Thirty-nine guinea pigs were used for four groups of experiments: 1. sacculotomy only, 2. sacculotomy and simultaneous obliteration of the endolymphatic duct, 3. sacculotomy followed by obliteration of the endolymphatic duct, and 4. obliteration of the endolymphatic duct followed by sacculotomy. Sacculotomy alone caused only minimal cochlear pathology, whereas sacculotomy on hydropic ears produced severe atrophy of the organ of Corti and cochlear neurons as well as connective cells of the limbus. There was histological evidence that Reissner's membrane in hydropic ears was ruptured by the sacc

BACKGROUND: Endolymphatic hydrops (EH) has been observed in both animal and human cochleae following cochlear implant (CI) surgery. We tested whether EH could be eliminated by administration of mineralocorticoid steroid antagonist spironolactone and explored the electrophysiological consequences of this.
METHODS: Sixty-four adult guinea pigs underwent cochlear implantation with a dummy electrode. Animals then survived either 2, 7, or 28 days. Auditory function was monitored by recording electrocochleography from the round window membrane preimplantation, and on the last day of the experiment.

OBJECTIVES: The mechanistic association between endolymphatic hydrops (ELH) and hearing loss (HL) is unclear. Although ELH severity has been shown to correlate in some studies with HL, injury of vital structures, including hair cells and the cochlear nerve, have failed to demonstrate correlation with ELH severity. The goal of this study is to evaluate the hypothesis that spiral ganglion cell degeneration is the principle pathologic site of ELH-related cochlear injury, correlates with ELH severity, and is most profound in the apical region.
STUDY DESIGN: Surgical induction of ELH in the guinea

Delayed endolymphatic hydrops (DEH) is a unique disorder characterized by fluctuating otologic symptoms in the setting of preexisting unilateral deafness. The symptoms include aural fullness, fluctuating hearing, and/or episodes of vertigo similar to those observed in Meniere disease and may occur ipsilateral or contralateral to the previously deafened ear. In most reported cases, the unilateral deafness has been a profound sensorineural hearing loss with a sudden onset that has been variously attributed to bacterial or viral labyrinthitis, acoustic or cranial trauma, otosclerosis, and congeni

Endolymphatic hydrops of the inner ear is identified as a swelling of the endolymphatic spaces. This morphopathology in man can only be confirmed at post-mortem examination although it is believed to underlie the auditory dysfunction and vestibular disturbances associated with Menière's disease. This is an illusive inner ear disorder characterized typically by the fluctuant hearing loss, tinnitus and episodes of vertigo. Menière's disease remains a major problem in otorhinolaryngology since the cause of the disease is not known and various treatments are recommended, often with unsatisfactory

The object of the present study was to evaluate electrophysiologically the therapeutic effect of glycerol on the cochlear function of guinea pigs in which experimental hydrops had been surgically induced. Fifteen albino guinea pigs were used. Each animal was chronically implanted to follow the evolution of hearing. The experimental protocol considered 3 groups of guinea pigs (5 animals each). Group 1 received no drug treatment, while groups 2 and 3 were given glycerol orally at 0.75 and 0.50 g/kg of body weight once a day for 4 months. The audiograms of the animals treated with glycerol showed

This paper describes a method for simulating transient endolymphatic hydrops based on the generation of vasomotor disorders in the labyrinthine artery area. The method is based on a deep short-term chloroethyl freezing of the lateral surface of the animal surface in the projection of the spinal artery. In histological terms, hydrops is characterized by two factors: vascular lesions (hyperemia of the vascular stripe, venous congestion) and changes in physico-chemical parameters of labyrinthine fluids (edema of cell elements of the labyrinth, membrane deformation, precipitation of the granular-r

The inner ear possesses remarkably stable homeostatic mechanisms for the maintenance of the functional integrity of the inner ear end-organs. The inner ear fluid maintains its homeostasis by a variety of subtle regulatory mechanisms, both locally and systemically. Any disturbance in one of these mechanisms can induce the disruption of homeostasis expressed by ionic, osmotic, or metabolic imbalance between the compartments. This can be manifested as membrane displacement or abnormal functions of the inner ear, depending on the degree of the disturbance. Further studies are necessary to clarify

IgG antibody and complement are both present in endolymphatic fluid, and it is possible that immune complex may form in endolymphatic fluid. However, there are still obscure points as to whether endolymphatic hydrops is due to immune complex, but it is quite possible that it is only one causative factor in endolymphatic hydrops.

BACKGROUND: Both vestibular schwannoma (VS) and Meniere's disease (MD) patients underwent hydrops MRI to clarify the relationship between VS and endolymphatic hydrops (EH).
METHODS: Eighty patients with VS or MD underwent an inner ear test battery followed by hydrops MRI, and were then divided into 3 groups. Group A comprised 58 MD patients (62 ears) with positive EH but negative VS. Group B included 18 VS patients (18 ears) with negative EH, while Group C consisted of 4 patients (4 ears) who had VS concomitant with EH. Another 14 MD patients who tested negative for EH on hydrops MRI were ini

BACKGROUND: Spontaneous intracranial hypotension may be associated with neuro-otological symptoms that might mimic Menière's disease.
CASE PRESENTATION: We report the case of a 53-year-old male presenting bi-frontal headache with recurrent spells of vertigo, left fluctuating hearing loss, and tinnitus. Dedicated brain and inner ear Magnetic Resonance Imaging, including a post-contrast 4 hours delayed FLAIR sequence, revealed typical signs of spontaneous intracranial hypotension associated with endolymphatic hydrops involving the left saccule and cochlea.
CONCLUSIONS: Audio vestibular manifes

Four-hour delayed three-dimensional fluid-attenuated inversion recovery (3D-FLAIR) sequence after intravenous gadolinium-based contrast agent administration is an optimal magnetic resonance imaging technique to evaluate endolymphatic hydrops in patients with known or suspected Ménière's disease. Nonenhanced endolymphatic space surrounded by enhanced perilymphatic space is evaluated in the cochlea and vestibule separately. In cochlear hydrops, the scala media is enlarged, potentially obliterating the scala vestibuli. In vestibular hydrops, the size of the saccule becomes equal to or larger than

An endolymphatic hydrops was induced in the left ear of each of 32 guinea pigs by obliteration of the endolymphatic sac. Both the hydropic ear and the control ear were examined by electrocochleography 1, 2, 4 or 8 months after obliteration of the sac using electrodes on the apex and near the round window. One month after obliteration, the threshold of the compound action potential (AP) and the increase of the AP amplitude with sound pressure level (SPL) were the same as the results recorded from the control ears despite the presence of a histologically confirmed hydrops. In the 2-month group,

Three pairs of temporal bones with otologic syphilis were studied histopathologically to detect obliteration of the endolymphatic duct and the associated hydrops. All cases showed infiltration and obliteration of the endolymphatic duct by round cells, giant cells, and fibrous tissue. These findings may be related to failure of medical treatment and sac surgery to arrest or alleviate symptoms of advanced syphilis.

A 38-year-old woman had suffered from recurrent vertigo, deafness and tinnitus for 2 years. After treatment for Meniere's disease, her symptoms did not improve. The threshold of pure tone audiometry was 68dBHL SNHL. High resolution CT of the temporal bone showed that a mass is behind the posterior semicircular canal. MRI showed that mixed signals are in the center of the mass and long T1T2 signal around the mass. The mass was surgically resected. Pathology report showed: endolymphatic sac tumor.
