Slipped capital femoral epiphyses in a patient with primary hyperparathyroidism.

Slipped capital femoral epiphyses are rarely associated with primary hyperparathyroidism, and only four cases have been documented in the English literature. We report such a case. A 13-year-old boy presented with bilateral slipped capital femoral epiphyses. Investigation showed that he had primary hyperparathyroidism due to a parathyroid adenoma. The adenoma was removed, and then the epiphyses were pinned in situ 3 weeks later. The pins were removed 3 months postoperatively when the physes had closed. The patient had a painless hip 1 year later at follow-up. We concluded that identification a…

Physeal remodeling after internal fixation of slipped capital femoral epiphyses.

Rounding off the femoral neck metaphysis helps improve hip function in slipped capital femoral epiphysis (SCFE). Remodeling by physeal growth, not previously reported, may also contribute. By studing the changes in physeal-shaft angle after internal fixation (IF), we found evidence of physeal remodeling in one of 21 mild, five of 23 moderate, and in all of 11 severe slips. Although the amount of remodeling averaged 11.7 degrees, it occurred in 100% of severe slips (p less than 0.001). This support the use of initial pin fixation and a wait of at least 2 years before realignment femoral neck os

In vivo determination of long bone flexibility--the epiphyses influence.

A lot of studies are devoted to the in vivo vibration analysis of long bones. In general they compare mechanical parameters relative to two instrumented bones or to one bone considered in different configurations. In this study a method of E. I product numerical determination is presented including for the first time the epiphysis influence in the calculation. The calculation of the maximum bending yield limit of the bone is carried out using this parameter. A method is proposed to determine the ultimate bending moment and the bone in vivo rigidity.

[Incidence and significance of epiphyseal and para-epiphyseal injuries in replantations in the area of the upper extremity in children].

A series of 15 cases of replantation in children is reviewed with special attention to the frequency and the implications of epiphyseal lesions. Definite conclusions, however, will only be possible after these patients reach maturity.

Micromelic dwarfism with cone epiphyses, metaphyseal dysplasia, and vertebral segmentation defects.

We present the clinical and radiological findings in a newborn male with severe micromelic dwarfism, short neck, short and narrow upper thorax, and brachydactyly. At the age of 1 year mental development is slightly retarded. The X-ray findings of severe vertebral segmentation defects and a generalized metaphyseal skeletal dysplasia did not lead to a final, conclusive diagnosis. The present patient may be the first example of a new type of micromelic spondylo-epi-metaphyseal dysplasias.

All-epiphyseal versus trans-epiphyseal screw fixation for tillaux fractures: Does it matter?

BACKGROUND: Tillaux fractures occur primarily in adolescents due to the pattern of physeal closure and are classified as Salter-Harris type III physeal fractures. Operative management with screw fixation is recommended for more than 2 mm of displacement or more than 1 mm of translation. However, the efficacy and complications of trans-physeal vs all-physeal screw fixation have not been investigated extensively. AIM: To compare the clinical and functional outcomes of trans-physeal (oblique) and all-epiphyseal (parallel) screw fixation in management of Tillaux fractures among pediatric patients

Bilateral epiphyseal migration following fixation for slipped capital femoral epiphyses in a hypothyroid child.

Progression of slipped capital femoral epiphysis following in situ screw fixation typically occurs through loosening of the screw in the metaphysis. Epiphyseal migration off the screw due to physeal growth is rare. We report epiphyseal migration off bilateral screws in a child undergoing thyroid replacement therapy. Patients with mild and moderate slipped capital femoral epiphysis and endocrine disease should be followed-up with radiographs taken at intervals which reflect the rate of growth. Fixation should be revised if the tip of the screw approaches the physis and initial fixation with two
