Epidermal inclusion cyst (EIC) is a recognized cause of an anterior neck mass in children. Controversy exists as to the proper surgical management of an anterior neck EIC: is simple excision adequate treatment, or is a Sistrunk procedure necessary? A retrospective review of the operative logs of the two senior authors (M.M.A., R.F.W.) from 1993 to the present revealed 16 children, ages 6 months to 9 years (mean, 4.5 years), with a diagnosis of anterior neck EIC. An accurate intraoperative diagnosis of an EIC in all cases allowed for a simple excision of the mass rather than a Sistrunk procedur

The aim of this study was to determine whether bacterial infection plays a significant role in the inflammatory process of epidermal cysts. Samples from 152 patients (115 cases of inflamed and 37 of uninflamed epidermal cysts) were subjected to aerobic and anaerobic bacterial culture and the isolates were investigated. The rate of bacterial growth and the recovered anaerobes were significantly greater in the inflamed than the uninflamed epidermal cysts. Specimens from 231 epidermal cyst abscesses were inoculated on media supportive for growth of aerobic and anaerobic bacteria. Of these, 192 yielded bacterial growth. Aerobic or facultative bacteria only were recovered in 84 specimens (44%), anaerobic bacteria only in 57 specimens (30%), and mixed aerobic and anaerobic bacteria in 51 specimens (26%). A total of 315 isolates (162 anaerobes and 153 aerobes) were recovered. The authors describe six cases of epidermoid cysts of the cerebellopontine angle. They insist on the progressive development, the variable duration and the polymorphism of the symptomatology. The imaging techniques relevant to diagnosis are reviewed. Among these computerized cisternotomography and N.M.R. are the methods of choice. It is suggested that surgical removal should be total whenever possible because of the risk of recurrence of these cysts but with great caution not to cause a vital or functional damage of the brain stem.

PURPOSE: An epidermal cyst is one of the most common benign tumors of the skin and its major treatment method is excision. Surgical excision is treatment of choice for epidermal cysts, but the cyst wall may be ruptured during operation, especially when the cyst wall is relatively thin. The incomplete excision of the cyst sometimes occurs because of a rupture of the cyst and incomplete excision may cause recurrence.
