[Structural epilepsy or herpes simplex encephalitis relapse: diagnostic problems].

The article gives the clinical case of herpes simplex encephalitis relapse with the resistant seizures in a child. What we describe is a clinical approach towards the differential diagnostic of the seizures in structural epilepsy, which are resistant to anticonvulsants, or late herpes simplex encephalitis relapse. Good clinical perspective may be the indication of the intratecal synthesis of the IgG-specific antibodies to the herpes simplex type 1 and 2. Conducting etiotropic treatment with the appointment of acyclovir and pathogenetic therapy with the use of Cytoflavin contributed to the rapi

Ketogenic diet restrains herpes simplex encephalitis via gut microbes.

Herpes simplex virus type 1 (HSV-1) infection-associated herpes simplex encephalitis (HSE) is an occasionally but severe neuronal disease that causes behavioral disorder and impairs cognition. Herein, we demonstrate that the consumption of ketogenic diet (KD), a low-carbohydrate high-fat diet, restricts the neurotropic infection of HSV-1 and HSE progression in mice. KD reduced weight loss, neurodegenerative symptoms, virus production and neuroinflammation, resulting in the enhanced survival rate of HSE mice. Notably, depletion of gut microbes by antibiotics attenuated the protective function o

The clinical approach to managing herpes simplex virus encephalitis.

This article explains the approach to managing a patient with herpes simplex virus encephalitis. Acute encephalopathy is a common and often intimidating presentation in an acute general medical setting. Application of key principles will enable the generalist to take life-saving action before obtaining any specialist input. Viral infection is the most common cause (48.2%) of encephalitis; another large group is cases of autoimmune aetiology. Early diagnosis of encephalitis is crucial to ensure that the right treatment is given on time. Guidelines on the management of viral encephalitis were pu

Autoimmune post-herpes simplex encephalitis of adults and teenagers.

OBJECTIVE: To report 14 patients with immune-mediated relapsing symptoms post-herpes simplex encephalitis (HSE) and to compare the clinical and immunologic features of the teenage and adult group with those of young children. METHODS: Prospective observational study of patients diagnosed between June 2013 and February 2015. Immunologic techniques have been reported previously. RESULTS: Among the teenage and adult group (8 patients, median age 40 years, range 13-69; 5 male), 3 had an acute symptom presentation suggesting a viral relapse, and 5 a presentation contiguous with HSE suggesting a r

Herpes simplex encephalitis following microvascular decompression for trigeminal neuralgia.

The authors present the first reported case of herpes simplex encephalitis (HSE) precipitated by trigeminal nerve microvascular decompression (MVD). The presentation of this specific case together with the pathogenesis and management of HSE are discussed, with a relevant literature review. This 29-year-old woman with treatment-resistant trigeminal neuralgia underwent a successful elective MVD of the right trigeminal nerve. She was discharged but was readmitted 1 week postoperatively with clinical signs and symptoms of meningitis. A CSF sample was obtained through lumbar puncture before she was

Unilateral negative myoclonus caused by herpes simplex virus encephalitis.

Various neurologic manifestations of herpes simplex virus (HSV) encephalitis have been reported on the literatures. Chorea, ballism, choreoathetosis and myoclonus were reported as movement disorders which might be related with brain lesion by HSV encephalitis, but negative myoclonus (NM) has never been reported before. NM can be characterized as a shock-like involuntary jerky movement caused by a sudden, brief interruption of muscle activity. We experienced a case of HSV encephalitis with NM in unilateral arm and leg. In polygraphic monitoring, electroencephalography (EMG) silent periods are 5

Therapeutic hypothermia for space-occupying Herpes simplex virus encephalitis.

Development of space-occupying edema in patients with severe Herpes simplex virus encephalitis is a major factor for high morbidity and mortality. Conventional intracranial pressure-lowering modalities are limited and more aggressive treatment options for such patients have rarely been described. This article reports on a HIV-positive patient with Herpes simplex type 2 encephalitis and elevated intracranial pressure refractory to medical therapy. Induced moderate hypothermia of 33 °C resulted in fast and sustained control of intracranial pressure. After three months, the patient had a fairly g

Common variable immunodeficiency presenting as herpes simplex virus encephalitis.

Common variable immunodeficiency (CVID) is a disorder in which patients are unusually susceptible to infections with encapsulated bacteria. Here we describe three adults with CVID in whom the correct diagnosis was not reached until they had Herpes simplex encephalitis. Failure to identify the underlying immune deficiency likely contributed to these debilitating infections.

[Herpes simplex virus encephalitis in a renal transplant patient].

