Clinical features of laryngeal myasthenia gravis: A case series.

BACKGROUND: Myasthenia gravis (MG) is an autoimmune disease. Dysarthria, dysphagia, and difficulty swallowing as exclusive initial and prima … response in the repetitive nerve stimulation (RNS) test, and 92.6% were positive in the neostigmine/edrophonium test. Fluctuating weakness was examined in 16 of 30 patients and observed in 14/16 patients. CONCLUSION: Laryngeal MG is a rare and possibly under-diagnosed condition. The patients can present with dysarthria, dysphagia, or difficulty swallowing. F …

Myasthenia gravis focal to the larynx. Report of a case.

Myasthenia gravis, focal to the larynx, should be considered in the differential diagnosis of intermittent breathy dysphonia and aponia. A 20-year-old woman had intermittent breathy dysphonia and aphonia as the sole manifestation of myasthenia gravis. The vocal cords were flaccid, and adducted to the paramedian position for phonation. Improvement of laryngeal function was demonstrated with edrophonium and neostigmine bromide. Therapy with pyridostigmine bromide (Mestinon) maintained normal phonation.

Myasthenia gravis presenting as laryngeal stridor after exposure to chlorine gas.

The patient described had myasthenia gravis manifested by laryngeal stridor after accidental exposure to chlorine gas. The diagnosis was established by correction of abnormal pulmonary function after injection of edrophonium chloride. The patient subsequently had generalized myasthenia gravis. The autoimmune injury causing this disease may have been the chlorine gas exposure, since the temporal relationship is very strong and chlorine is important in the modulation of acetylcholine synthesis.

Efficacy of Gaze Photographs in Diagnosing Ocular Myasthenia Gravis.

BACKGROUND AND PURPOSE: The various tests that are routinely used to diagnose generalized myasthenia gravis, such as the edrophonium test, serum anti-acetylcholine-receptor antibodies (AChR-Ab), and repetitive nerve stimulation (RNS) tests, have lower diagnostic sensitivity in ocular myasthenia gravis (OMG). Diagnosing OMG becomes even more difficult when the clinical symptoms are subtle. There is no gold-standard diagnostic test available for OMG patients, and so this study compared the diagnostic sensitivity of gaze photographs with conventional tests in OMG. METHODS: Records of gaze photog…

Muscarinic receptor modulation of GABA-mediated giant depolarizing potentials in the neonatal rat hippocampus.

1. The whole-cell patch clamp technique was used to study the role of muscarinic receptors in regulating the frequency of giant depolarizing potentials (GDPs) in CA3 hippocampal neurones in slices from postnatal (P) P1-P8 rats. 2. Atropine (1 microM) reduced the frequency of GDPs by 64.2 +/- 2.9 %. The acetylcholinesterase inhibitor edrophonium (20 microM) increased the frequency of GDPs in a developmentally regulated way. This effect was antagonized by the M1 muscarinic receptor antagonist pirenzepine. 3. In the presence of edrophonium, tetanic stimulation of cholinergic fibres induced either…

Extensive Postradiation Ocular and Diffuse Cranial Neuromyotonia Mimicking Myasthenia Gravis.

BACKGROUND: Ocular neuromyotonia is a rare, but well-recognized, complication of cranial irradiation. CASE REPORT: Using figures and videos, we report a 52-year-old man with extensive ocular, brainstem, and lower cranial nerve neuromyotonia postradiation therapy for a fourth ventricle glioma who, in the context of an apparently positive edrophonium test, was initially misdiagnosed with myasthenia gravis. CONCLUSIONS: This is the first case of postirradiation neuromyotonia to be reported with such extensive cranial nerve and brainstem involvement.

Neurologic complications of a coral snake bite.

A 27-year-old man was bitten by a coral snake in Mexico. Within 24 hours he had ptosis, dysphonia, difficulty chewing, and limb weakness. His symptoms peaked at 72 hours with loss of ambulation. Neurologic examination was consistent with severe myasthenia. Repetitive stimulation of the median nerve showed a postsynaptic defect that was not corrected by edrophonium. He was monitored in an intensive care unit, but received no antivenom globulin or acetylcholinesterase inhibitors. The syndrome abated in 3 weeks.

[Electromyographic and histopathological study of the Lambert-Eaton myasthenia syndrome. Apropos of a case].

In this case of Eaton-Lambert myasthenic syndrome the electrophysiological study demonstrated some modifications of latency of the muscle action potentials evoked by repetitive nerve stimulation at 30 c/sec. specially after administration of edrophonium chloride. This data suggest as possible some fluctuations in the mechanism of the neuromuscular transmission defect. The muscle biopsy showed changes secondary to a peripheral motor nerve involvement; electronmicroscopic study of the motor end-plates revealed a highly developped subneural apparatus.

[Dysphonia as the first sign of myasthenia gravis].

