[Dizziness in old age].

Dizziness is a frequent and often multifactorial symptom in older patients that can significantly impair the quality of life. The causes are diverse and differ from younger patients. Polyneuropathy, orthostatic dysregulation, drug-related causes, positional vertigo and bilateral vestibulopathy are more frequent in old age. A detailed and vertigo-specific patient history, including the assessment of risk factors such as frailty, polypharmacy and comorbidities, is essential for the diagnostics. The treatment depends on the underlying cause and includes physiotherapeutic interventions as well as …

[Dizziness and Blood Pressure].

Dizziness is most frequently caused by blood pressure dysregulation comprising a broad spectrum from constitutional and orthostatic hypotension to severe conditions like endocrinopathies and neurodegenerative diseases with autonomic dysfunction like in multiple system atrophy.

[Persistent postural-perceptual dizziness].

AIM: To study persistent postural perceptual dizziness (PPPD) in outpatients with benign paroxysmal positional vertigo (BPPV) and patients with presbiataxia (PAt). MATERIAL AND METHODS: Eighty-four patients with PPPD, including 14 with Meniere's disease (MD), 19 with BPPV, 17 with a history of ischemic stroke (IS) in the vertebrobasilar system and 34 with Pat, were examined. For the diagnosis of anxiety, the original 15-point questionnaire with the Likert Scale structure was used. Patients received anvifen (aminophenylbutyric acid hydrochloride) in dose of 250 mg 3 times a day for 6 weeks. Re…

Dizziness: a diagnostic approach.

Dizziness accounts for an estimated 5 percent of primary care clinic visits. The patient history can generally classify dizziness into one of four categories: vertigo, disequilibrium, presyncope, or lightheadedness. The main causes of vertigo are benign paroxysmal positional vertigo, Meniere disease, vestibular neuritis, and labyrinthitis. Many medications can cause presyncope, and regimens should be assessed in patients with this type of dizziness. Parkinson disease and diabetic neuropathy should be considered with the diagnosis of disequilibrium. Psychiatric disorders, such as depression, an…

Differential diagnosis of dizziness.

PURPOSE OF REVIEW: Dizziness is one of the most common complaints among patients presenting to primary care physicians, neurologists, and otolaryngologists. This symptom is nonspecific and includes a broad differential diagnosis. The current review aims to present a general overview of the approach to dizziness as well as to discuss the more common causes in detail. RECENT FINDINGS: The term dizziness encompasses a large spectrum of symptomatology. Understanding how to differentiate between vestibular disorders and other types of dizziness is the key to the evaluation and management of dizzy …

Driving disability and dizziness.

PROBLEM: People with dizziness caused by vestibular (i.e., inner ear) disorders complain of difficulty driving. Physicians occasionally warn their patients with vestibular disorders not to drive. Few studies have asked patients about their driving performance, so little data are available. METHOD: Using the Driving Habits Questionnaire, the authors did structured interviews with people with several different vestibular disorders and with normal subjects. The self-reported crash rate and rate of citations for moving violations did not differ between the subject groups. RESULTS: Patients repor…

The epidemiology of dizziness and vertigo.

This chapter gives an overview of the epidemiology of dizziness, vertigo, and imbalance, and of specific vestibular disorders. In the last decade, population-based epidemiologic studies have complemented previous publications from specialized settings and provided evidence for the high burden of dizziness and vertigo in the community. Dizziness (including vertigo) affects about 15% to over 20% of adults yearly in large population-based studies. Vestibular vertigo accounts for about a quarter of dizziness complaints and has a 12-month prevalence of 5% and an annual incidence of 1.4%. Its preval…

Vertigo and dizziness in children.

Vertigo and dizziness of at least moderate severity occur in >5% of school-aged children and cause considerable restrictions in participation in school and leisure activity. More than 50% of dizzy children also have headache. Vestibular migraine and benign paroxysmal vertigo as a migraine precursor are the most common diagnoses in dizziness clinics for children and adolescents. They account for 30-60% of diagnoses. Other common causes are somatoform, orthostatic, or posttraumatic dizziness. All other disorders that are known to cause vertigo and dizziness in adults also occur in children, but …

[Epidemiology of dizziness and vertigo].

Dizziness and vertigo rank among the most common symptoms in medical practice and belong to the 10 most common reasons for a neurological examination. Epidemiological data on dizziness, vertigo and underlying specific disorders of vestibular origin are useful for clinical decision making, may contribute to a better understanding of disease mechanisms and help evaluate the state of patient care. This article gives an overview on the epidemiology of dizziness/vertigo and of four specific vestibular disorders: benign paroxysmal positional vertigo, vestibular migraine, vestibular neuritis and Meni…

Approach to the dizzy patient.

As dizziness can be caused by so many different pathophysiological mechanisms, it is crucial to determine the type of dizziness before proceeding with the diagnostic evaluation. Vertigo, defined as an illusion of movement, is an important subtype of dizziness that indicates a lesion somewhere within the vestibular system. Probably the most useful feature for differentiating between peripheral and central causes of vertigo is the associated symptoms. Vertigo of peripheral origin is typically associated with auditory symptoms such as hearing loss and tinnitus, while vertigo of central origin is …

Diagnoses in a dizziness unit.

Data are given on 2,515 patients who attended the Dizziness Unit on 3,113 visits. The sex incidence was 58% female, 42% male, and the sex preference most marked in the psychogenic group from ages 20 to 50. Dizziness is mainly a condition of middle-age; with a steadily aging population we can expect a disproportionate increase in patients with dizziness. Psychogenic dizziness was the commonest of all diagnoses (21.1%), followed closely by "undiagnosed"; together, these two comprised 40% of all diagnoses. Obviously, they deserve more clinical attention than such relatively rare conditions as del…

Evaluation of the dizzy patient.

The differential diagnosis of dizziness is difficult, yet challenging. There continues to be a need for improvement in assessment and evaluative procedures for patients with symptoms of dizziness. The otologist must exclude a wide variety of potential disorders which could be sources of the symptoms. The key to a well-made differential diagnosis is a very carefully taken, thorough, medical history, and the integration of that history with the physical examination, the audiometry, and the vestibular evaluation.

The dizzy, the giddy and the vertiginous.

For optimal examination of the giddy patient the case history is the best basis. The description of what the patient feels is not as important as has been stated rather often. The words dizzy, giddy and vertiginous from a patient do not really mean different things - they only mean: I feel unbalanced or disequilibrated. In the anamnesis it is much more important to find out firstly whether the disturbances are: (1) paroxysmal; (2) one acute attack; (3) chronic, (4) positional (positioning), and secondly whether the giddy feeling is accompanied by other disturbances (deafness, headache, pains,
