A 44-year-old woman presented with right upper abdominal pain and nausea with a 4-day duration. An abdominopelvic CT scan revealed a 2.3-cm sized cystic lesion in the duodenum with inflammatory changes and partial thrombosis in the distal main portal vein. Empirical antibiotics were initiated considering acute duodenal diverticulitis, but the symptoms worsened. Thirty-two patients with biliary or pancreatic disease were evaluated by duodenoscopy so as to establish the macroscopic characteristics of the papilla of Vater. In two cases, endoscopic pancreatography was performed as a diagnostic evaluation of the papilla, 30 patients were operated. Our results seem to indicate that the diagnosis of Odditis may be established by endoscopy.

On the basis of 18 personal cases, the authors discuss the different radiological appearances of tumours of the ampulla of Vater region. After a review of the histopathological aspects, they attempt to separate the respective indications for the various endoscopic and radiological studies possible, stressing the importance of duodenoscopy. They give an illustrated classification of the different appearances seen.

Adenocarcinoma is an uncommon gastrointestinal malignancy. Fiberoptic duodenoscopy is the diagnostic procedure of choice, allowing direct visualization as well as biopsy of the neoplasm. Conversely, in the vast majority of cases, CT is unable to demonstrate the duodenal tumor. We describe a technical procedure that has permitted CT visualization of a small ampullary tumor in two cases.

BACKGROUND: Current guidelines recommend performance of oesophago-gastro-duodenoscopy at the time of diagnosis of hepatic cirrhosis to screen for oesophageal varices. These guidelines require people to undergo an unpleasant invasive procedure repeatedly with its attendant risks, despite the fact that half of the people do not have identifiable oesophageal varices 10 years after the initial diagnosis of cirrhosis. We aimed to assess the frequency of celiac disease (CD) in patients with Familial Mediterranean Fever (FMF). This prospective study was carried out from October 2015 to March 2016 and included 303 patients with FMF. We used 98 sex- and age-matched healthy subjects as a control group. Two cases of children with relapsing pancreatitis due to intramural gastrointestinal duplications with ductal communication to the pancreas are reported. A gastric duplication with ectopic pancreatic tissue was detected by endoscopic cholangiopancreatography in the gastric antrum of a 6-yr-old girl. A periampullary duodenal duplication was visualized preoperatively by duodenoscopy and computed tomography in a 10-yr-old boy. Resection of the duplication was curative in each case.

Hemorrhage through the pancreatic duct (hemosuccus pancreaticus) is discussed in a patient with alcoholic chronic pancreatitis and anemia. The pancreatic duct as the source of bleeding was determined by use of duodenoscopy; abdominal arteriograms were normal. It is postulated that the patient's bleed was intermittent and of venous origin, probably associated with pancreatic pseudocyst formation. Total parenteral nutrition could be considered in treatment of the condition if bleeding is not life threatening.

Brain abscess is a rare complication of esophagogastro- duodenoscopy (EGD) with few reported cases in the literature. In this report, we discuss a patient presenting with altered mental status, headache, and dysarthria due to brain abscess caused by S. intermedius shortly after an EGD with an esophageal biopsy showing a new diagnosis of eosinophilic esophagitis. We highlight the rare association of EGD and brain abscess, and discuss the importance of prompt diagnosis and treatment.

The method of choice to detect tumors of the papilla of Vater is endoscopic diagnosis. However, no definite decision on the dignity can be made through the macroscopic appearance itself. Only the combination of various diagnostic methods (side view duodenoscopy with biopsy, endosonography, ERCP, IDUS) allows a reliable statement on the local growth of tumors and the dignity. If there are no signs for local infiltration, histological indications for a carcinoma or distant metastasis, the endoscopic resection in specialized centers is recommended.

A new technuque of ERC(P) using a forward-veiwing fiberscope is described in details. By this procedure the position of the maximal bended tip of the instrument provides a more selective cannulation of the common bile duct, preventing the risk of pancreatic duct filling. The method was performed in 15 cases and succeeded in 12. ERCP, using this technique, can be performed after normal esophago-gastro-duodenoscopy. However, at present time it is not a routine procedure. Technical improvements on the instrument are necessary to facilitate the method.

A thirty eight year old female with a long standing history of a chronic duodenal ulcer presented at the Nyeri Provincial General Hospital with vomiting, abdominal pain and abdominal distension. Oesophago-gastro-duodenoscopy revealed a tight pyloric stenosis while abdominal ultrasonography showed ascites. At laparatomy, she was coincidentally found to have pneumatosis cystoides intestinalis (PCI). The recent literature is reviewed and this case of PCI associated with ascites and pyloric stenosis secondary to a chronic duodenal ulcer is reported.

Annular pancreas, an uncommon condition, is often hard to diagnose before operation. We report a case of annular pancreas with pancreatitis of the annulus that was diagnosed by the use of computed tomography followed by endoscopic retrograde cholangiopancreatography. An upper gastrointestinal series and duodenoscopy revealed stenosis of the second portion of the duodenum. Computed tomography revealed enlargement of the annulus of the pancreas. Two years later, follow-up computed tomography demonstrated calcifications of the annulus, although the remaining pancreas seemed to be normal.

Liposarcoma is the most common malignant soft tissue tumour in adults occurring predominantly in the retroperitoneum and extremities but very rarely within the gastrointestinal tract. We report on a 77-year-old gentleman who presented with a history of melaena and anaemia. On oesophagogastric duodenoscopy a duodenal polyp was identified. Surgical excision was performed and on histology, the duodenal polyp revealed a primary duodenal well differentiated liposarcoma. A literature review confirmed the rarity of primary duodenal liposarcomas, with only four cases previously reported.