Ten cases of myasthenia are presented in which dysphonia was the initial sign. Isolated signs of dysphonia continued for several months or years, presenting diagnostic difficulties. Eventually neurologic examination followed by electrophysiological investigation (classical repetitive nerve stimulation and single fibre EMG) as well as edrophonium test allowed proper diagnosis of myasthenia, then confirmed by the clinical course. A possible diagnosis of myasthenia should be taken into consideration in cases with isolated dysphonic signs of uncertain origin.

Sporadic oculopharyngeal myopathy with abnormal mitochondria.

A case of late onset ophthalmoplegia and dysphagia is presented. Serum enzymes, ECG, EEG, thyroid function and edrophonium test were normal. Muscle biopsy revealed the presence at oxidative enzymes of 5% of fibres displaying subsarcolemmal hyperactivity and a coarse network pattern, mainly involving type II fibres. At electron microscopy mitochondrial abnormalities with paracrystalline inclusions were detected. The case is discussed in the light of the heterogeneity of ocular myopathies and their overlapping both with one another and with the so called mitochondrial myopathies.

Evaluation of Angelchik antireflux prosthesis. Long-term results.

Fifteen patients with intractable reflux or its complications were sequentially studied after the placement of the Angelchik antireflux prosthesis. In all, 16 devices were inserted. Parameters were measured before and 3, 12, 24, and 36 months after prosthesis placement and included symptom scoring, esophageal manometry with Tuttle test, endoscopy, suction biopsy, barium swallow, and gastroesophageal scintigraphy. In addition, a subset of patients underwent stimulation/inhibition of the lower esophageal sphincter (LES) with pentagastrin, metoclopramide, edrophonium, and atropine. At a mean time…

Idiopathic ventricular fibrillation induced with vagal activity in patients without obvious heart disease.

BACKGROUND: Recently, idiopathic ventricular fibrillation (VF) has gained much attention. Although several subgroups have been described, it … soproterenol, atropine, and exercise stress testing and induction or exacerbation with propranolol, edrophonium, and hyperventilation. VF was induced by programmed electrical stimulation in all cases but two, in which it was induced only after edrophonium injection. In two cases, VF was exacerbated by propranolol, and in all cases, it was uninducible with iso …

Chest pain associated with mitral valve prolapse. Evidence for esophageal origin.

The cause of chest pain associated with mitral valve prolapse remains unclear. A young woman with chest pain ascribed to mitral valve prolapse is described. Response of chest discomfort to atenolol therapy had been poor. The patient's chest discomfort and concomitant esophageal spasm were provoked by intravenous infusion of edrophonium chloride during esophageal manometry. A Bernstein acid infusion test also induced her chest pain. Review of systems revealed intermittent dysphagia and postprandial heartburn. In certain patients with mitral valve prolapse, esophageal motility disorders may be t…

A case report of myasthenia gravis localized to the larynx.

This report describes a very rare case of myasthenia gravis (MG) localized to the larynx, with the onset of dyspnea requiring tracheotomy. The vocal cords of this patient were fixed in the paramedian position. Under fiberscopic observation, improvement of laryngeal function was demonstrated after intravenous injection of edrophonium. However, no voice change occurred. The posterior crico-arytenoid muscle has the sole responsibility of abducting the vocal cord; thus, it is concluded that the dyspnea was caused by the selective paralysis of this muscle. We suggest that MG should be considered in…

Endplate dysfunction causing respiratory failure in a patient with prior paralytic poliomyelitis.

A 56-year-old man with late amyotrophic sequelae from poliomyelitis experienced progressive dyspnoea requiring intubation and artificial ventilation in the intensive care unit. Repetitive stimulation studies showed a marked decrement of the trapezius muscle response reversible with edrophonium. Ventilatory function considerably and lastingly improved under anticholinesterase treatment. In the absence of biological evidence for autoimmune myasthenia gravis, it is suggested that a mechanism implying endplate dysfunction related to postpolio syndrome. Repetitive stimulation procedure should be co…

Simultaneous closure of the tricuspid and mitral valve in Ebstein's anomaly.

Echocardiographic studies of a 23-year-old man with Ebstein's anomaly and type B Wolff-Parkinson-White syndrome showed almost simultaneous closure of the tricuspid and mitral valve. After administration of edrophonium chloride, the tricuspid valve closed 20 msec earlier than the mitral valve. However, the closure of the mitral valve preceded that of the tricuspid valve after administration of ajmaline and atropine sulfate by 90 msec and 60 msec, respectively, concomitant with obliteration of preexcitation. From these observations, the simultaneous closure of the tricuspid and mitral valve of t…

Myasthenia Gravis Presenting with Myasthenic Crisis Mimicking Status Asthmatics.

A 23-year-old woman was transferred to our hospital due to exacerbating dyspnea with wheeze. After admission, we started mechanical ventilation immediately, and she was diagnosed with status asthmatics. On the following day, she was able to be weaned from the ventilator. However, she required re-intubation because of an unstable respiratory condition just after extubation. Detailed neurological investigations identified blepharoptosis and muscle weakness with easy fatigability. An edrophonium test was positive. Anti-acetylcholine receptor antibody was detected in her serum. She was finally dia…
