Japanese encephalitis is a mosquito borne arbovirus disease that is endemic over a wide area of Asia, India, and the south eastern USSR. The disease is associated with a severe encephalitis, a high death rate and a high incidence of neurological sequelae in survivors. While natives of endemic areas may develop immunity, Western travellers are at risk. To date there have been two reported cases of Japanese encephalitis in people returning to Australia from overseas. A vaccine against Japanese encephalitis is available.

Inactivated Japanese encephalitis (JE) vaccine was given to healthy children and their neutralizing antibody titres against nine JE virus strains were measured. After two shots of the vaccine in the first year, most of the children showed seroconversion, but their neutralizing antibody titres were not high, and one year after vaccination their titres had decreased to low levels. However, a booster dose of vaccine in the second year had a marked effect on the antibody response. The neutralizing antibodies induced by the vaccine reacted to almost the same extent with nine different JE virus stra

Two women, 29 and 30 years of age, who had visited Indonesia and Thailand, respectively, during the summer, presented with diarrhoea, headache, fever and later neurological symptoms. The first patient had to be sedated because of restlessness and was admitted to the intensive-care unit for intravenous antimicrobial therapy; the second became comatose and received intravenous rehydration and antipyretics. No diagnosis was made during the acute phase ofthe illness, but later there was serological evidence of Japanese encephalitis. Both patients recovered, but memory and concentration difficultie

Japanese encephalitis, caused by the JE virus transmitted by mosquitoes, is the most common type of epidemic encephalitis in Asia. It is endemic in most of South and Southeast Asia, but the number of cases can vary greatly between areas. While many infections do not lead to disease, the symptomatic cases can be very severe and life-threatening. It mainly affects children, whereas adults are generally immune to the disease due to either being infected in childhood or receiving vaccination. However, individuals who are not immune, such as travelers from non-endemic countries, are susceptible to
