis of the appendix is a relatively rare pathological finding. The majority are acquired pseudodiverticula. True congenital diverticula are very rare. Appendiceal diverticulosis is usually an incidental finding and clinically asymptomatic. When symptomatic, it is usually complicated by acute or chronic diverticulitis with or without acute appendicitis. It presents with atypical abdominal signs and symptoms, mostly in adult males. Appendiceal diverticulitis is a distinct entity with several clinical and pathological differences from acute appendicitis. It has a more rapid progression…

Diagnostic imaging for diverticulitis.

The misdiagnosis rate of diverticulitis based solely on clinical and laboratory criteria is high and given the potential severity of diverticulitis and its complications, early and routine radiologic evaluation is recommended. Imaging is used to establish the diagnosis and its extent and severity, and to detect the presence of any complications so that management can be directed appropriately. The most widely used examinations for the diagnosis of diverticulitis are ultrasound, barium enema, computed tomography (CT), and recently, magnetic resonance imaging. Over the past decade CT has replace…

[Pylephlebitis in the course of diverticulitis].

Pylephlebitis or septic portal thrombophlebitis is a rare but serious condition which may occur following intra-abdominal sepsis from any source. Sigmoid diverticulitis is one of the most common sources. Modern imaging modalities, particularly CT, have increased the recognition of this condition. Standard treatment consists of anticoagulation plus antibiotic therapy to cover anaerobic and gram negative organisms. The duration of anticoagulation therapy remains controversial. Sigmoid colectomy may be required in cases of perforated diverticulitis or failure of medical therapy.

[Standards in diagnosis of diverticulitis].

The diagnostic procedure is determined by the severity of the diverticulitis. In complicated cases of diverticulitis, it is necessary to detect those patients with obstructive ileus, perforation, and peritonitis who require instant emergency surgery. In all other cases, diagnostic procedures serve as a tool to determine the best therapeutic options. The CT scan of the abdomen seems to have the highest reliability for determining therapy. Other investigations such as barium enema and coloscopy are not indicated in emergency cases or cannot exactly describe the stage of the diverticular disease.…
