[Middle Ear Malformations].

Middle ear malformations (MEMs) represent a diverse group of congenital anomalies with significant implications for auditory function. These malformations, which occur in approximately 0.5 to 3% of conductive hearing loss cases, can arise from various genetic and environmental factors. They often manifest unilaterally and may occur in isolation or as part of a syndromic condition. MEMs are closely associated with abnormalities of the external ear and less frequently with inner ear anomalies.Embryologically, the middle ear develops from the first and second pharyngeal arches, with interactions …

Otology: Ear Infections.

Acute otitis media (AOM) is a common diagnosis in children who present with symptoms of otalgia, fever, or irritability and is confirmed by … ent treatment with or allergy to this drug. Otitis media with effusion (OME) is fluid in the middle ear without symptoms of AOM and typically resolves within 3 months. Tympanostomy tube placement is the most common ambulatory surgery for children in the United States. It is used to ventilate the middle ear space and may be performed to treat recurrent …

[MIDDLE EAR CHOLESTEATOMA].

Cholesteatoma is the most severe middle ear disease. In most cases cholesteatoma results from under-aeration of the middle ear cleft. It is characterized by otorrhea and hearing loss, and may lead to severe complications, intra- and extra-cranial. There is no non-surgical treatment for cholesteatoma. Surgical treatment is the only option. Cholesteatoma is diagnosed by micro-otoscopy. When cholesteatoma is suspected and otoscopy is inconclusive, imaging is mandatory. The goal of treatment in cholesteatoma is to achieve a stable and safe ear. In most cases, upon diagnosis, cholesteatoma is quite…

Robotic Ear Surgery.

Robots under research and development for otology can be classified as collaborative (intervention is constrained by the robot but surgeon directly actuates the end-effector), teleoperated (surgeon remotely controls a tool with modification [eg, tremor reduction] by the robot), or autonomous (surgeon monitors the robot performing a task). Current clinical trials focus on more accurate stapes surgery, minimally invasive access to the cochlea, and less traumatic insertion of cochlear implant electrode arrays. Autonomous approaches to major aspects of surgical interventions (eg, mastoidectomy) wi…

Endoscopic ear surgery.

OBJECTIVES: This article reviews the advantages and disadvantages of endoscopic ear surgery (EES). METHOD: Pubmed, Google and the Proquest Central Database at Kırıkkale University were queried using the keywords "endoscopic ear surgery", "ear surgery" and "endoscopy" to identify the literature needed for the review. RESULTS: Endoscopes allow for enhanced surgical visualisation. The distal part of the apparatus is illuminated and contains lenses angled to allow a wider view of the operative area. Transcanal endoscopic techniques have transformed the external ear canal (EAC) into an operative …

Inner ear disorders.

Auditory part of the inner ear transforms mechanic energy to the electric energy. The key component of this process are hair cells, which damage leads to often irreversible hearing impairment. The damage can occur due to several external and internal causes and the treatment focuses mainly onto the mitigation of the consequences. Sometimes sensorineural hearing loss can be symptom of some severe disease and thus it can lead to its early diagnosis.

Middle ear damages.

Middle ear damages. The eardrum and middle ear are often exposed to blunt and penetrating trauma, blasts, thermal or caustic injuries. These injuries may result in tympanic membrane perforation, middle ear haemorrhage, dislocation and fracture of the ossicular chain, perilymphatic fistula and damage to the chorda tympani and/or facial nerve. In case of life-threatening injuries and/or mass casualty incidents, middle ear trauma obviously does not take highest priority. However, middle ear lesions should be suspected and recognized as early as possible. After meticulous history taking, physical …

Ear wax management.

BACKGROUND: Ear syringing is a very common practice among general practitioners (GPs). It is used by many as the treatment of choice for cerumen (ear wax), and is usually effective and safe. However, complications from syringing are an increasingly common reason for presentation to ear, nose and throat (ENT) specialists and medico-legal complaints against GPs. OBJECTIVE: The aim of this article is to provide GPs with the knowledge to safely manage cerumen. DISCUSSION: Ear wax is an important part of a normal functioning ear. It is mostly asymptomatic and requires no treatment. Softening ear …

Natural ear rings.