The incidence of herpes simplex virus (HSV) encephalitis is estimated to occur annually in 2-4 cases per million population. HSV encephalitis is exceptional in renal transplant patients, we have found two previous reports after an extensive bibliography search. We report a case of a 47 years old women renal transplant recipient who presented 3 months after transplantation fever, stupor and aphasia. The diagnosis HSV encephalitis was achieved by PCR in cerebrospinal fluid and magnetic resonance imaging. She was treated with aciclovir for 4 weeks and recovered completely without neurologic seque

Depressive symptoms following herpes simplex encephalitis--an underestimated phenomenon?

OBJECTIVE: In view of the herpes simplex virus' neurotropism for the limbic system and the temporal lobe, little is known about potential negative effects of this necrotizing encephalitis on affective functioning and health-related quality of life (HRQoL) after recovery. We therefore set out to explore an association between herpes simplex virus encephalitis (HSE) and both depressive symptoms and HRQoL. METHODS: A structured telephone interview was conducted in 26 subjects (F/M=10/16; age at follow-up, 49.3+/-15.0 years; range, 29-80) at a mean time interval of 5.2+/-3.1 years (range, 1-11) a

Herpes simplex encephalitis in a domestic rabbit (Oryctolagus cuniculus).

An adult domestic rabbit showing neurological signs was subjected to euthanasia. At necropsy, macroscopical lesions were absent. Histopathologically, extensive lesions were seen, particularly in the cerebral cortex. Non-suppurative meningitis was present and there was lymphocytic and plasmacytic perivascular cuffing in the neuropil. The cerebral cortex showed extensive segmental neuronal and glial necrosis. Within the necrotic areas, large amphophilic intranuclear inclusion bodies were present in neurons and glial cells. Immunohistochemically, neurons and glial cells in the affected areas were

Polymerase chain reaction-proved herpes simplex encephalitis in children.

OBJECTIVE: To investigate the clinical features in PCR-proved herpes simplex encephalitis (HSE) in children, excluding neonates. METHODS: We studied the clinical manifestations and laboratory findings of 24 children in whom the diagnosis of herpes infection was confirmed by the PCR assay and compared them with those of 38 children with central nervous system infections other than HSE. RESULTS: There were no significant differences between groups in the percentage with fever or convulsions, the initial neurologic symptoms or the level of consciousness. Analysis of cerebrospinal fluid showed n

Specific viral antibodies associate with anti-NMDAR encephalitis after herpes simplex encephalitis.

Herpes simplex encephalitis (HSE) patients may develop secondary anti-N-methyl-D-aspartate receptor (NMDAR) encephalitis (NMDARE), associated with worsened long-term neurological outcome. Immunosuppressive treatment can limit NMDAR autoantibody-mediated pathology, but early predictive biomarkers for the risk of NMDARE are lacking. In a multicenter study, we performed unbiased antibody reactome profiling using Phage ImmunoPrecipitation Sequencing (PhIP-Seq). HSE patients with secondary NMDARE (n = 13) versus those without (n = 10) showed enhanced antibody responses against HSV-1, but not HSV-2,

[Autoimmune Encephalitis Triggered by Herpes Simplex Encephalitis: Main Syndrome, Diagnosis, and Treatment].

Autoimmune disorders, such as choreoathetosis and abnormal behavior secondary to herpes simplex encephalitis (HSE), are discussed in this review. These disorders are known to develop without any evidence of HSE relapse, while they respond to immunotherapies such as intravenous corticosteroids. Recent evidence, including a prospective Spanish cohort study of HSE, revealed that autoimmune encephalitis (AE) can be triggered by HSE, which is closely related to several neuronal surface antibodies (NSAs). Anti-N-methyl-D-aspartate encephalitis (NMDARE) is the most common phenotype of AE post-HSE. Mo

Transient Sinus Node Dysfunction Associated with Herpes Simplex Encephalitis.

Sinus arrest and inappropriate sinus bradycardia are sinus node dysfunction (SND), which cause loss of consciousness. Cardiac pacing is recommended in patients with symptoms, such as syncope or dizziness. Several conditions can induce these arrhythmias; however, whether or not intracranial infectious diseases, such as herpes simplex encephalitis (HSE), can cause secondary SND is unclear. We encountered a patient with sinus arrest and transient sinus bradycardia associated with HSE. Since cardiac pause was never monitored and the bradycardia improved after HSE treatment, HSE was suspected to be

Untreated herpes simplex virus encephalitis without a fatal outcome.

Herpes simplex virus encephalitis (HSE) is the most common sporadic fatal encephalitis. Although timely administered acyclovir treatment decreases mortality, neuropsychiatric sequelae is still common among survivors. Magnetic resonance imaging is frequently utilized for the diagnosis of HSE, which typically involves temporal lobe(s) and can be mixed with brain tumors involving the same area. Here, we report a case of HSE, who received acyclovir with a delay of 90 days because of presumptive tumor diagnosis and survived with minimal sequelae.
