Coronary Vein Exit and Carbon Dioxide Insufflation to Facilitate Subxiphoid Epicardial Access for Ventricular Mapping and Ablation: First Experience.

OBJECTIVES: This study assessed the feasibility of intentional coronary venous perforation and exit with subsequent pericardial carbon dioxide (CO2) insufflation as a novel method for assisting subxiphoid pericardial puncture in the setting of epicardial mapping and ablation for ventricular tachycardia. The technique required that coronary venous perforation would not lead to significant bleeding. BACKGROUND: Widespread adoption of first-line endoepicardial ventricular tachycardia ablation has not been taken up because of the risk of lacerating coronary vessels and puncturing the right ventri…

Percutaneous epicardial access for mapping and ablation is feasible in patients with prior cardiac surgery, including coronary bypass surgery.

BACKGROUND: Prior cardiac surgery, especially the presence of coronary artery bypass grafts, is thought to preclude percutaneous epicardial access (EpiAcc) and, therefore, mapping and ablation. We evaluated the feasibility and safety of EpiAcc in patients with a prior cardiac operation. METHODS AND RESULTS: We retrospectively analyzed all patients who underwent EpiAcc for ablation for ventricular tachycardia or symptomatic premature ventricular complexes between 2004 and 2013 at Mayo Clinic, Rochester, MN. Of 162 patients who underwent EpiAcc, 18 had prior cardiac surgery (median age, 64 year

[Local features of electrophysiologic and epicardial mapping of posterior septal accessory pathways in patients with surgically corrected preexcitation syndrome].

Posterior septal accessory pathways were documented in 6 patients with preexcitation syndrome who underwent detailed electrophysiologic evaluation and epicardial mapping. All 6 patients were cured by surgical ablation of accessory atrioventricular pathway. After follow-up of 9 months on average, 6 patients remain cured. No patient had recurrent supraventricular tachycardia and none had persistent atrioventricular conduction disturbance. The results of electrophysiologic study and epicardial mapping showed local characteristics of posterior septal accessory pathways. Accurate localization of po…

Identification of interatrial epicardial connections between the right-sided pulmonary veins and right atrium using coherent map.

UNLABELLED: A 43-year-old man underwent circumferential pulmonary vein isolation (PVI) for persistent atrial fibrillation. Although first-pass circumferential PV antrum ablation was performed, complete PVI was not obtained. A gap map showed the site of earliest activation was the right-sided PV carina, which was the same site of breakthrough on the left atrium map before ablation. Using a coherent map enabled us easily and clearly to evaluate the breakthrough sites. To identify whether the conduction from the right PV carina connected to adjacent structures, an activation map was obtained duri

Coexistence of functional Kent and Mahaim-type tracts in the pre-excitation syndrome. Demonstration by catheter techniques and epicardial mapping.

An unusual patient with Ebstein's anomaly of the tricuspid valve and the Wolff-Parkinson-White syndrome is presented. Ventricular pre-excitation related to conduction over both a right posterior Kent bundle and Mahaim fibers coursing from the atrioventricular node to the right ventricle. Two types of supraventricular tachycardia were demonstrable. These were due to re-entry involving antegrade conduction over either the normal or Mahaim paths, and retrograde retrograde conduction via the Kent bundle. Surgical division of the Kent bundle has abolished the clinically debilitating arrhythmias.

Epicardial atrial mapping can predict elimination of chronic atrial fibrillation after the box pulmonary vein isolation during mitral valve surgery.

BACKGROUND: The pulmonary veins (PV) and posterior left atrium (LA) may contribute to the occurrence and maintenance of atrial fibrillation (AF). We evaluated whether simple epicardial electrophysiological mapping can predict elimination of chronic AF after the box PV isolation procedure. METHODS AND RESULTS: Using a computerized 48-channel mapping system, we performed intraoperative atrial mapping in 16 patients with chronic AF associated with mitral valve (MV) disease. Patients' ages ranged from 48 to 76 years (mean, 61.4 years). AF duration ranged from 1 to 16 years (mean, 7.5±5.4 years).

High-resolution 8-spline mapping and radiofrequency catheter ablation for middle cardiac vein diverticulum epicardial accessory pathways: Efficacy and safety.

UNLABELLED: Posteroseptal accessory pathways (APs) often involve epicardial connections, making radiofrequency catheter ablation (RFA) particularly challenging. We present the case of a 14-year-old boy in whom a heart murmur was detected after birth. Echocardiography revealed an isolated ductus arteriosus aneurysm with tricuspid regurgitation, while electrocardiogram (ECG) showed pre-excitation due to an AP. The aneurysm spontaneously closed during infancy. At age 13 years, the patient experienced brief monthly palpitations but did not seek medical treatment. By age 14 years, echocardiography

Epicardial and Endocardial Ablation Based on Channel Mapping in Patients With Ventricular Tachycardia and Chronic Chagasic Cardiomyopathy: Importance of Late Potential Mapping During Sinus Rhythm to Recognize the Critical Substrate.

Background Ventricular tachycardia (VT) in patients with chronic chagasic cardiomyopathy (CCC) is associated with considerable morbidity and mortality. Catheter ablation of VT in patients with CCC is very complex and challenging. The main goal of this work was to assess the efficacy of VT catheter ablation guided by late potentials (LPs) in patients with CCC. Methods and Results Seventeen consecutive patients with refractory VT and CCC were prospectively included in the study. Combined endo-epicardial voltage and late activation mapping were obtained during baseline rhythm to define scarred an

Left atrial appendage as a vantage point for mapping and ablating premature ventricular contractions originating in the epicardial left ventricular summit.

Idiopathic ventricular tachycardia arising from the LV summit epicardial area can be successfully mapped and possibly ablated from the left atrial appendage.

A case of endocardial mapping for the epicardial pure origin of premature ventricular complex using the time difference between bipolar and unipolar potential.

In endocardial mapping of premature ventricular complexes of intramural or epicardial origin, we must be aware that pure origin and exit do not always coincide. In this study, we describe a case of endocardial mapping for epicardial-side premature ventricular complexes of pure origin using the time difference between bipolar and unipolar potentials.

High-density mapping and ablation of an incessant ventricular tachycardia with endo-epicardial involvement in a patient with situs inversus totalis and left ventricular noncompaction.

The association of situs inversus totalis and left ventricular noncompaction is very rare and poses several and unique challenges if endo-epicardial ablation has to be performed, both for anatomical access to the target area and for arrhythmia complexity. We report a case of incessant ventricular tachycardia with endo-epicardial involvement that required ablation in both surfaces to obtain final noninducibility.

Epicardial right atrial free wall mapping in chronic atrial fibrillation. Documentation of repetitive activation with a focal spread--a hitherto unrecognised phenomenon in man.

BACKGROUND: Previous studies have shown that atrial fibrillation of recent onset in man is based on a varying number of simultaneously present activation waves reentering either themselves or each other. In the present study, right atrial activation during chronic atrial fibrillation in man was studied. METHODS AND RESULTS: In 16 patients with chronic atrial fibrillation multiple epicardial recordings of 8 s each were made at the right atrial posterior free wall and at the appendage using a 20 x 35 mm electrode array with 56 bipolar measurement points. The preferable activation pattern of eac