Ear lobe keloids are common following ear piercing and these lesions are conspicuous and cosmetically unappealing. Multiple methods including surgery, radiotherapy, anti mitotic agents, silicone sheet, pressure clips, and cryotherapy have been used. The Challenge is to have a good cosmetic outcome with minimal recurrence.

Inner ear disorders.

OBJECTIVES: To present a framework for the diagnosis and treatment of inner ear disorders, with an emphasis on problems common to neuro-rehabilitation. INTRODUCTION: Disorders of the inner ear can cause hearing loss, tinnitus, vertigo and imbalance. Hearing loss can be conductive, sensorineural, or mixed; conductive hearing loss arises from the ear canal or middle ear, while sensorineural hearing loss arises from the inner ear or auditory nerve. Vertigo is a hallucination of motion, and is the cardinal symptom of vestibular system disease. It should be differentiated from other causes of dizz…

[Middle-ear adenoma].

In our case we present a 47-year old female patient, who had a history of one year of right-sided recurrent ear pain without any signs of ear-infections. A computer tomography (CT) imaging was carried out. At the mid-modiolar level, the axial CT-scan showed a small contrast-enhancing density in the middle ear space. She refused further investigation until the ear-pain increased and slight hearing loss was present. Again, six months later a CT-scan was performed, and the hyperdense tissue formation engaged the entire middle ear cleft of the right temporal bone. Typical of MEA, no osteolytic sig…

[Middle ear cholesteatoma].

Cholesteatoma is a serious form of chronic otitis media. The aim of this paper is to present the state of the art of disease management, including recent data from the literature and the authors' derived from their Mentors' teaching, Professor Pierre Roulleau (Paris, France) and Professor Robert Charachon (Grenoble, France). The main recent advances concern the use of cartilage grafts to reconstruct the canal wall and/or tympanic membrane (allowing a significant reduction in residual cholesteatoma) and progress in medical imaging allowing more acute preoperative determination of extension of t

[External ear melanoma].

Cutaneous melanomas are the tumours that have increased more their incidence in the last fifty years. Melanomas arising from the external auditory canal are extraordinariously unfrequent. These tumours show an aggressive and silent behaviour, and due to this the diagnosis is frequently made in an advanced stage. A male with a malignant melanoma arising from his left external auditory canal was attended in our department, suspecting an epidermoid carcinoma. The clinical findings and the extension of the lesion required a lateral temporal bone resection, parotidectomy and neck dissection to achi…

Acquired ear defects.

Trauma and tumor are the causes of acquired ear defects that surgeons are frequently called on to treat. Trauma may result in hematoma or laceration of the ear. In addition, both trauma and tumor excision may result in skin or cartilage loss. Prevention of recurrence depends on complete excision of the tumor m both lateral margins and depth. Tumors of the ear (as well as the nose) are often incompletely excised because of the difficulty in reconstruction. The absence of subcutaneous tissue in the ear allows fixation and perichondrial involvement to occur early. The tumor affinity for perichond…

Middle-ear myoclonus.

Tinnitus produced by repetitive contraction of the middle-ear muscles is a rare condition. We present an interesting case of bilateral middle-ear myoclonus causing incapacitating tinnitus in a patient with multiple sclerosis. Otological examination demonstrated rhythmic involuntary movement of the tympanic membrane. These movements correlated with a rhythmic 'rushing wind' noise perceived by the patient. Oropharyngeal examination showed no evidence of palatal myoclonus. Impedance audiometry confirmed rhythmic change in the middle-ear volume. Medical management was unsuccessful. The patient's t…

Middle ear meningiomas.

Meningiomas arising in or presenting as middle ear lesions are relatively uncommon. This study retrospectively reviews the clinicopathologic features of six meningiomas arising in or extending into the middle ear. The patients comprise five women and one man ranging in age from 45 to 67 years (median, 55 years) at the time of surgery. Five tumors arose in the posterior fossa or temporal bone region and one tumor arose from the auditory canal itself. Three tumors arose on the right side and three on the left. Duration of symptoms before surgery involving the middle ear was known in five patient…
