During a 6-year period, 312 boys (mean age 4.7 years) underwent orchidopexy for cryptorchidism. A total of 388 epididymides were examined and the configurations were recorded as a prospective trial. This study consists of 166 ectopic testes and 222 undescended testes. The observed configurations of epididymides in the ectopic testes were: type 1a in 99 cases (59.6%), type 1b in 35 (21.1%), type 2 in 24 (14.5%), type 3 in 7 (4.2%), and type 4 in 1 (0.6%). We present a case of papillary cystadenoma of the right epididymis in a 52-year-old man. He visited our hospital complaining of painless mass in his right scrotum. This painless hard mass was palpable at the head of his right epididymis and was 1 cm in diameter. Right epididymectomy was performed. Ureaplasma urealyticum organisms (ureaplasmas) were isolated from the urethra and epididymal aspirate of a man aged 24 who had acute right sided epididymitis. No other microorganisms were detected, and he had no chlamydial antibody response. A fourfold antibody response to the epididymal ureaplasma isolate was detected by two methods, however, and the patient responded clinically to doxycycline, to which the ureaplasmal isolates were susceptible in vitro. These findings suggest that U urealyticum had a causative role.

The types and incidence of deformities in the proximal seminal pathways have been carefully studied in 150 consecutive orchidopexies. 85% of the testicles were found in the subcutaneous tissue, 15% were in the canal or the abdomen. Malunion between the testis and the epididymis was the type of anomaly most frequently encountered. Deformities considered to affect fertility were seen in 70% of the gonads located proximal to the external inguinal ring and in 12% of the subcutaneous testicles. The difference is statistically significant.

In the present study the lipid composition of human epididymidis is reported. The total lipids represented 1.25 per cent of the total wet weight of the tissue. The phospholipids formed 64 per cent, cholesterol 12 per cent and glycerides 24 per cent of the total lipids. In patients with obstruction of efferent ducts, the total lipids were increased due to the significant in creases observed in glycerides and cholesterol of the epididymidis. There was a marked fall in epididymal phospholipids due to a fall in phosphatidyl choline and phosphatidyl ethanolamine.

BACKGROUND: During the post-testicular maturation that occurs in the epididymis, spermatozoa need to face biochemical and morphological changes that may make them vulnerable to oxidative damage. Benign smooth muscle cell proliferation commonly involves a variety of body organs, yet is a rare finding in the spermatic cord or paratesticular tissue of the male genital tract. Here we discuss a case of smooth muscle hyperplasia of the epididymis which presented as an intrascrotal mass. This is a very rare condition and should be considered in the differential diagnosis of any paratesticular mass.

The authors report the case of a 44-year-old man in whom a poorly differentiated primary carcinoma of the head of the epididymis was discovered incidentally. Due to the rarity of this diagnosis, a comprehensive assessment was performed looking for a primary tumour, but without success.
