Ectopia lentis has a myriad of causes, with Marfan's syndrome and homocystinuria being well-known causes. PURPOSE: To describe 3 cases of ectopia lentis (EL) associated with primary congenital glaucoma (PCG) and differentiate between primary and secondary EL regarding pathogenesis.

Three siblings with ectopia lentis were examined for associated abnormalities. Case 1 had Axenfeld-Rieger anomaly and retinal detachment. Case 2 had Axenfeld-Rieger anomaly, ciliary staphyloma, and glaucoma. Case 3 had no associated ocular abnormality. The parents had normal eyesight. This is the first report of ectopia lentis associated with Axenfeld-Rieger anomaly. The mode of inheritance is likely autosomal recessive.

Hereditary simple ectopia lentis affected nine patients in three generations of a family. Inheritance appeared to be autosomal dominant. Examination of 12 family members, employing body proportion measurements, chest x-ray, echocardiogram, and urinary cystine or blood methionine levels, revealed no evidence of any systemic disease. The purpose of this article is to report an unusual bilateral inferior subluxation of the lens in a patient with Marfan's syndrome. A 14-year-old boy presented with gradual painless diminution of vision in both eyes. His family history showed that his maternal uncle also had similar complaints. Although the removal of the lens from patients with ectopia lentis has long been associated with a high rate of surgical complications, the surgical outlook for these patients is improving now that more sophisticated surgical modalities are available. We treated a series of eight consecutive patients with pars plana lensectomy (12 eyes) whose visual acuity could not be satisfactorily improved with optimal optical correction (phakic or aphakic).
